Critical Deferred Risk
Critical deferred failure risk — 4 safety-critical or high-cost components are past their service interval. Without immediate attention, repair costs will compound significantly.
No odometer data on record. Mileage is estimated at ~144,000 mi based on average annual driving. Items marked EST. OVERDUE may already be due — verify service history with the seller. Items marked OVERDUE are confirmed past-due based on vehicle age alone.
EST. OVERDUECriticalTiming Chain Tensioner
Service interval: 10 yr · Vehicle age: 12 yr · Past due by age
Service Timing Chain Tensioner immediately — already past the 120,000 mi / 10 yr interval
Worn chain tensioners cause timing chain skip or snap — common on GM 5.3L/6.2L, Ford EcoBoost, VW/Audi EA888, BMW N47/N63. Listen for cold-start rattles.
EST. OVERDUECriticalWater Pump
Service interval: 8 yr · Vehicle age: 12 yr · Past due by age
Service Water Pump immediately — already past the 100,000 mi / 8 yr interval

On April 5, 2026, while driving downhill around a corner in my 2014 Toyota Prius, the brake booster and brake booster pump assembly failed without any prior warning. When I pressed the brake pedal the car did not slow down and instead appeared to accelerate due to a complete loss of braking assist. I was unable to stop the vehicle normally and had to rely on gravity and coasting to slow down, creating a serious risk of collision. There were no warning signs prior to the failure. Immediately after the incident, three warning lights illuminated on the dashboard: the BRAKE light, the ABS light, and the VSC (vehicle stability control) light, which have remained on since. The vehicle was taken to DARCARS Toyota Baltimore, 6007 Eastern Ave, Baltimore, MD 21224, where the failure was confirmed on April 15, 2026. Technician Victor Lawson diagnosed diagnostic trouble code C1391, which is the exact qualifying code listed under Toyota's Customer Support Program ZJB for brake booster and brake booster pump assembly failure (RO# XXX). The vehicle remains at the dealership and is available for inspection. Toyota corporate was contacted and opened case number XXX but declined to cover the repair despite this being a documented issue that Toyota has publicly acknowledged. While driving, three warning lights suddenly appeared on the dashboard . Abs warning, traction light, and break warning light At the same time, the brake pedal became very hard to press, and the vehicle did not slow down as expected. This caused a significant increase in stopping distance, and I nearly rear-ended the vehicle in front of me. The issue appears to involve the braking system and created an immediate safety risk. No prior warning signs were noticed before this incident. The vehicle has not yet been inspected but is available for inspection upon request.

Subject: Brake Booster/Pump Assembly Failure – Toyota Program ZJB / 23TE07 Description of Incident: I am reporting a failure of the brake booster and pump assembly on my 3rd Generation Prius (82,000 miles). While the vehicle is 11.3 years old, it is well under the 150,000-mile threshold of Toyota’s Customer Support Program ZJB. The brake pump began cycling every 13 seconds, indicating an internal pressure leak in the accumulator. This is a documented precursor to a total loss of power-assist braking. Despite the clear mechanical failure, no warning lamps or Diagnostic Trouble Codes (DTCs) were triggered, creating a safety hazard where the system could fail without prior electronic notification. Repair and Cost Details: Because the vehicle was 1.3 years past the arbitrary 10-year "Secondary Coverage" window, I was forced to pay $3,461.18 out-of-pocket for a known manufacturing defect. I would like to highlight a discrepancy in labor billing: while Toyota’s own technical bulletins (ZJB/23TE07) state the repair should take approximately 5.3 hours, the dealership charged 8.9 hours of labor for the same procedure. Safety Concern: By limiting coverage to 10 years, Toyota has left many low-mileage owners with a critical safety defect that degrades over time regardless of mileage. I request that the NHTSA transition this from a "Customer Support Program" to a formal Safety Recall to ensure all affected vehicles are repaired and owners are reimbursed, regardless of the vehicle's age.

the cylinder head gasket blew because of the poor design of the intake manifold and the EGR system. There are updated parts to fix this issue, but there was no recall. This poor design led to the vehicle having "the Death Knock." It started doing this knock on my trip driving my daughter to college. The engine could have seized in traffic on the way to [XXX] from [XXX] . No warning lights came on telling me there was an issue. The vehicle is being taken apart now to see if my connecting rods were bent from the coolant entering the combustion chamber. The contact owns a 2014 Toyota Prius. The contact stated that while driving at an undisclosed speed, the vehicle failed to slow down as intended with the brake pedal depressed. The ABS warning light was occasionally illuminated. The vehicle was pulled over to the shoulder of the roadway and restarted. The vehicle was taken to a local independent mechanic, where it was diagnosed, and determined that the brake master cylinder, brake booster, and brake lines needed to be replaced. The local dealer was contacted; however, the vehicle was not diagnosed or repaired. Upon further investigation, the contact related the failure to an undisclosed recall; however, the VIN was not included in the recall. The manufacturer was notified of the failure. The contact was referred to the NHTSA Hotline for assistance. The failure mileage was approximately 152,000.

Vehicle: 2014 Toyota Prius v Hybrid Mileage: 79,000 miles The vehicle experienced sudden failure of the brake booster / brake accumulator system. Multiple warning lights appeared including ABS, traction control, and the red brake warning light. Diagnostic code C1391 was retrieved. The brake pedal became soft/spongy and the electric brake pump began cycling frequently. Braking performance felt reduced and inconsistent, creating a potential safety risk. This vehicle has very low mileage (79,000 miles) and has been well maintained. The failure appears to be related to a known defect in the Prius braking system. Toyota previously issued Customer Support Program 20TE07 addressing brake booster and pump failures on 2012–2014 Prius v vehicles. Despite this known issue, the repair is not being covered and the cost quoted is several thousand dollars. A failure of the brake booster/accumulator system significantly affects braking performance and should not occur at this mileage. This appears to be a widespread issue affecting Prius vehicles and represents a potential safety defect involving the braking system.

Head gasket failure. No check, engine, light or warning. My car was very well maintained regularly. After research, I found this to be a very common problem. Yes it is available for inspection. The brake booster/accumulator failed on my vehicle. This part was a common failure during the 2010-2015 production run so much so that Toyota extended coverage to replace. Mine failed recently beyond the warranty extension of 10 years/150k miles. My vehicle has only 108k miles and is 15 months beyond warranty. Toyota Corporate will not replace this vital part, despite the fact it was a defective design. I can’t drive it any longer since I don’t have the $4000 to make repair. Toyota Corporate has shown little concern despite this critical safety defect.

On Jan 5, 2026 I was driving locally at about 40-45 mph. All of a sudden 3 indicator lights came on ABS, Brake, and Brake System Warning Light. There were no noises, but the brakes felt different. They did stop when needed. I left space between myself and others just in case, did not want an accident. Called my regular mechanic. They advised to not drive the car, but to bring it into the shop at 8am the following day. On Jan 6th, the mechanic stated it was beyond their expertise. It was towed to the local Toyota Dealer for repairs. It ended up being the Brake Booster and Pump. Upon investigation online, we saw this was a recall, but we were not notified by Toyota.

