Deferred Failure Risk
Vehicle-specific risks from this model-year's complaint record, known defects, and service milestones
Owners of the 2016 FORD F-150 report engine/powertrain issues more than most other systems. Complaints concentrated in one area often signal a systemic weakness worth a targeted pre-purchase inspection.
Worn chain tensioners cause timing chain skip or snap — common on GM 5.3L/6.2L, Ford EcoBoost, VW/Audi EA888, BMW N47/N63. Listen for cold-start rattles.
Water pump failure causes rapid overheating. If the engine overheats even once, head gasket damage is likely — turning a $400 part into a $3,000+ repair.
Degraded transmission fluid destroys clutch packs and solenoids. CVT transmissions are especially sensitive — neglected fluid can result in a $4,000–$8,000 replacement.
On 4WD/AWD vehicles, neglected differential or transfer case fluid causes metal-on-metal wear. Replacement costs $1,500–$4,000.
Degraded coolant loses corrosion inhibitors and attacks aluminum engine components. Can cause head gasket failure and water pump corrosion.
Worn spark plugs cause misfires that can foul and crack catalytic converters. A $150 tune-up can prevent a $1,200 catalytic converter replacement.
Dirty or failing injectors cause rough idle, reduced fuel economy, and increased emissions. Cleaning costs $100–$200; replacement $800–$1,600.

What Happened MY 2016 Ford F-150 Platinum, equipped with a rear/360-degree camera system, has failed and displays a blue screen instead of providing a usable rearward image. This blue screen was confirmed by the Lugoff Ford Service Department (authorized Ford dealership) around June/July 2026. What I did to correct the problem I took the 2016 F150 Platinum to my local authorized Ford dealership (Lugoff Ford, Lugoff, SC). The Lugoff Ford dealership inspected the vehicle, confirmed the blue-screen rear camera failure, and determined that the camera requires replacement under the Ford Customer Satisfaction Program 25N17. The repair work would be in October 2026. Repair status In September 2026, I got an oil change service at Lugoff Ford. I asked about the status of the recall for the inoperable backup camera and was told that parts and an advanced software update have been changed and will now be available in February 2027. Safety As you are aware, an inoperable backup camera is a safety concern because it does not provide the driver with the rearward view expected from the vehicle's factory-installed camera system. When reversing, the driver cannot rely on the camera to identify pedestrians, vehicles, objects, or other hazards located behind the big F150 Platinum. Manufacturer Contacted I contacted Ford Motor Company's Customer Relationship Center twice to request assistance with an expedited repair, a formal Ford case, and/or a dealer-to-dealer parts search. I was unable to obtain a formal case or an escalation for an expedited remedy. Legal Complaint I am filing this complaint because the safety-related camera failure has been diagnosed, but the manufacturer has not provided an available remedy, and I have been given an estimated repair date of February 2027. I am requesting that NHTSA document this complaint and review the safety implications of an inoperative rear-view camera and the extended delay in obtaining the replacement remedy.
2016 Ford F-150, 5.0L, 6R80 transmission, 112,083 miles. Safety Recall 26S28 was performed on 08/05/26 at an authorized Ford dealer. The wrench/powertrain warning recurred on 08/15/26. On 08/20/26, at 111,998 miles, the dealer found DTCs P0720 and P0722 (output shaft speed sensor) and stated in writing that the transmission molded leadframe must be replaced. Ford denied coverage under Customer Satisfaction Program 26N04 because those codes are not on the program's list. The transmission then failed on two consecutive days, within about 85 miles of driving since the dealer visit. On 09/23/26, while I was accelerating across two lanes of traffic, the engine revved abnormally high during an upshift, as if the transmission failed to engage a gear. It then stayed in 3rd, and I had to make a left turn from a stop at an intersection in 3rd gear with greatly reduced power. On 09/24/26, after crossing a four-lane highway intersection, the warning appeared while I was accelerating uphill. The transmission stayed in 3rd at a complete stop and would not downshift to 1st. Shifting through Park and using manual mode did not restore 1st gear; only an engine restart did. In both events the warning light cleared itself while the fault continued, so the dashboard gave no indication of the problem. Turning left across oncoming traffic and pulling away from stops near highway traffic with reduced power is a safety risk. The recall remedy did not prevent this failure, and the repair the dealer says is required is being denied under the related program.
I am reporting that the Corporate Ford Company is knowingly requiring a software update that minimizes a safety hazard instead of eliminating a safety hazard (NHTSA Recall 26V237). Here are the facts: ---Lebanon Ford service said that they cannot tell me what the software update 26S28 will do to fix the danger of driving my vehicle. The mechanic (Michael) said that the software update is meant to fix the transmission molded lead frame, but that it does not. My research about this software update states that the truck will unexpectedly downshift to 4th gear (instead of downshifting to 1st gear without the update). This downshifting issue poses a danger to people driving on the highway! Consider trucks suddenly downshifting to 4th gear in front of you on the highway (some towing)! Ford Recall: 26N04 is a fix that will eliminate danger, but Ford is NOT fully implementing it! Molded Lead Frame Failure The Issue: The internal wiring harness (the lead frame) holds the speed and temperature sensors. When it fails, it sends bad data to the computer. Symptoms: Erratic shifting, a wrench light on the dash, or a check engine light with speed sensor codes. Unexpected Downshifts (Recall 26V-237) --THIS IS THE SOFTWARE UPDATE THAT DOES NOT SOLVE THE DANGEROUS RISK. On XXX, Ford did NOT complete the Recall 26N04 (Ford #), only the NHTSA 26V237 software update, which does not eliminate dangerous driving on the highway. I am attaching the event information that I gave to Lebanon Ford at the time of service. ALTHOUGH THERE WAS NO CRASH AND NO CASUALTIES, I CANNOT, IN GOOD CONSCIENCE, DRIVE MY 2016 FORD F-150 ON THE HIGHWAY BECAUSE FORD IS ONLY LESSENING THE SYMPTOM, WHICH IS STILL A DANGER TO PEOPLE. FORD CAN FIX THE PROBLEM TO ELIMINATE DANGER, BUT THEY CHOOSE NOT TO DO SO.