I was driving on the freeway, and a few lights(including ABS, brake lights) all came on and a beeping noise started, as i immediately lost complete control of my brakes! That God it was late, and no cars were in front of me. The car slowed down as I removed my foot of the gas right away. I tried to pull over as soon as I heard the lights, and that’s when I realized my brakes were completely gone! I was able to stop after about two miles; car stop on its own as it slowed down. Apparently, this was a major known issue and had warranty up to 150k miles. At the time of the incident, my car had about 165k miles. I had the car for years, and no one at the dealer ever mentioned anything about this or even tried to make me aware of it! I called the Toyota dealer and corporate number several times and they declined providing me with any help at all! This caused me fear and traume because I never thought this was possible! It could’ve ended my life and the life of people around me if there were any cars in sight! The problem has been confirmed by a reputable mechanic! He did the repairs! He also called the Toyota dealer and corporate and attempted collecting anything since he found out about the warranty and common issue. They also denied help. No lights were on at all before this incident happened! It all came at once! I even have all records of monthly/ bimonthly complete inspections at the dealer with everything normal on the results!

The brake booster system (electric brake booster pump) in my 2014 Toyota Prius has been making frequent, audible cycling noises inside the cabin, particularly during cold weather. I have also noticed a faint odor outside the vehicle that seems related to electrical movement. No ABS or brake warning lights have illuminated, but the cycling pump may indicate increased stress on the brake system. The issue has not yet been diagnosed or confirmed by a dealer or independent service center. The problem has not caused a loss of braking ability to date, but the unusual noises and electrical odor suggest potential risk if the booster were to fail completely. No recall or customer support program currently covers this issue. The component is part of the vehicle’s brake system and would be available for inspection by a dealer or manufacturer upon request. Toyota has offered no help or possibility of Goodwill assistance inspection for this issue.

The brake system malfunctioned suddenly. While driving and while stopped, the brake pedal became very hard and braking behavior became unpredictable. The vehicle sometimes brakes aggressively and other times requires excessive pedal force to stop. The ABS and BRAKE warning lights illuminated. The vehicle no longer provides normal braking assistance. This issue is related to a failure of the brake actuator / brake booster, which is a known issue affecting Toyota Prius models from 2010 to 2015. This defect creates a serious safety risk, as braking performance is inconsistent and may increase stopping distance or cause loss of control during emergency braking situations. The vehicle has not been repaired yet because the cost is very high and the manufacturer has declined assistance despite this being a known safety-related defect.

No brake issues were reported when I brought it in for inspection in 2023. I never received any mail stating that this was a know defect and I needed to bring my car in to get it inspected. It's obvious to me that Toyota designed a defective part that impacts safety, and is forcing the owner to bear the cost of the fix by creating parameters that can never be met before the warranty expires.

ODI #11689489SERVICE BRAKES

My brakes consistently lock and the car lunges forward. Driving down the highway I tapped my brakes and they didn’t work. I lost traction in the brakes. I had no control over the car or speed. I’m scared everyday I have to drive it. This is extremely dangerous and I've seen this comment often with this vehicle. Toyota said they had an extended warranty for this but it expired. I never received any mail or phone calls stating that this was a known defect. They say they can’t do anything now and are quoting me over $3000 to fix it.

The brake actuator failed, it can be available for inspection anytime. My ABS light, brake light, and traction control lights all came on to indicate the failure. Since the hybrid engine requires the breaking system to recharge, failure of this part will eventually lead you to be stranded if it isn’t functional. Additionally, the responsiveness of the breaks makes driving at higher speeds a lot riskier. Toyota offered an "extended warranty" to have the parts replaced for free for 10 years or 150,000 miles, whichever came first. My make is 2014, at only 89,000 miles.

Apparently Toyota knows of a recurring problem with the brake actuator. I took my Toyota Prius to the dealer for an unrelated recall issue that they repaired and I asked them what the sound was that I kept hearing every 10 seconds or so. The told me it is the brake actuator, but did not seem concerned at all that I get it repaired right away. They did quote me a very high price to have it fixed by them. I have chosen to take it to my mechanic for repair. In the mean time, I decided to research this on the internet and discovered that it is a known problem by Toyota and there is a class action suite filed on them. I want Toyota to pay to have it fixed! I fear for my life and lives of others with the same issue.

There was a safety recall issued for brake system malfunctions involving the brake booster and brake booster pump assemblies on my vehicle. Unfortunately, the recall expired on December 31, 2024. On March 4, 2025, at 4:10 PM, I contacted the District Manager Advocate in Plano, Texas. I explained that I had been in the ICU twice and am now [XXX]. During the recall period, I was in and out of hospitals for more than four years, making it impossible for me to be aware of or respond to the recall in time. My vehicle has since begun exhibiting the exact symptoms outlined in the recall. When I brought this to their attention, I was told that while the issue was indeed covered under a past recall, there is nothing they can do now because it has expired. This vehicle is our only mode of transportation, which is critical for my ongoing medical treatments, as well as for my husband who recently suffered a heart attack. As I write this, we have no other option but to continue using the vehicle—despite the serious safety risk—because of the urgent and necessary care both of us require. On August 4, 2025, I contacted Toyota headquarters. They reached out to the dealership and spoke with management, but unfortunately, no resolution was offered except for information about a separate, newer recall. I followed up again with the District Manager and spoke with Ashley, who informed me that after escalating the case, there was still nothing further they could do and she expressed her apologies. This situation has caused further deterioration to my health due to the stress and anxiety of potential vehicle failure, and it has added to the burden of my husband’s fragile health condition. At this time, he has been unable to return to work, and we have no income to cover the cost of a repair that should have been addressed through the original recall. We would deeply appreciate any assistance, reconsideration, or compassionate review of our case. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I am writing to express my profound concern and disappointment regarding a recent, terrifying incident involving the brake system of my 2014 Toyota Prius. On Saturday, June 21, 2025, while driving with other passengers, my vehicle experienced a complete and sudden brake failure. Multiple warning lights illuminated on the dashboard, and despite my efforts, the brakes became unresponsive, making it impossible to stop the car. The sheer terror and helplessness I felt in that moment, with the very real potential for a catastrophic accident involving myself, my passengers, and other road users, is indescribable. Fortunately, through sheer luck and quick thinking, I was able to make an emergency stop on the side of the road, preventing any collision or injury. However, the emotional toll of that experience, the lingering stress, and the fear of a recurrence remain significant. I was subsequently informed that the cause of the failure was a leak in the brake accumulator. I am attaching the diagnosis and repair invoice. The necessary repairs were not completed until Wednesday, June 25th. This unforeseen and critical malfunction also necessitated the expense of a rental car for four days, adding to the financial burden and inconvenience. My frustration is compounded by the fact that this brake accumulator issue is a well-documented and known defect in Toyota Prius models, specifically affecting 2010-2015 vehicles. Toyota itself has previously issued a Technical Service Bulletin (TSB T-SB-0043-13) acknowledging this problem and offering warranty extensions for affected vehicles, typically up to 10 years or 150,000 miles. Despite this being a recognized safety concern, I was informed that my vehicle falls approximately eight months outside the 10-year warranty period, thus disqualifying me.

We were driving on a city street about 25-30mph and the car jerked suddenly and the lights on the dashboard illuminated and the "Check Hybrid System" message came up as well. The car was able to coast to a nearby parking lot. We attempted to drive the car home and the car stalled in the middle of the street. We were able to push the car off the road and then get it towed. The presented a potentially very dangerous situation as the car had a complete failure while driving in the middle of the road. The Toyota service department ran a diagnosis which returned DTC P0A94. All of this indicates that the car should be covered under the CUSTOMER CONFIDENCE PROGRAM 23TE03 however that program seems to only cover the 2010-2015 Prius and 2011-2017 Prius V models and not the Prius C. There are several reports of this same issue happening on the Prius C as well though so it doesn't make sense that it is not covered.

After 120K miles and 11 years of use, my Prius 3 brake booster pump failed while I was driving. The warning lights came on, including the ABS and brake lights. My brake felt strange, and I had to finish my drive to the dealership keeping a large driving distance so the brakes would work in time. I was advised that this was a result of a C1391 error and that the brake pump part is only covered under extended warranty for 10 years. The advisors at the dealership said it is extremely common for cars to come in with this problem just after the warranty period has ended. I have spoken to other Prius user who have complained of the same issue and are considering a class action lawsuit because this safety issue is so common.

While driving my dashboard lit up with Brake, ABS, and traction control warning lights. Brake pedal fell to the floor when pressed. I was then able to stop the car. I brought the car to a local auto parts store, where they were able to determine that the error code was C1391 brake accumulator pressure leak. I made an appointment and brought my car to my local Toyota dealer the next day. I was informed that the brake booster assembly, brake booster pump, and master cylinder would have to be replaced, at a cost of approximately $4035.00. I was informed that Toyota issued a Customer Satisfaction Program for this exact issue on my car. While my car has 129,000 miles and falls within their coverage range of 150,000, the car’s age of 11 years did not fall within their limit of 10 years. According to Toyota, this is a known failure and my only problem is that my brakes did not fail sooner. Toyota’s corporate office denied my appeal to cover the cost of the repair. The local Toyota dealer charged me $150 to read the code and confirm what the local Auto parts store was able to do for free.

My 2014 Toyota Prius C experienced a complete hybrid inverter failure. The part number of the inverter is the same as the one covered under Toyota’s inverter recalls (such as Recall J0V), yet my VIN is not included. The failure created a serious safety concern, as the vehicle lost power while in motion. Toyota’s recall appears to be too limited in scope and may need to be expanded to cover Prius C models that share the same faulty part. I am requesting that NHTSA investigate this issue, as it appears to involve the same hardware defect acknowledged in other Prius models.

Apparently there is a known defect on the brake actuator/master cylinder/booster on the 2014 Prius that would have been addressed if the car had been driven for less than 10 years or 150,000 miles. We purchased our car in April 2014 and the part just failed yesterday at only 126,000 miles. We understand that there is a pending class action suit (or perhaps one that was settled) but do not know how to become a member of it. It seems as if they knew that the part was defective, but did not offer to do a voluntary recall and remediate this before a failure, they should have, but instead, now we have a $3400 repair bill. Requesting assistance in evaluating whether there was negligence and or liability for them not addressing this issue in a timely manner.

Yesterday, while driving on the interstate, all of the brake related warning lights on my dashboard lit up- ABS, traction control, brake, caution)- causing me concern, so I looked for the next exit. While exiting and slowing, I noticed my brakes felt different and were not as responsive as usual. I called my mechanic and cautiously headed to his shop a couple of miles away. Today, I met with him and found out that there is a leak in the brake booster line, but the extent of the problem is yet to be determined. I immediately did an internet search for my model year and found that there was a safety campaign on other Toyota hybrids for that year, so I called the Toyota customer service number. I was told that my vehicle had an extended warranty for the brake booster, but it expired 9 months prior, 10 years post purchase, The rep has escalated my issue but I will have to wait to see if someone with more authority can potentially give me an additional extension. Meanwhile, my car is not drivable, as the brakes could fail at any moment. In my estimation, this should not be a time limited coverage, but an actual recall covered by Toyota. This is not normal brake pad rotor wear and tear, but a known issue with the assembly system and a true safety hazard that one cannot foresee without experiencing minor to catastrophic failure of the system.

I researched what it was and found on reddit that there was customer support program ZJB to repair failed brake accumulators, which affects hundreds of thousands of these cars. The problem was that my brakes were only in the process of failing rather than being fully failed enough to send codes and light up the dashboard, which is the only way the customer support program would cover the $4,000 repair. The failure finally happened when I was three hours from home and had half the braking power as usual and had to limp the car home for fear of the brakes going completely out. I can't believe Toyota is letting this many people drive around with a potentially catastrophic brake issue.