I am filing an emergency complaint against Ford Motor Company for failure to provide a safety recall remedy within a reasonable timeframe under 49 U.S.C. Chapter 301. My vehicle has an active safety recall (Recall 20S31) for a compromised brake master cylinder, making it entirely unsafe to operate. The vehicle is currently grounded and inoperable at my residence. Ford Corporate has kept the required repair parts on a national backorder for over 3 months. On August 18th, Ford Corporate sent me a formal written denial explicitly refusing to provide a rental vehicle or alternative transportation while my truck sits stranded. Ford is forcing me to choose between driving a vehicle with known catastrophic braking failure or having no transportation, actively violating federal recall compliance guidelines.
XXX: I scheduled an oil change online and selected the open transmission safety recall. [XXX] Ford executed Recall 26S28 (PCM Programming) under verification code RGLKT7MASIPIJ. The truck had absolutely no shifting issues prior to this flash. XXX: Within two weeks of the software update, the truck began experiencing the exact erratic shifting and violent gear-dropping safety hazards described in the recall. I returned the truck to the dealer. XXX: After days of complete silence, I had to initiate contact. First by phone, then by physically walking into the service desk on XXX to get answers because the dealer ignored my vehicle. Mid-XXX: After the truck sat on their lot for three weeks, they finally scanned it and retrieved code P2701. The dealer stated the recall wouldn't cover it and immediately quoted me $8,000 to rebuild or $10,000 to replace the transmission. They claimed they would submit the case to Ford Corporate. XXX: Having heard nothing, I returned to the dealership in person. I was told they still had no response from corporate. The Paperwork Contradiction: The technician's notes on Page 3 explicitly state: "No update so that one recall cannot be performed, must been updated at some point." This proves the technician failed to look at their own shop's service history from XXX (Page 2), where they signed off on completing the update. Furthermore, they condemned the entire transmission based strictly on a digital code scan; they never dropped the pan or performed any physical diagnostic work. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
The component that failed is the Back Up Camera Screen. It is all blue. It is available for inspection upon request. My safety or the safety of others is at risk because I can't see directly behind my truck without this camera. I could end up hitting a child or small item directly behind my truck. We will be contacting the dealer about this problem. My vehicle has not been inspected by the manufacturer, police, insurance or others at this time. No warning lamps, messages or other symptoms of this problem occured prior to the failure. This problem first occured on Saturday, September 19, 2026 I don't know what the cause of this problem is.
I participated in Safety Recall 26V237 -2016 F-150 Ford Truck transmission issues. Under the recall, Ford completed a transmission programming update to resolve the safety concerns. The campaign DID NOT resolve the transmission issues. The truck is still a hazzard to drive & needs further repairs or transmission replacement. Please do not allow Ford to remain unaccountable for the failure to correct the issue.
There is currently a recall on 4 of the sensors on the same rail n the transmission due to it locking in 3rd or 5th gear. When I took the truck in they pulled the codes and it was not one of the 4 so Ford does not have an active recall. There was another person in line in front of me who had the same issue on the same two codes.
The contact owns a 2016 Ford F-150. The contact stated that while driving 65 MPH, the transmission unexpectedly downshifted, and the vehicle lost motive power and decelerated. Additionally, there was an abnormally loud sound coming from the vehicle with the power train warning light illuminated. The dealer was contacted by phone and made aware of the failure. The vehicle was taken to the dealer where the failure was confirmed. The contact was informed that an unknown PCM software update was completed. The vehicle was repaired but the failure reoccurred. The contact referenced an unknown recall; however, the VIN was not associated with a recall related to the failure. The contact stated that the vehicle was unsafe to drive. The manufacturer was contacted by phone and made aware of the failure. The failure mileage was approximately 100,000.
Driving down Highway 6/34. Cruise set at 65 mph. The truck on its own Shifted into 2nd gear and revving up the engine caused the vehicle to slow uncontrollably, which is very unsafe driving. The 2016 has a safety recall to prevent this from happening. I am writing this to report that my truck has had the recall done, and this still happened. I will park the truck until Ford can fix it. But they are trying to get out of fixing this safety issue with all these trucks. At 65 mph, I lost control of the truck, and thank goodness there were no other vehicles on the road around me
Performance Ford in Utah falsified a federal safety recall completion for Recall 26S28/19V446000 on my 2016 F-150. They only performed a software flash, manually wiped the transmission's safety fault codes from the memory cache, and joyrode my vehicle for 200 unauthorized miles. They returned the vehicle to me unrepaired, where it immediately suffered an erratic downshifting hazard and engine limp mode, endangering my safety.