I was driving on the highway during rush hour when a loud beeping noice began. My 15 year old cousin was in the passenger seat and began to check for the source of the beeping. I was driving 55mph on a busy highway with no safe shoulder stop. As I was preparing for the exit, the brakes suddenly became dysfunctional. The pedal was extremely hard to press down and I found myself unable to slow down the car in enough time to avoid hitting upcoming traffic. I found myself in an urgent, life threatening situation and needed to pull over into the emergency lane located before the exit. I could not just consider my safety, but that of other drivers and my 15 year old passenger. She is traumatized. I was able to get the car to stop with my entire weight on the pedal. Once stopped, I scrambled to find the reason for the sudden brake problem, checking if anything rolled under my foot, if the floor mat was loose, if the gear shift was wrong, even checked if I accidentally hit the emergency brake. Nothing was out of place. No new dash lights. I looked in the manual. I had no information. I went to jiffy lube and they performed a diagnostic test and informed me they are unable see all the codes with hybrids because Toyota’s computer can only see Just in August I went to Toyota for my oil change, nothing was said about the brakes needing to be fixed and how repairing them now would prevent the failure of the accumulator pump system. If I had known paying $400-$500 for the brakes then would prevent having to pay $3000 to replace a pump. I would have made that sacrifice and went outside of my budget. This is paramount information that should be disclosed to all Toyota customers that take their car in for care and pay. My 2014 prius experienced a failing brake booster actuator assembly. I was driving when suddenly my traction control light came on and my brake pedal turned mushy. I pulled over to a parking lot and put my vehicle in park. As soon as I did this, the ABS and parking brake light both turned on. I had the vehicle towed to the Toyota dealership. They diagnosed the problem as an internal leak in the brake booster actuator assembly. This is a common problem with these vehicles, but instead of Toyota issuing a recall, they only did an extended warranty coverage of which my prius didn't fit the parameters. Now I am stuck with a vehicle that is unsafe to drive and a repair estimate of $3,900 because of one of their parts failing. According to NHTSA this is a known defect on this vehicle and it affects more than 100k Toyota Prius vehicles. So basically, we've been driving around in a vehicle that the brakes may fail at any moment and potentially kill us all but is only addressed by the manufacturer when the known failure occurs. Even then, they only address it if the error occurs during their set time frame. It seems this has been brought to NHTSA's attention on a few occasions and nothing has been done. Brake Booster Assembly in my 2014 Toyota Prius began cycling more often than usual in December 2023. By March 2024 it began cycling approximately every 30 seconds. Randomly while stopped or parked, there would be four warning lights illuminated on the dash - SLIP indicator, both Brake System Warning Lights - the word "Brake" and the "((!))", and ABS Warning Light. By June 2024, I would also randomly lose stopping power & a great distance was necessary to stop the vehicle. I took the Prius into my local Toyota Service Center where it was recommended both the Brake Booster Pump and Brake Booster Actuator be replaced. When I brought up Toyota's Customer Support Program for which my Prius is eligible in terms of age, mileage, and symptoms, I was told a DTC is required to verify the issue. According to this local Toyota dealership, the Master Tech confirming the issue and recommending replacement is not sufficient. Since the lights were illuminated, I asked why there was no stored DTC. My understanding of sound was that it was normal for vehicle, however, within the time span of approximately 2 weeks, sound became more frequent. Brakes did not show signs of loss of function and there was no indicator lights on dash to warn me of problem with vehicle. When I inquired with dealership of how much longer they would last, they informed me they could not say, and that it was likely they could go out at any time. According to support program 21TE01, Toyota was willing to replace brake boosters that had already failed and could be confirmed with diagnostic code. However, as the part had not failed or showed signs of failure at that time, vehicle would not have qualified, and it was prior to current ownership. Owners should not have to wait for their brakes to fail in order to recognize they have an issue. Code C1391 - Abnormal Leak in Accumulator. Accumulator for the brake pump is not holding pressure so the pump has to keep turning on to refill. Brake pump and ABS unit have to be replaced. In emergency situations, a malfunctioning ABS module leads to increased stopping distances and loss of steering control during hard braking. Lastly, tires can be worn more easily, which could lead to tire blowouts. This puts myself and others at risk for deadly consequences due to an increased chance for collision if I am unable to control or stop my car. This problem has been confirmed by Toyota and multiple years of Toyota vehicles have had their brake actuator assembly warranty enhanced. The warranty enhancement program has secondary coverage, which is applicable for 10 years from the first date of use, or 150,000 miles, whichever occurs first. Currently I meet the 10 year requirement but have 162,000 miles on my prius, which is completely frustrating. Many have posted with the same problem of being just outside the warranty. The vehicle has been inspected at a Toyota dealership. A symptom was noticeable starting from over 1 ½ years ago when I was under 150,000 miles. The car produced clicking sounds when the brake pedal was held down at traffic lights. Once released, the clicking sound would stop. I spoke with a Toyota mechanic and he told me it was an issue with the brake pump and ABS unit, but Toyota would not repair it until it completely failed. He also mentioned that there was coverage available for the ABS unit once it failed as it was a common issue for the Toyota Prius. If Toyota warranty allowed for the brake pump and ABS unit to be replaced when the symptoms appear instead of when it completely fails, I would have qualified with my mileage. Not only is the repair costly, but for those who cannot afford it and need to continue to use their car despite the brake pump and ABS unit failure, risk their own and others safety when continuing to operate their vehicle.

I was dropping my daughter off at school and the car completely shut down on me and turned off in the middle of an intersection. I have just had brakes and oil changed but ever since I got the car codes keep popping up and when checked it’s all good. Front lights keep glitching and even with new brakes there is still a bad sound. My dash computer thing keeps going in and out. It happens a lot more when it rains and makes a sizzling sound I’m afraid to drive it in the rain. I’m besides myself how the car turned off on me and my child we couldn’t move I could accelerate and we almost got hit then another light came on and I couldn’t even turn my wheel. Very dangerous

the brakes have not failed yet, but there is a known issue with the brake booster on my model year. I would like to have the government force Toyota to guarantee the brakes for a longer period of time. currently Toyota has extended the warranty out 10 years / 150k miles from date of first use. Since these cars last so long, this should be 20 years / 300k miles from date of first use.

The ABS and traction control system in my 2014 Toyota Prius 4 are not functioning. They ABS light an the traction control lights are both on. I work in the automotive industry and I used a code reader to pull the codes and got a C1391 which is a brake booster and brake booster pump. That means either the brake booster has failed or the master cylinder is leaking. I contacted Gosche Toyota in Hemet and after waiting for a service writer to contact me (which they never did) I called 5 times till I finally got someone on the phone. They were very familiar with the problem. The problem is so common that SOME vehicles have been recalled. But for others like mine only a "Customer Support Program" was made. That program covers repair "if the parts are available" only for 10 years or 150,000 miles whichever comes first. But for someone like myself who has 170,000 miles Toyota says there's nothing they can do. The cost for me to repair it is $4800. The customer support program bulletin "ZJB" even says that service writers are not allowed to tell customers about the problem or the customer support program. That way when the car doesn't meet the requirements, they won't be responsible for the repairs, even though they know it will eventually fail. There is a lawsuit somewhere regarding this very same problem but for me, I'm screwed. I depend on my car to make money and now me and my family are out of a car. This is a known defect and a safety concern. Toyota is okay with leaving us with nothing.

After I started my car and began driving away from a shopping center, I noticed that there were 3 static warning lights on the dashboard that had not been there before. They were the slip indicator light, the brake light, and the ABS light. I also heard a short, repeating buzzing sound coming from the front of the car that I had never heard before. As I was driving, the brake suddenly felt loose. Luckily, this happened as I was pulling out of a shopping center parking lot, so I was able to pull over to the side of the road instead of getting on the highway, which I was about to do. The following day, a Toyota dealer indicated that the issue was the brake booster pump/actuator system and that my car returned with error code C1391. The dealer indicated that this is a known problem with Priuses within my year range. I was told that I would have to replace both the pump and actuator. After talking to the dealer, I realized that there had been subtle clues of this problem beforehand. In the few months leading up to this incident, sometimes if I hit a big bump in the road, for a moment the Slip indicator light would flash and I would feel like I had no control of the car i.e. I was ever so briefly slipping. However, because this happened so briefly and when I hit a bump and everything went back to normal, I didn't think this was an actual problem, but now I believe this was an early sign of the brake failure. I contacted Toyota Corporate Office. They confirmed that this was an issue known to them with Priuses and said that they had sent letters to the previous owner of the car in 2019 offering an extended warranty not a recall. However, apparently, the previous owner had not fixed the problem and the extended warranty expired. Toyota refused to pay for the repair for this and refused to forward a copy of the 2019 letter to me,. This brake defect is a serious issue with no major warning signs. As of this writing, the part is available for inspection.

I heard a whirring sound but couldn’t tell where it was coming from. The only way I could get it to drive was to turn the car off and restart. Good Evening, I'm writing to request a callback in regards to my 2014 Toyota Prius which is subject to a recall. "The involved vehicles were designed to enter a failsafe driving mode in response to certain hybrid system faults. Toyota has found that in rare situations, the vehicle may not enter a failsafe driving mode as intended. If this occurs, the vehicle could lose power and stall. While power steering and braking would remain operational, a vehicle stall while driving at higher speeds could increase the risk of a crash." I recently just moved back to South Florida, I was living in NJ for a few years and had the Inverter malfunction earlier this February, 2023, forcing the car to enter the fail safe mode described giving me the warning to "Check Hybrid System, Stop the Vehicle in a Safe Place". The car was at Toyota Universe, Little Falls, NJ for about 45 days, since this was going to take the complete replacing of the inverter and an update of some sort of software. After receiving the car back around March 17th, I moved back to Florida about 2 weeks later. This evening around 5:30 returning from work, the car stalled again while I was driving surrounded by dense traffic, wouldn't accelerate with all lights flashing. Thankfully I was able to pull over slowly into a parking lot after which my steering wheel locked, and my car was no longer able to be put into Drive or Reverse. Car was only able to shift into Neutral and Park. This was the most terrifying experience of my life had something worse happen in the middle of traffic. I have dropped off my car to the dealership parking lot and will be checking it in first thing in the morning. I'd like to ascertain what I should expect from Toyota Manufacturing should this issue be unfixable? I am more than happy to let them attempt fixing it, but I also need to live a normal life and don't feel the need to be responsible for a manufacturing defect putting my life in jeopardy every time I go drive.