Ford mailed me a XXX Customer Satisfaction Program 26N04 letter for my 2016 F-150, VIN [XXX]. That letter offered a one-time, no-charge replacement of the transmission molded lead frame if the dealer determined replacement was needed, and it described a malfunction indicator lamp and/or wrench light when that part fails. My truck did exactly that. It stored P0715 and P0717, went into limp mode, and the wrench came on. Titus-Will Ford in Tacoma confirmed the turbine speed sensor is part of the lead frame and replaced it (part AL3Z-7G276-D). They denied 26N04 because those codes are not P0705, P0706, P0707, or P1702 — a code list the customer letter never disclosed. The failed part is the part named in your letter. The symptoms are the symptoms named in your letter. I paid invoice XXX dated XXX. The lead-frame line was $1,630.02. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
The body control module was destroyed when water leaked on it. Apparently there is a failed windshield seal(maybe) that causes leaking down the A pillar. That pillar leads the water directly to the wiring harness for the BCM. When my BCM went out, wipers, blinkers, external lights, speedo/tach, power windows (in the front), all HUD, all stereo, and I think the security system too, stopped working. It was raining hard, it was a very dark day/difficult to see. I and others could easily have been killed. VERY, VERY DANGEROUS! Why the BCM isn’t in a sealed box, is mind blowing. The fact that the computer to my truck can get wet at all is nothing short of asinine. The vehicle was inspected by ICDI Diesel in [XXX] [XXX]). The part is currently on order and isn’t expected to be delivered until XXX. The truck is available for inspection. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
Powertrain recall stated because the code didn’t show on a SAFETY RECALL that can cause vehicle to shift from 5th to 1st gear while driving per stated recall Ford will not honor because it’s not one of the codes they state but the recall doesn’t specify a code. We had it at the dealership twice and they said they would fix it for $ 1400.00 well we didn’t cause the recall and we should not be responsible for paying. Would just like someone to contact us before my husband or someone else gets hurt in an accident.
The vehicle experienced a sudden and unexpected loss of power steering assistance while being driven on a highway. The failure made the vehicle extremely difficult to steer and nearly caused an accident, creating a serious safety hazard. After pulling over to get off the highway, it was observed at lower speeds, the vehicle did not have its full range of steering also creating a safety hazard. The problem was confirmed by the dealer, who determined that the power steering gear system had failed. The dealer replaced and repaired the entire power steering system. No warning lights or other alerts appeared before the failure. The sudden loss of steering assistance occurred without warning while the vehicle was in motion, significantly increasing the risk of a crash.
The contact owns a 2016 Ford F-150. The contact stated that while attempting to depress the horn pad on the steering wheel, the horn failed to provide an audible sound as intended. The air bag warning light illuminated. The vehicle was taken to a local dealer where it was diagnosed and determined that the clockspring needed to be replaced. The manufacturer was made aware of the failure. The failure mileage was approximately 39,825.
The contact owns a 2016 Ford F-150. The contact stated that while driving 70 MPH, the transmission unexpectedly downshifted to 3rd gear. The contact received notification of NHTSA Campaign Number: 26V237000 (Power Train); however, the dealer and manufacturer stated that the VIN was not included in the recall, and the fault code listed in the recall was not retrieved from the vehicle. The contact stated that after the vehicle skidded on the roadway after the unexpected downshift, there was an orange WRENCH light illuminated on the instrument panel. The manufacturer was notified of the failure. The vehicle was not repaired.
In April of this year my 2016 F150 began having issues related to the automatic transmission. Symptoms included unexpected and irregular downshifting from 6th gear into 4th gear while traveling on the highway, random and irregular shifting from gear to gear, and not gearing down below 3rd gear when coming to a complete stop. While these symptoms were happening the truck displayed the MIL and powertrain warning lamp. These issues put myself and others at risk because having the vehicle unexpectedly downshift while traveling at a high speed causes the vehicle to become difficult to control, increasing the risk of a crash. It is also difficult for the vehicle to increase speed in a timely and safe manner when it is starting out in 3rd gear. In May I received a recall notice from Ford, eventually in July having the dealer closest to me perform a PCM software upgrade. While waiting for Ford to make this software upgrade available I received a notice from Ford referencing Customer Satisfaction Program 26N04. This notice states that if my vehicle set a MIL and/or powertrain warning light (which my vehicle did both) I may need to get the transmission molded leadframe replaced as part of this recall program. When I picked up my vehicle from servicing dealership I was informed my vehicle "did not have the appropriate code" to warrant this repair. However, the software upgrade performed has done nothing to fix the problem I am experiencing. My vehicle still downshifts randomly and unexpectedly while displaying a powertrain warning light, and making my vehicle unsafe for highway travel.