Was at Toyota dealership looking at 2023 Prius and 2023 Corolla Cross for daughter. I decided to leave and when I started backing up, my daughter noticed the Brake, ABS and Anti-Skid lights all came on at once. She asked if that was normal. It was not. A few minutes later, they stated flashing, then went out after I turned the car off. I live over an hour away and decided to drive it to Walmart, which is approximately 2 miles from the dealership and upon getting into the Walmart parking lot the lights started flashing again and upon applying the brakes, we heard a loud grinding sound. We didn’t think we were going to be able to stop! I immediately parked it and called my husband to tell him our horror story and to come pick us up. Our prayers were answered when he came and we were able to drive it slowly to the dealer. It was a cold rainy night to be on the road with braking issues. This is a very dangerous if not deadly situation! Shame on Toyota for not recalling this. How many lives have to be lost before you take ownership of your faulty hybrid braking system. We demand this be fixed for free regardless of mileage and be made a recall…NOT A CUSTOMER SUPPORT PROGRAM. RIDICULOUS!!! I don’t think our daughter wants a Toyota hybrid anymore…

My brakes completely stopped working while driving on a busy road. I took it to Toyota and they said it was my brake booster pump assembly. There is a known issue with the booster pump assembly and Toyota has a program to extend their warranty to fix this, but I was never once notified of this issue and never received any mail or notifications. They refuse to cover the repair because my mileage is out of warranty, despite the fact that this is an issue they caused and I almost wrecked and could’ve died. I was on a very busy road and my car was completely uncontrollable due to the brakes not responding. My passenger, others on the road, and myself could’ve died. The Toyota customer service representatives confirmed that this is part of the “customer service” program and it does not qualify due to it exceeding 150,000 miles. No warnings appeared on the dash until the brakes stopped working. The vehicle was inspected by Toyota and was confirmed to have said issue. This needs to be immediately recalled, I almost got into a very serious wreck.

The air temperature was probably about 50 or 60 F. On a route I routinely drive , I accelerated up hill normally from 0mph to about 45 to 55 mph. I did not accelerate aggressively. Just after climbing the short hill, The engine suddenly loses all power. I had to pull over. I look under the engine to find five pieces of metal blown out of the bottom of the engine block above the oil pan and oil spilling out onto the ground. The car has about 98,000 miles on it and was maintenance regularly including on schedule oil changes, coolant change, state inspections. There were no check engine lights or electronic error codes to indicate anything was wrong with the vehicle which I would have been able to diagnose and possibly fix. The car had been maintenanced in August/September for winter by myself. On the last maintenance, oil was discovered inside the engine air manifold and throttle body that had to be cleaned out and (The EGR was also moderately carbon clogged). I'm very dissatisfied with how Toyota handled my case. They called twice to ask when I wanted to schedule my next service appointment when they had the car sitting on their parking lot for three months. I also wrote them a detailed email similar to this report asking how Toyota could help and that I could not afford to buy another car. The engine failed and it can be inspected upon request. The vehicle had complete loss of power. The vehicle was towed to the nearest dealer. I have seen videos on youtube.com that show other people replacing blown prius engines and head gaskets. You would see chunks of metal missing from the engine block. The vehicle was not inspected by the manufacturer police or insurance. I could have been hit by another car from behind especially if the hill were longer like on a mountain side or some other situation. Separate issue: Mice enter the two rear ventilation openings to nest in the car. Mice chewed on of the oxygen sensor wires to cause an electrical fault.

Blizzard Pearl White paint started peeling off the car's body around the top areas of the 4 side doors (passenger & driver sides). There seems to be an active recall on this from Toyota for the same paint color and type, but the Prius model is not included in the active recall for this same condition. It has become a hazard such that such paint chips can be ingested by children. I took it to the Santa Monica, CA Toyota dealership and it was verified by the dealership representative there that this was a condition caused by a defect in the original paint with exposure to sunlight.

The dealer’s diagnostics resulted in a $2,087 replacement of the brake booster.  The dealer did contact the Toyota district manager along the way.  He denied coverage based on the diagnostic codes reported BUT again I am arguing that my crazy expensive repair had the EXACT same symptoms, safety issues, and repair (e.g, brake booster replacement). While driving, I received a warning "hybrid system malfunction". Within seconds, the vehicle powered off and the steering wheel LOCKED. I had no control of the car and came to an abrupt stop in lane 1 of a two lane road. I was fortunate that it was late in the evening with few cars on the road, had I been on the highway during rush hour the outcome, due to loss of power and control (steering), could have been catastrophic and perhaps fatal for me or other commuters. I note that in 2014, Toyota notified NHTSA of a recall to, "rectify software in two of the Prius's electronic control units, one for the motor/generator and the other controlling the hybrid system." Further Toyota said, "In rare circumstances, it could shut down the hybrid system itself, resulting in loss of power and the vehicle coming to a stop". This is exactly what happened to me the other night. Toyota claims, in 2014, that "it has received no reports of accidents or injuries related to the problem." This seems to be an extraordinarily amount of good luck as I could have been easily rear-ended the other night. Curiously, in this NHTSA data base, another owner reported an incident (11439720) on November 8, 2021, that is similar to the experience I had just days prior. And our VINS are eerily close. Coincidence? Not likely. Does the 2014 recall apply to my car and if not, why not given I experienced the exact same symptoms?

Brake failure / Unintended Acceleration / Unknown cause Driving at approx 25mph on urban street (road in good condition no potholes, ramps, etc) I began to slowly brake as I approached a red traffic light. Instead of braking, the car began to accelerate quickly. I pressed the brake pedal again - no response. Pressed down hard for a third time which executed an emergency stop. This all happened within 1 or 2 secs. There were no injuries or involvement of other cars on this occasion. However, had I been any closer to the car in front, I believe I would have rear-ended it. Had this occurred a few moments later, at the busy junction 400m ahead, it could have been fatal. I am absolutely sure it was not driver error. I was not distracted, drive regularly, and have been a confident and safe driver for over 30yrs. I have never experienced this in a car before, and in fact wasn't aware that it could happen until I researched it on this site. No warning lights or indicators came on. The car ran normally for the remaining 2 miles back home. I didn't think anything of it before, but prior to this incident (in our 5 yrs ownership) there were approx. 5 instances when the engine would "roar" as if it wanted to accelerate but didn't when I was breaking or slowing down on a highway. Also, there were perhaps 3 instances when the car felt as though it was "running away" i.e. a slight acceleration - for no obvious reason. As neither of these represented safety issues, I assumed the car was just a bit old and these issues would be addressed as part of any service. Now I wonder if they are related to the sudden acceleration. The car is currently having a "health checks" run, but nothing has been found. I am reluctant to drive this vehicle if neither my dealer or Toyota can discern the problem. Overall we have been happy with this car and its performance, but this is an unacceptable "anomaly" as far as safety is concerned. Vehicle available for inspection.

Originally, Toyota issued a safety recall in 2014 to address the inverter defect in model years 2010 to 2014, an attempt to fix the problem on about 800,000 cars in the U.S. by modifying software that controls the hybrid electrical system. It was intended to fix the overheating problems. The inverter controls the power flowing between the battery and electrical motors. The inverter boosts the battery from 200 - 500 volts. They inspected my car saying it was in excellent condition. After, the software upgrade i had major problems. My original Prius battery with around 65,000 miles stopped holding a charge. My 12 volt battery also died shortly after. And my power outlets in my got super hot and spiratually worked off and on. And my driver seat gets hot when i turn on car even though seat button is off. I am sweating all the time. Plus, i have skin conditions and asma. I talked to dimitri, and they tried to charge me 1200for unnecessary repair. Also, failed to call NHTSA as promised

MY WIFE WAS DRIVING OUR 2014 TOYOTA PRIUS EASTBOUND ON BROADWAY RD., APPROACHING SOSSAMAN RD., IN MESA, AZ, ON FRIDAY DECEMBER 13, 2019 AT ABOUT 10:00 AM. WEATHER CONDITIONS WERE CLEAR AND DRY. SHE WAS ALONE IN THE VEHICLE, WEARING HER SEAT BELT, TRAVELING AT 20 MPH IN A CONSTRUCTION ZONE, WANTING TO TURN SOUTHBOUND ON SOSSAMAN RD., SOME 200 YARDS FURTHER EAST. CONSTRUCTION CREWS HAD REMOVED A PORTION OF THE ROADWAY, CREATING A DITCH, ABOUT 8 FEET LONG IN THE DIRECTION OF TRAVEL, ABOUT 10 TO 12 INCHES DEEP AND ABOUT TWO LANES WIDE. AFTER PASSING A CONSTRUCTION VEHICLE, SHE BEGAN HER TURN TO THE RIGHT AND RAN INTO THE DITCH. THE FRONT WHEELS OF THE PRIUS DROPPED INTO THE DITCH AND PROCEEDED ACROSS THE OPENING SOME 8 FEET, AT A SLIGHT ANGLE, AND IMPACTED THE CONCRETE ROADWAY ON THE OTHER SIDE, CAUSING A SUDDEN AND VIOLENT DECELERATION. DUE TO THE RELATIVELY SHALLOW DITCH, AND THE LOW GROUND CLEARANCE OF THE PRIUS, THE CAR RODE UP ON THE CONCRETE AND THE BRUNT OF THE IMPACT WAS ABSORBED BY THE UNDERCARRIAGE. THERE WAS SOME FASCIA AND BUMPER DAMAGE. THE WHEELS DID NOT IMPACT THE CONCRETE BARRIER. THE DRIVER'S SIDE AIRBAG DID NOT DEPLOY. I FILED A CLAIM WITH TOYOTA, AND THEY AGREED TO HAVE THE VEHICLE'S EVENT DATA RECORDER DOWNLOADED. ANALYSIS OF THE EVENT DATA RECORDER INFORMATION, PROVIDED BY TOYOTA, SHOWS A TOTAL TIME OF 123 MILLISECONDS FOR THE VEHICLE TO REACH A COMPLETE STOP FROM THE POINT OF INITIAL IMPACT, WITH A MAXIMUM LONGITUDINAL DECELERATION OF -14G'S (MY CALCULATION.) MY WIFE SUFFERED SEVERE NECK INJURIES. TOYOTA SENT ME A LETTER SAYING THE AIRBAG SYSTEM OPERATED AS DESIGNED AND WAS NOT AT FAULT (MY SUMMARY.) I DON'T SEE HOW THAT CAN BE, DUE TO THE EXTENT AND NATURE OF MY WIFE'S INJURIES. OR, TOYOTA'S DESIGN CRITERIA ARE AT FAULT AND DID NOT ANTICIPATE THIS TYPE OF COLLISION.

THE PRIUS VEHICLES COVERED ONLY BY WARRANTY ENHANCEMENT ZJB ARE SUFFERING THE SAME DANGERS FROM BRAKE BOOSTER PUMP FAILURES AS THOSE TOYOTA COVERED SAFETY RECALLS D0H AND K0L. THE MECHANICS STATE WHEN BUILDING THAT PART, A MACHINE CLAMPED THE 2 PARTS TOGETHER. BUT THE MACHINE DOESN'T WIPE IT DOWN BEFOREHAND AND ANY OIL WILL KEEP THE 2 PIECES SEPARATED, WHICH EVENTUALLY FORM A CRACK. THE EDR REPORT CLEARLY SHOWED THAT THE BRAKE OIL PRESSURE CHANGES DID NOT CORRELATE WITH THE PROGRESSIVELY INCREASING VEHICLE DECELERATION. THE ISSUE IS OCCASIONAL, RARE AND CANNOT BE REPRODUCED EVERY SINGLE TIME. THIS LEADS TO DISPARITY IN ASSUMED BRAKING DISTANCE FROM ROUTINE EXPERIENCE BY DRIVING THE VEHICLE AND ACTUAL BRAKING DISTANCE AT THE TIME OF FAILED BRAKING WHICH SUPPOSEDLY MAY BE AS A RESULT OF INVERTER ISSUES THAT ARE RESPONSIBLE FOR EXCLUSIVELY REGENERATIVE BRAKING AT LOW (<50 MPH) SPEEDS.

AFTER A MINUTE OR SO, THE COORDINATION BETWEEN THE STEERING WHEEL AND THE WHEELS RE-ESTABLISHED AND I COULD MANAGE TO DRIVE TO A NEARBY PARKING LOT. THE SITUATION HAPPENED AGAIN IN THE PARKING LOT BEFORE IT CAME BACK TO NORMAL. WE TOOK THE CAR TO A LOCAL TOYOTA DEALERSHIP AND LEFT IT THERE FOR 2 DAYS. HOWEVER, TECHNICIANS SAID THEY COULD NOT DUPLICATION THE ISSUE MEANING THEY COULD NOT FIND ANY PROBLEM WITH IT. THEY TOLD US THAT IT IS PROBABLY THE ELECTRICITY PART! THE HEADLIGHTS ON PRIUS ARE NOT BRIGHT ENOUGH TO SEE THE ROAD AHEAD OF THE CAR. THE ARE ON BUT SOMEWHAT DIM. OTHER DRIVERS CAN SEE THE LIGHTS, BUT THE LIGHTS ARE NOT BRIGHT ENOUGH TO ILLUMINATE THE ROAD IN FRONT OF THE CAR. THIS OCCURRED AFTER I TOOK THE CAR IN TO THE DEALER TO COMPLAIN ABOUT A SERIES OF ELECTRICAL RELATED PROBLEMS THAT ALL STARTED HAPPENING AT THE SAME TIME. THE DEALER REPLACED THE HEADLIGHTS AFTER THEY DETERMINED THE HEADLIGHTS HAD BURNED OUT. IMMEDIATELY, I NOTICED THAT DURING TIMES WHEN I WAS DRIVING AT NIGHT, THE NIGHT VISIBILITY WAS NOT WHAT IT HAD BEEN PRIOR TO PROBLEM FIRST OCCURRING. I HAVE TAKEN THE CAR BACK TO THE DEALER BU THEY SAY THERE IS NOTHING WRONG WITH THE HEADLIGHTS.