The contact owns a 2016 Ford F-150. The contact received an unknown recall notification. The dealer was contacted by phone and scheduled an appointment for a technician to pick up the vehicle. The technician drove the vehicle to the dealer. The unknown recall repair was completed. The contact received a phone call from the dealer and was informed that while the technician was in route and driving the vehicle back to the residence, the transmission independently shifted from fourth gear to first gear. The technician drove the vehicle to the dealer. The vehicle was with the dealer for two weeks. The contact received a phone call and was informed that no warning lights were displayed, and the failure could not be duplicated. The contact was informed to pick up the vehicle. The dealer informed the contact that there was no fix available. The contact was informed that the dealer would not be offering any additional repairs. The contact stated that the vehicle had no issues prior to the unknown recall repair. The failure mileage was approximately 128,000.
The contact owns a 2016 Ford F-150. The contact stated that vehicle was taken to the local dealer for a recall repair associated with the PCM reprogramming. Upon retrieval, the vehicle went into LIMP Mode while driving. The contact stated that while driving at an undisclosed speed, the transmission unexpectedly downshifted to a lower gear, and the vehicle lost motive power. The dealer was contacted and provided the contact with an estimate for a diagnostic test. The contact spoke with a supervisor, but no additional assistance was provided.
The contact owns a 2016 Ford F-150. The contact stated that the vehicle was repaired under an undisclosed recall; however, the recall repair failed to prevent a failure. The contact stated that while driving at an undisclosed speed, the vehicle hesitated while upshifting or downshifting between 4th and 6th gear. The same local dealer was contacted and advised the contact that the 4th and 6th clutches were burned. The manufacturer was made aware of the failure. The failure mileage was approximately 249,000.
I would like to file complaint against Ford for their latest recall (26N041) and how they are not doing replacing the defective Lead Frame in customers vehicles. It's a big scam and a waste of time. I brought my 2016 Ford F-150 in for Ford’s latest recall (26N04) affecting 2015–2017 models due to the transmission leadframe issue. I explained that I’ve been dealing with an intermittent problem for the past seven months: while driving, the gear indicator lights on the dash begin to flicker, and when that happens, the transmission shifts extremely hard and slams into the next gear. Ford kept my truck for a week and told me they were unable to duplicate the issue. They stated that multiple technicians drove the vehicle and none of them observed any problems. When I arrived to pick it up, I was handed a bill for more than $250 for a diagnostic fee. Within four miles of leaving the dealership, the gear indicator lights began flickering again. It’s difficult to accept that several technicians drove the truck over multiple days and never encountered the issue I experienced almost immediately. The entire process felt like a waste of time and money, and I’m left without any resolution to a problem that clearly still exists. They are not honoring the recall, they are trying to charge customers for the repair.
Truck downshifts into low gear, yellow wrench and engine light come on and the truck goes into limp mode, no warnings or anything. These symptoms first started about 5 months ago and I have stopped driving the truck for that time. I took it to the dealer after looking it up and seeing there was a recall 26S28 for the molded leadframe. At the dealer they told me they couldn't perform the service because my truck was showing codes: P061A-00, P0720-00, and P0722-00 and they needed: P0705, P0706, P0707, P1702 to perform the recall service. My codes also point to the molded leadframe but the speed sensors instead of the range sensors.
XXX, my Ford F-150 began experiencing serious and unpredictable transmission shifting problems. The truck would downshift unexpectedly while traveling at various speeds. At times, it would not downshift properly when coming to a complete stop, and it would sometimes shift back up into third gear from a complete stop at a stoplight. On one occasion, while traveling approximately 65 mph, the truck unexpectedly downshifted. The sudden change in the vehicle's behavior was extremely unsettling and made me concerned about maintaining control. What made the situation even more frightening was that I had no center lane or safe area to pull over without potentially becoming a hazard to the traffic around me. I had nowhere to simply get out of the way and figure out what was happening. That is an extremely unsettling feeling when you are traveling at highway speeds. Another incident was even more frightening. While traveling approximately 55 mph on the highway, the truck unexpectedly downshifted while a logging truck was traveling directly behind me. Fortunately, I was able to maintain control of the vehicle and safely pull onto the shoulder. I contacted my local Ford dealer and was told that its a Software repair, was not yet available and that I would need to wait for Ford's software fix. Extremely concerned about continuing to drive a vehicle that was behaving this way, I contacted Ford directly and asked Is it safe to continue driving the vehicle? Response -Ford was not advising owners to discontinue operation of the vehicle. Took place on XXX, at approximately 8:11 a.m. with Ford representative [XXX]. On XXX, I bring the truck to the Ford dealership for the software update. I received the vehicle back the same day, expected the problem had finally been corrected. Like I never had the truck "software" fix. Back to dealership - they redid the software fix, not fixed. No codes, and still no fix for the transmission shifting. On August 21, 2026, my 2016 Ford F-150 was serviced at Five Star Ford to perform safety recall 26S28 (Powertrain Control Module software update). Immediately following this software flash, the transmission's shifting logic became severely corrupted and unsafe to drive. The vehicle currently acts as if it has completely lost 2nd gear, experiences a severe lag when shifting up into 6th gear, and repeatedly fails to lock itself into 3rd, 4th, or 5th gears. The dealership is currently using a temporary lack of hard Diagnostic Trouble Codes (DTCs) to deny coverage for an internal physical lead frame component replacement under the 26S28 extended warranty guidelines, claiming instead that a complete, out-of-pocket transmission replacement assembly is required. The safety update has directly induced a severe driving hazard, leaving the vehicle unable to properly find or maintain standard highway gears.