BEFORE I COULD PUT IT IN DRIVE FROM REVERSE IT STARTED ROLLING DOWNWARD ON ITS OWN. WHEN I WENT TO APPLY THE BRAKES, THEY WERE SQUISHY AND DID NOT ENGAGE. I TRIED SEVERAL TIMES TO GET THEM TO ENGAGE BUT THEY NEVER DID. I WENT TO PUT THE EMERGENCY BRAKE ON BUT IT IS LOCATED SO HIGH UP IT WAS DIFFICULT TO GET MY FOOT UP THERE QUICKLY. MEANWHILE THERE WAS A TRUCK PARKED ACROSS THE END OF THE PARKING LOT LANE SO MY CAR ROLLED INTO THE TRUCK CREATING DENTS IN THE FRONT DOOR PANEL AND REAR DOOR PANEL. THIS WAS OBVIOUS BRAKE FAILURE. I WENT TO THE LOCAL TOYOTA REPAIR AND DEALERSHIP AND SPOKE WITH A SHOP EMPLOYEE. HE TOLD ME THAT THIS CAN ACTUALLY HAPPEN WHERE YOU CAN BACK A CAR UP AND TURN IT AND IT WILL GO ON ITS OWN (BUT IT SEEMS HE IMPLIED THAT THE "READY" LIGHT MIGHT NOT HAVE ENGAGED) AND THAT HE "HAD SEEN IT HAPPEN HIMSELF". IF THE TRUCK HAD NOT BEEN THERE, MY CAR WOULD HAVE ROLLED DIRECTLY OUT INTO ONCOMING TRAFFIC ON THE MAIN THOROUGHFARE THROUGH TOWN WHICH HAD THE HUGE POTENTIAL OF CAUSING A MAJOR ACCIDENT WITH INJURIES. I DID TRY TO RECREATE THE EVENT IN A NONPOPULATED AREA IN A VERY SAFE MANNER AND WAS UNABLE TO DO SO. THAT EVEN SCARES ME MORE!!! AS YOU HAVE NO IDEA IF IT CAN OR WILL HAPPEN AGAIN. I SPENT $20,000 PLUS FOR A NEW CAR THAT I AM ALWAYS HYPER-CAUTIOUS IN BECAUSE NO ONE WILL FESS UP TO PROBLEMS. IF I SELL THE CAR IT WILL BE AT A GREAT LOSS WHICH I CANNOT AFFORD! IN THE INTERNET I NOTE THAT THERE WERE SIMILAR PROBLEMS WITH THE BRAKES, ESPECIALLY AROUND 2007 - MANY WERE DOCUMENTED BUT TOYOTA WAS NEVER TAKEN TO TASK RE: THESE CONSUMER COMPLAINTS AS I SUPPOSE THE CONSUMERS JUST COULDN'T "PROVE" THEM. MUSTY SMELL IN VEHICLE, SICK W/ RESPIRATORY SYMPTOMS, INCLUDING DISTRESS AFTER USING AIR CONDITIONING OR HEATING SYSTEM OR DEFROSTERS. I BROUGHT MY CAR TO DEALER FOR 5000 MILE SERVICE AND MANAGER TOLD ME THE "THERE IS MOLD INSIDE THE AIR CONDITIONER AND HEATING AND ALL VENTED AIR SYSTEM. THIS IS THE WAY THE NEW DESIGN IS, THE AIR SYSTEM IS CLOSED OFF AND THE MOISTURE LAYS IN THERE GROWING MOLD." HE SAID "YOU HAVE TO OPEN THE WINDOWS, IF YOU ARE SENSITIVE, OR HAVE IT CLEANED EVERY WEEK TO KEEP THE SMELL OUT." HE SAID " THAT IS A CHARGE NOT COVERED BY YOUR LEASE, I'LL TAKE THE DASH OFF AND PUT A NEW FILTER AND FLUSH THE AIR HANDLING SYSTEM OUT FOR YOU, THIS TIME. IT USUALLY COST OVER $250.DOLLARS. YOU CAN DO IT YOURSELF BY SPRAYING A WHOLE CAN OF LYSOL BEHIND THE CABIN FILTER EVERY WEEK". MOLD IS KNOWN TO BE DEADLY AND THIS SHOULD NOT BE AN ALLOWED DESIGN. I HAVE SOUGHT MEDICAL ATTENTION FOR THIS PROBLEM AND NOW REFERRED TO A PULMONOLOGIST FOR SPECIAL TEST. I DEFINITELY WOULD NOT HAVE OBTAINED THIS CAR IF THE DEALER HAD DISCLOSED THIS KNOWN PROBLEM. BAD. *TR

SHORTLY AFTER PURCHASING MY 2014 TOYOTA PRIUS IN JUNE 2014, I STARTED NOTICING A MOLDY SMELL COMING FROM THE AIR CONDITIONER. THE HOTTER OUTSIDE IT GOT, THE WORSE THE SMELL INSIDE GOT. I TOOK THE CAR TO THE DEALER TO DISCUSS THE PROBLEM WITH THEM. THEY INFORMED ME THAT TOYOTA SAYS THIS IS AN "ENVIRONMENTAL ISSUE" AND CAN HAPPEN IN NEW CARS AND IS THEREFORE NOT WARRANTIED. THE SERVICE MANAGER INSTALLED A CHARCOAL AIR FILTER IN THE CABIN AIR FILTER AND SAID THAT IT WOULD HELP ELIMINATE THE ODOR. IT DID NOT. IN FACT, THE CHARCOAL AIR FILTER REDUCED THE AIR FLOW INTO THE CABIN WHICH CAUSED THE AC TO NOT COOL. I HAD THE SERVICE DEPARTMENT REPLACE THE CHARCOAL WITH AN ORIGINAL AIR FILTER IN SEPTEMBER AFTER COMPLAINING THAT THE AC WASN'T COOL. THE SMELL IN THE AC PERSISTS. IT'S NOT JUST A BIT WHEN THE CAR IS TURNED ON. NO MATTER WHAT SETTING I PUT THE AC ON, IT DOES NOT HELP (FRESH AIR OR RECIRC). WHEN IT RAINS OR WHEN IT IS REALLY HUMID OUTSIDE, THE CAR SMELLS LIKE A SWAMP (EVEN BEFORE IT IS TURNED ON). THERE IS ALSO HUMIDITY INSIDE THE CAR WHEN OPENING THE DOOR. I'VE ASKED MULTIPLE TIMES TO HAVE THE EVAPORATOR REPLACED BECAUSE I FEAR THAT IT IS NOT DRAINING AND CAUSING MOLD AND MILDEW TO FORM. BREATHING IN THE MOLD IS A SERIOUS HEALTH CONCERN AND TOYOTA WILL NOT AGREE TO ANY KIND OF FIX. I'M ALSO HAVING OTHER VARIOUS HVAC PROBLEMS SUCH AS CONTINUOUSLY FOGGING WINDOWS AND A DRAFT FROM THE DRIVERS SIDE DOOR. THE CAR DOES NOT HOLD HEAT AND IS VERY COLD DURING THE WINTER MONTHS. *TR