The contact owns a 2016 Ford F-150. The contact had received notification of NHTSA Campaign Number : 26V237000 (POWER TRAIN) and the vehicle was taken to a local dealer where it was remedied. The contact contact continued to drive the vehicle when the failure occurred while driving at 60 MPH the transmission downshifted from 5th gear into 3rd gear, the service vehicle soon warning light was illuminated. The vehicle was taken back to the dealer where it was diagnosed and the contact was advised that the molded plastic lead frame was faulty and would need to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 116,000.
The contact owns a 2016 Ford F-150. The contact stated that while driving at approximately 30 MPH, the vehicle hesitated to maintain acceleration while the accelerator pedal was depressed, which caused the vehicle to jerk forward into the rear bumper of another vehicle in traffic. The front bumper struck the rear bumper of the other vehicle. A warning message reading "Cannot Exceed 80 MPH" and an additional unknown warning message were displayed. The air bags did not deploy. No injuries were reported. A police report was not filed. The vehicle was taken to an independent mechanic, where the front bumper was replaced at the contact's expense. The vehicle was later repaired under NHTSA Campaign Number: 26V237000 (Powertrain); however, the repair failed to prevent the failure from recurring. The vehicle was taken back to the same dealer, where it was determined that the vehicle needed further diagnostic testing for a possible transmission repair. The vehicle was not repaired due to cost and remained at the dealer. The manufacturer was notified of the failure. The failure mileage was approximately 88,000.
The contact owns a 2016 Ford F-150. The contact stated that the vehicle was taken to a dealer to be serviced under NHTSA Campaign Number: 26V237000 (Power Train). The vehicle was not experiencing any failure prior to the recall repair; however, after the recall repair was completed, the transmission started downshifting roughly and would not shift beyond second or third gear, creating a severe road hazard while driving on the highway. The vehicle was driven to the contact's residence. The dealer was notified of the failure but informed the contact that the issue could not be resolved. The vehicle was not repaired. The manufacturer was notified of the failure and advised the contact to reach out to the dealer; however, the dealer was unwilling to assist. The failure mileage was 200,000.
Vehicle transmission downshift to a lower gear while traveling at city and highway speeds without any prior warning or driver input. Diagnostic showed codes P0715 and P0717. The following day we received notification from Ford of the recall "Transmission molded leadframe replacement" (Campaign 26N04)" - which was investigated by NHTSA as part of Part 573 Safety Recall Report 26V237. When we spoke to our local dealership in [XXX] they felt it was the same issues as the recall describes. However, when we submitted the vehicle for repairs, they reported that these specific diagnostic codes (P0715 and P0717) are NOT considered part of the recall and are therefore NOT covered by the recall. But the repair needed for our codes are the SAME remedy as recall indicates. So, we are left with out-of-pocket expenses of over $2,000 because the code is one digit off. We would like to confirm with the NHTSA if these codes are actually covered, and if not, WHY? Our vehicle is experiencing the exact same issues as reported in your evaluation and my [XXX] old son is not safe in this vehicle. "While operating my 2016 Ford F-150, the vehicle lost power-steering assist. Ray Skillman Ford diagnosed an internal failure of the power steering control module and replaced the steering gear assembly. The vehicle had approximately 94,620 miles. Sudden loss of steering assist significantly increased the effort required to steer the truck and created a safety concern. The repair cost me $3,804.27."
My truck at 45 mph suddenly downshifted to first gear. It locked my tires causing me to fish tail. Luckily I lost control beside a shoulder and not into a ditch or another vehicle. I was near my home and turned onto a country road and was able to drive 20 mph or less to get it home. It is due to a recall that currently has no clear repair. Ford is aware of this issue and has done next to nothing to assist me. I have no vehicle and have not had one for nearly a month. The service team manager called to tell me the parts were not available but are ordered. He also mentioned his mechanics were afraid to drive my truck from the lot to the garage because the truck is acting so erratically. There is no way this should not be addressed. I’ve only been asking for Ford to do the right thing. They’ve done nothing. It is dangerous I could have been killed in an accident because of this. My truck could have also fish tailed into oncoming traffic endangering others as well. I am not sure what else can be done. I’m out of options. Ford has refused to give me a loaner vehicle or pay for a rental due to company policy. I’m looking for any assistance at this point. I look forward to hearing your thoughts.
The contact owns a 2016 Ford F-150. The contact stated that while operating the vehicle, the vehicle lunged forward. Additionally, while attempting to slow down to stop the vehicle, the transmission unexpectedly downshifted while driving. The vehicle was taken to the local dealer and serviced under NHTSA Campaign Number: 26V237000 (POWER TRAIN). After retrieving the vehicle, the vehicle lunged forward and independently accelerated, and the transmission failed to properly shift into the intended gear. During the failures, the vehicle was restarted, and the failure was self-corrected. The cause of the failures was not yet determined. The manufacturer and local dealer were notified of the failures, but no assistance was offered. The contact was later sent a letter from the manufacturer stating that additional service to the powertrain leadframe might be needed after the recall remedy was performed. The contact was informed that the additional service was not covered under the recall. No additional information was available. The failure mileage was 148,000.