THERE IS A VISIBILITY ISSUE ON BOTH THE RIGHT HAND AND LEFT HAND SIDE OF THE WINDSHIELD. THE RIGHT HAND SIDE HAS A GLARE/BLURRY EFFECT WHEN MAKING TURNS. THE LEFT SIDE HAD SOME SORT OF CIRCULAR EFFECT THAT NEGATIVELY EFFECTS MY VISION. THE DEALER ATTEMPTED TO TREAT THE WINDSHIELD WITH RAIN X AS SOME SORT OF FIX. THIS DID NOT HELP. THIS WAS A VERY UNPROFESSIONAL FIX. IN FACT, I BELIEVE THE RAIN X MADE THE PROBLEM WORSE. I DON'T REMEMBER THE ISSUE OCCURRING UNTIL A FEW WEEKS AFTER THE INITIAL PURCHASE OF THE VEHICLE.

ODI #10668230AIR BAGS

I PURCHASED THE VEHICLE BRAND NEW FROM A DEALERSHIP. WITHIN A FEW WEEKS, I NOTICED THAT WHEN MY HUSBAND SAT IN THE FRONT PASSENGER SEAT (300LBS) THE NOTIFICATION LIGHT SAID "PASSENGER AIR BAG ON". WHEN MY STEPSON SAT IN THE FRONT PASSENGER SEAT THE LIGHT SAID "PASSENGER AIR BAG OFF". I TOOK A PICTURE TO SHOW THE NOTIFICATION LIGHT AND THE SIZE OF MY 15 YR OLD STEPSON 5' 10" & 175LB-FULLY ADULT SIZE. I CALLED THE DEALERSHIP AND THEY TOLD ME THEY HAD "NEVER HEARD OF THE PROBLEM" AND SAID IT WAS JUST THAT THE PASSENGER AIRBAG NEEDED TO BE RECALIBRATED. AFTER DOING SOME SEARCHING, I REALIZED THAT THE 2012-2013 MODELS HAVE ALREADY BEEN RECALLED FOR THAT EXACT SAME REASON. I AM VERY CONCERNED THAT THE PROBLEM HAS NOT BEEN FIXED WITH TOYOTA, AND THAT NEW CARS ARE BEING SOLD WITH THE SAME PROBLEM. NEEDLESS TO SAY, I AM NOT HAPPY WITH TOYOTA RIGHT NOW AND THIS IS A HUGE SAFETY PROBLEM. I ALSO HAVE CONCERNS ABOUT THE BACK SEAT BELTS NOT LATCHING PROPERLY WHEN A CAR SEAT IS BEING USED, BUT THAT'S ANOTHER STORY.... *TR

ODI #10664218TIRES,WHEELS

WE PURCHASED A NEW TOYOTA PRIUS THAT CAME WITH YOKOHAMA ALL SEASON RADIALS. BECAUSE THE TIRE LIGHT CAME ON, I CHECKED THE AIR PRESSURE IN THE TIRES AND SET THEM TO THE PSI AS RECOMMENDED AT ABOUT 1000 MILES. THERE WAS A MINOR IMBALANCE OF 3-4 PSI IN SOME TIRES, BUT NO SERIOUS UNDER INFLATION. WE NOTICED SOME MINOR ROAD NOISE AND VIBRATION. WE HAD A MECHANIC INSTALL MUD FLAPS AND INSPECT THE TIRES, WHEELS, AND BRAKES AT ABOUT 1500 MILES TO SEE IF THERE WAS A VISIBLE REASON FOR VIBRATION. THERE WAS NOTHING OBVIOUS. WE WERE CONCERNED ABOUT THE ROAD NOISE AND ABOUT 2000 MILES WE TOOK THE CAR BACK TO THE TOYOTA DEALER WHO INSPECTED THE TIRES, CHECKED THE AIR PRESSURE, AND DECLARED THAT THEY DIDN'T SEE ANYTHING WRONG. AT ABOUT 3000 MILES DRIVING AT A SLOW SPEED (35 MPH OR LESS) IN A SCHOOL ZONE, WE HAD A COMPLETE BLOW OUT OF THE LEFT REAR TIRE. FORTUNATELY, THERE WAS A GRASSY SHOULDER AND NO TRAFFIC, SO WE HAD NO INJURIES OR OTHER DAMAGE. THE TREAD APPEARED TO SEPARATE AND THERE WAS A SIDEWALL TEAR STARTING AT THE TREAD. WE RETURNED TO THE TOYOTA DEALER AND ALSO SHOWED THE TIRE TO THREE DIFFERENT TIRE SHOPS (ALL THREE SELL YOKOHAMA TIRES). THE TOYOTA DEALER DID NOT OFFER AN OPINION OTHER THAN TIRE DEFECT - SO WE NEEDED TO DEAL WITH THE MANUFACTURER. TWO TIRE SHOPS THOUGHT IT WAS A DEFECT (EVEN THOUGH THEY SAID IT WAS A HASSLE TO GET A REPLACEMENT PAID FOR BY YOKOHAMA). ONE SHOP REPORTED IT WAS A DEFECT UNLESS THE TIRE WAS UNDER INFLATED, BUT THEY DIDN'T SEE ANY WEAR PATTERN THAT INDICATED UNDER INFLATION. BECAUSE OF THE BLOWOUT AND A SECOND SUSPICIOUS TIRE OF THE FOUR, WE REPLACED ALL FOUR WITH A DIFFERENT BRAND. AS FAR AS WE CAN TELL, THE YOKOHAMA TIRE(S) WERE DEFECTIVE AS NEW ON THE CAR. *TR

Chart Analysis

The blue line shows 227 total NHTSA complaints filed over 13 years, with the worst year being 2024 (31 complaints).

The amber reliability line averaged 15/100 across the vehicle's life — dropping as low as 10 in 2014 when complaint severity peaked.

The recalls (red dashed lines) were issued in the same year or before complaints peaked, suggesting the manufacturer identified and acted on the defect relatively quickly.

Red dots on the amber line flag 9 years (2014, 2015, 2016, 2017, 2018, 2019, 2020, 2022, 2023) where complaints included crash events — these are the highest-severity incidents in the dataset.

Hybrid Battery / HV Battery Pack
COVERAGE:10 years / 150,000 miles from original sale date (federal minimum for HV battery in ZEV states; 8 years / 100,000 miles in non-ZEV states)

Federal emissions regulations require Toyota and Lexus to warrant the hybrid battery system for a minimum of 8 years / 100,000 miles nationally, and 10 years / 150,000 miles in California and states that adopted California emissions standards. The HV battery, battery ECU, and hybrid control module are covered.

HOW TO CLAIM: Coverage is federally mandated. Visit any Toyota or Lexus dealer. If your HV battery warning light is on or fuel economy has degraded significantly, request a hybrid battery health check. No special program code needed — it is covered under the standard federal emissions warranty.

Coverage eligibility depends on your VIN, original sale date, and current mileage. Always verify with an authorized dealer or manufacturer customer service — programs may have been updated since this database was compiled.