This vehicle 2016 ford F150 is known to have shorting issues. From literally all wiring harnesses. I had a small fire in my truck due to this issue, caused melting of the ribbon cable in my seat heater control module, which is a known issue from Ford. I have shorting in my turn signal switch (no wipers). Is that safe? I am a research scientist and engineer, I can tell you now it is not safe. If i need to prove it, I can do so with ease and write the necessary literature. If I cant use my AC sometimes, the windows fog, then i cant use the wipers to unfog them, how is this not a visibility issue? Not to mention all the shorts. I have a 40 gallon fuel tank, if any shorts get near it, is that going to be a fun experience for me? Please give Ford a penalty, the average owner of this truck pays thousands a year in repairs. I paid $17,800.00 in 2025 to repair this piece of shit. And it still has major issues. This is something ford needs to address, i want my wiring to be replaced, i bought a new engine, what if i cant use it especailly with the 3 year warranty bc the wiring consistently shorts and fails. unacceptable for a truck that's sticker price is $80k. This is a major problem, I will file claims until im afforded a response
Ford Motor sent notice 26s28 to me regarding a possible problem with my transmission/powertrain. On August 12 they reprogrammed my Powertrain Control Module. In the letter regarding this upgrade they also stated, and I quote: " If your dashboard displays a Malfunction Indicator Lamp(MIL)or Powertrain Warning Lamp(wrench symbol) either before or after the software update is installed, your dealer will run a diagnostic test. If specific diagnostic trouble codes relating to the transmission codes relating to the transmission are found, the dealer will replace the lead frame at no cost to you under Extended Warranty Program 26N04." The day after the software update my truck did for first time ever display a yellow wrench symbol. They said bring it back in and they'd run a diagnostic. They did and it showed a stored code P0717. This code relates directly to the Input/Turbine Speed Sensor. It is a sensor built directly into the lead frame mentioned earlier, which Ford said they'd replace at no cost under 26N04. But from Fords point of view, this is not one of their acceptable codes for free lead frame replacement. WHAT? It is built "directly into the lead frame, and can't be independently replaced." In essence, it IS the Leadframe. This code should obviously be included in the group of acceptable Ford codes as relating to this issue. My case number with Ford is CXH-08937978-D1N4V6. Thank you for your time in reviewing this situation.
I made an appointment for July 22,2026 with Purvis Ford in Fredericksburg to have the 28S26 recall for a transmission issue outlined in the recall. The process was estimated to be 4-6 hours. Purvis Ford called said the software upgrade was completed and i could get my truck. The truck would not shift out of 1st gear so i turned around and went back to the dealer and told the service advisor the situation. He told me i had to drive it for about two hundred miles to get it adapted to my driving habits. I drove it about 20 miles to my home and never shifted out of low gear. I called the service advisor and he said it needed to be towed back to the dealership for a second recall procedure. That was scheduled for the following Friday. On Thursday they called and said it needed a new transmission. The transmission was working fine when i delivered it to them. I did exact all what they told me to do. I’ve made several calls to Ford customer Service with no help from them. Monday the service manager told me it was going to be test driven by a factory technician and they would get back to me. It’s now Wednesday and no word on what the results are. I called this morning and was supposed to get called back but as of this hour nothing. What course of action do i have at this point.

The faulty panoramic sunroof is a known issue by Ford Motor Company. Yet they have done nothing to resolve it. The panoramic sunroof on the 2016 Ford F-150 is known to develop operational and structural problems, including sticking, binding, excessive wind noise, water leaks, and failure of the glass panels or sunroof mechanism to open and close properly. In some cases, the sunroof can become misaligned or the tracks and components can fail, resulting in expensive repairs and potential water intrusion into the vehicle. This recurring issue has been reported by numerous F-150 owners and can significantly affect the vehicle’s reliability, usability, and resale value.

I was at my granddaughters and I went to leave and it was like dead except the wrench came up in the dash panel , I waited before calling my husband and I went back to try and start it again and it started no wrench in the display or anything , this was in February 2026. In March of 2026 I was in the middle turn lane on [XXX] by Tuttle, Ok. And it stalled as I was trying to turn and would not move I put it back into park and then back to drive and it turned . Next incident was in April2026 and I was on [XXX] south of Mustang , Ok. And I was in the passing lane passing a vehicle going 65 mph and it just stalled and went down into 1 st gear I managed to get it to the shoulder of road an then it died it would not start but the radio and lights were still on and there was a wrench popped up in the panel . I waited for about 30 minutes and kept trying to start it after several attempts it started again and I made it home . The next incident my husband drove to doctors office and it did the same thing to him and would not move in the parking lot . About 15 minutes later he got it to start and he made it home . We quit driving it. Then we got a letter from Ford with the recall 26V237 .i waited for 2 nd letter and had not gotten it with a fix for it so I called the Ford Service Repir Center and they got us on the phone connected to a Ford Dealership in Yukon , Ok and they came and got the pickup on August 10 th and took it and brought it back on August 11 th and they did the soft ware update and told us no codes came up and we know better . When I had previously talked to the Ford Repair Center I was told if the wrench came up that they would replace the lead frame at no cost to us and give us a 150000 mile warranty. So it is now our word against the mechanics that the wrench ever came up INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
There was no incident. Ford announced the recall in May with no remedy until August. I had to rent a truck to pull a trailer to [XXX] from [XXX]. They said there was no compensation as there was no "stop driving" notice on the recall but it stated the the vehicle could abruptly slow down /downshift increasing the risk of a crash. Wanted to go on record that compensation should be covered by Ford as this was a considerable safety issue. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

dangerous, unpredictable transmission behavior Violent Downshifting Sudden jerks and Deceleration and Power Loss: heavy jerking loud clunks gears making noises following the forced downshift, the truck will experience a massive, unexpected drop in speed, sluggish and difficult maintaining speed like limp mode driving in normal speeds then loss of power engine stalling in the middle of rush hour traffic The Orange Wrench Light and check engine light will come on with codes p0720 I went did the reprogramming as recommended Ford dealer performed Safety Recall 26S28, which immediately aggravated the transmission failure and left the vehicle completely disabled. in the highway the dealer's recall software update directly caused a total transmission failure the next day, and i feel unsafe driving the vehicle due to the severe unprompted downshifting risks outlined in NHTSA Campaign 26V237 Felt unsafe to drive truck and also it became undrivable and had to have it towed to the Ford dealership that did recall.

On May 10, 2026, my 2016 Ford F-150 (6R80 transmission) went into limp mode and displayed a wrench warning light while driving. The truck lost normal transmission function and would not shift properly, creating a hazardous situation in traffic. The PCM has stored active Output Shaft Speed sensor codes P0720 and P0722, confirmed by a Paul Miller Ford IDS diagnostic scan, indicating a failing molded leadframe inside the transmission. When the OSS signal drops out at road speed, the PCM can command a sudden unintended downshift into a low gear, causing abrupt deceleration and potential rear-wheel lockup and loss of control — a serious safety hazard. Ford Recall 26S28 (26V-237) / Program 26N04 for 2015–2017 F-150s only covers a PCM software recalibration for the Transmission Range Sensor codes (P0705/P0706). It does not replace the leadframe hardware for the OSS codes (P0720/P0722) my truck is actually throwing. Ford's system auto-rejected the claim, and a dealer SSSC prior-approval request was denied. I am being told to pay out of pocket for a known defective part. For 2014 trucks, Ford replaced this same leadframe for free under Recall 24S37 when P0720/P0722 were present. My 2016 has the same transmission, same part, and same failure, but the current recall remedy is software only. I'm filing so NHTSA has documentation that the OSS leadframe failure is occurring on 2015–2017 F-150s and to request the remedy be expanded to cover leadframe replacement. No crash or injury occurred; the defect was confirmed by a Ford dealer.
Ford recently did a software update for a unexpected downshift recall that had previously happened to me while doing 60 miles an hour in my vehicle downshifted into first gear in my truck went sideways because the rear end locked up. Drove my vehicle home and took it out today and while doing 45 miles an hour it downshifted into first gear locking up the rear wheels again. Thankfully, this was at a lower speed so I was able to control the vehicle easily.. the turnpike speed limit in Maine is 70 miles an hour so if this happened at that speed in traffic, I’m almost positive there could have been a serious accident. So their update did not fix this issue.

The contact stated that while driving at various speeds, the transmission unexpectedly downshifted and became stuck in a lower gear before shifting into the intended gear. The powertrain warning light was intermittently illuminated. The vehicle was taken to the dealer, where DTC: P0705, P0706, P0707, and P1702 were retrieved, indicating a powertrain control module (PCM) and leadframe failure. The vehicle was not repaired because the warranty had expired the day prior. The contact later received notification of NHTSA Campaign Number: 26V237000 (Power Train), and the vehicle was taken to the dealer, where the PCM software update was completed. The contact was advised to drive the vehicle for an extended period and return to the dealer to determine if the leadframe needed to be replaced. The failure persisted while driving, and upon returning to the dealer, DTC: P0717 and P0715 were retrieved. The dealer submitted a claim to the manufacturer, but coverage for the leadframe replacement was denied because the diagnostic trouble codes did not match the recall specifications. The contact stated that the PCM software update erased the initial diagnostic trouble codes, preventing the leadframe replacement under the recall. The vehicle was not repaired. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The failure mileage was approximately 81,000.
I went to start my truck and the brake went all the way to the floor. I had no braking power and it had to be towed back home over an hour away. There is a recall on 2016 Ford F150 with the 3.5 Eco Boost engine which I have. This recall involves a seal in the master cylinder failing and losing braking power and affecting the brake booster. Referencing recall 20S31. I went to Ford and they told me the recall did not apply to this specific truck due to a different batch of parts. This truck is now having the exact problem as the specific ones which have already been recalled. I believe the recall should be expanded. This occurred shortly after driving an hour in the rain with my kid in the back. It could have easily killed us on the highway with no brakes. We had no idea this recall was in effect. It’s the last time I’m buying a Ford. I want to feel safe with my kids in the back and Ford does not seem to care.
On XXX I lost all brakes in my 2016 Ford F-150 while towing a fishing boat. I had just decended a steep grade on a busy downtown street when I received an alert that I was low on brake fluid. The next pump of the brake went to the floor. I received an audible alarm. Had this occured on the steep grade, this could have been a very serious accident. I understand the loss of brakes was due to failure of the master cylinder just and as described by the open recall on my vehicle. I added a quart of brake fluid to an empty resevoir, tried to pump the brakes up and the fluid vanished. I could not find any evidence of a leak. Clearly the fluid is in the brake booster, which is also replaced under this recall. I had the vehicle delivered to the closest dealership, Pierre Ford of [XXX] on Monday the XXX. The service advisor tod me he would need ot for two days. Four days later, the advisor text me to report that parts are not available, and that he would let me know when they were. I asked him to make sure that a VOR report had been submitted so that my repair repair parts could be expidited by Ford Corporate. He assured me the VOR had been placed. Today, on XXX, I called Customer Care who verified my recall eligability, and also informed me that no VOR had been requested against my VIN. I have found genuine ford parts for the repair of my vehicle in many Ford facilities in the nation. I dont feel that Ford is working very hard to get my only vehicle back on the road INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

The message “Service Vehicle” was displayed and the wrench symbol was illuminated. The vehicle was taken to the dealer, who confirmed that the vehicle was included in NHTSA Campaign Number: 26V237000 (Power Train) and the vehicle was repaired. The contact stated that approximately 5 miles after retrieving the vehicle, the failure reoccurred. The contact called the dealer and confirmed that the recall had been closed because the recall repair was previously performed, and that any further inspections or repairs would be out-of-pocket. The contact took the vehicle to a certified mechanic, and the vehicle was repaired. The contact was uncertain of the repair but confirmed that the failure had not occurred since the repair. The manufacturer was not informed of the failure. The failure mileage was unknown.
I was driving on the highway speed limit 65, and all of the sudden it down shifted to first gear, jerking the truck almost to a complete stop, very scary and unsafe. I pulled over to recoup myself . It is now happening more frequently, wrench light on the dash . Not sure what I need to do , my truck is my life support for me and my family! Help me please! Thank you,

The contact owns a 2016 Ford F-150. The contact stated that while driving approximately 30 - 45 MPH and attempting to accelerate, the transmission unexpectedly downshifted with the wrench warning light illuminated, with the number 6 displayed. The vehicle was coasted into a nearby parking lot, and the contact waited for her husband, who was able to get the vehicle to the dealer. However, the vehicle failed to exceed 35 MPH. The dealer diagnosed the vehicle with a powertrain control module failure. The vehicle was included in NHTSA Campaign Number: 26V237000 (Power Train). The dealer informed the contact that the recall repair would be completed. After retrieving the vehicle, the warning light illuminated approximately 20 minutes later. The vehicle was returned to the dealer, who diagnosed that the lead frame needed to be replaced. The vehicle was not repaired. The vehicle remained at the dealer. The manufacturer was informed of the issue and opened a case. The failure mileage was approximately 160,000.
I have a problem with my truck with the Molded Lead Frame. It is shifting erratically and not shifting below 3rd gear. I has an open recall for it, 26N04, but Ford won't perform the service because they are not getting the correct code from the computer system. So I am now forced to drive this truck in this unsafe condition until I get the correct codes.
The contact owns a 2016 Ford F-150. The contact stated that while driving at various speeds and while the vehicle was idling, the transmission suddenly disengaged. The accelerator pedal was released for a while, then the contact attempted to engage the transmission to accelerate. The vehicle failed to accelerate intermittently and then suddenly accelerated. The contact became aware that the message “Service Vehicle”, and the wrench symbol were temporarily illuminated. The contact stated that the vehicle was previously included in NHTSA Campaign Number: 26V237000 (Power Train), and the powertrain control module software was updated. The contact stated that, immediately after the software was updated, the failure persisted and became more persistent. The contact stated that the warning lights became illuminated again. The vehicle was returned to the dealer, and a week later the dealer diagnosed the vehicle with transmission failure. The contact was informed that the transmission needed to be replaced. The vehicle was not repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 100,000.
Vehicle suddenly seemed to shift while driving causing a loud sound, jerk and caused a loss of control suddenly.
I own a 2016 Ford F-150 involved in Safety Recall 26S28. The vehicle suffered an unexpected downshift and also temporarily locked into 5th gear. The dealership confirmed the transmission lead frame failed. Ford is denying coverage because the failure registered as code P0717 (Input Sensor) instead of an Output Sensor code, even though both sensors are manufactured as a single, un-severable piece on the exact same recalled lead frame part. Beacuse my sensor that is housed within the exact same lead frame broke down, instead of other sensors, Ford is denying to cover the cost. The issue is that the lead frames are place in a location that they get very hot and can become brittle over time. Causing any wire or sensor housed within it to become under extreme stress and potentially fail. I dont understnad how Ford can say some sensors inside the lead frame are covered but others inside the very same part are not. My truck is currently in the dealership and the technician has informed me that this lead frame part is bad. I did have the wrench light illuminate in my truck and the PO717 trouble code was present. Because an abrupt downship can occur, this is a safety risk with known reported injuries already reported via the recal. Recall #26S28 Case # with Ford Motor COmpany is CXH-08688952

My truck is equipped with a 6R 80 transmission and as I was pulling off an offramp onto interstate 295 my transmission shifted into first gear and my truck skidded and came to a stop sideways with oncoming, traffic, barely avoiding me. The rear end of my truck literally locked up.
