The contact is an attorney concerned about a crash involving a 2022 Toyota Rav4. The attorney stated that the vehicle had a comma.ai device installed on the vehicle. The attorney stated that while 2 couples were occupying the vehicle and driving at night on an expressway, there was a police vehicle on the side of the road responding to a separate vehicle crash. The driver of the vehicle may have been asleep, along with the front passenger, while relying on the comma.ai device to operate the vehicle. The vehicle failed to engage the brakes in order to avoid crashing into the police vehicle, and as a result, the Toyota Rav4 crashed into the rear of the police vehicle. The vehicle speed was unknown at the time of the crash. The owner of the vehicle rear-ended a Louisiana state trooper. Air bags deployed in the front and on both sides of the vehicle. The vehicle was towed to a tow yard and then to a Progressive Insurance lot. No warning lights were noticed. All occupants were wearing seat belts. The driver of the vehicle sustained concussion injuries, and the front passenger occupant sustained arm and leg injuries. The two rear occupants succumbed to their injuries at the hospital. No other injuries were reported as the police officer was not occupying the vehicle during the impact. The comma.ai device failed to avoid the f vehicle in front. The manufacturer was not notified of the failure. The failure mileage was unknown.

My infotainment screen went black while I was driving at night and it will not come back on. There was no warning. I took it to the dealer and they said that the system had failed. I saw on line that there was a recall for this very issue but my car is a '22 and not included in the recall, but I think it should be. This poses a safety hazard and I don't believe that this should happen under normal circumstances for a car of its age and relatively low mileage. I think this model year should be included in the recall and that Toyota should pay for the repair, which is quite expensive. The receipt on the diagnostic says the radio works, but it does not. It goes on when you push the power and immediately goes off.

We were traveling across country from Michigan to Arizona. We were 50 miles outside Tucson, AZ on [XXX]. Traveling 80 mph in a 75 mph zone, in passing lane and felt like car exploded and all airbags deployed and we came to a quick stop. No major injuries to either of us. One vehicle incident, did not collide with anything or hit any debris. It was a clear day, road conditions good, no noises, lights etc. My husband was able to keep from rolling and we came to a stop with many semi's etc flying by, very scary. When we were able to safely get out and call 911, there was significant drivers side front end damage around tire area and inside near axle. Looked like something broke inside car, sheered off bottom of tire. Police arrived and had to push vehicle on to shoulder of road. He assessed scene and thought it had to be a mechanicla failure as there was no other vehicle or debris on site. We did contact our insurance company and subsequently it was totaled. We also have Toyota investigating what the cause was if it was a faulty part or mechanical failure. We have not heard back but did receive the "recording of time of crash". I was driving home in the rain for about 75 minutes and when I arrived in my driveway I saw steam coming from the front grille area. It wasn't due to a high engine coolant temperature so I decided to carefully open the hood and check for the source. I saw water inside the bottom of the engine bay behind the radiator steaming from the heat of the engine. A few seconds later, the engine stalled and the following message appeared: "Engine Stopped, Steering Power Low" which came on with a check engine light, red battery light, and steering light. I used my scan tool and obtained the code: P0171, System Too Lean. After doing research, I've come up with a conclusion to the potential cause. Starting with the 2022 model year RAV4 non-hybrid (and onwards to the 2025 model year RAV4), the air intake inlet was redesigned from the prior one in the 2019-2021 RAV4 non-hybrid models, as seen in numerous comparison images online. This new design presents a dangerous fault. When driving in rainy conditions, especially at higher speeds with mist kicked up from cars in front, the air intake will take in water. The water will flow to the air filter and then reach the Mass Air Flow sensor (MAF). The MAF sensor monitors the amount of air entering the engine and sends signals to the car's computer to help determine how much fuel to send to the engine to maintain an ideal ratio for optimal efficiency. Considering that water is getting to the MAF sensor, it thinks that the water is excess air, so less fuel is sent to the engine as a result. Since I came to a stop in my driveway, the engine is now at its idle speed and only commands a small amount of fuel to remain running. Now, when combined with the water, that lower fuel flow is practically cut off, leading to the stall. This happened to me before when driving in the rain, but in more dangerous conditions in stop-and-go traffic. Toyota must follow up on T-TT-0763-24 because it has been well over a year since their initial investigation.

Driving up a slope of about 20-30 degrees gradient of a straight lane going into a private parking lot. Braked and decided to make a 3-point turn to get back out. There was no traffic in the lane. Put gear on reverse while on the slope with the vehicle pointing in the up direction of the slope. Vehicle suddenly surge backwards, accelerating. Vehicle sped down the slope and crossed a street. Could not stop. Vehicle was stopped only after placing gear into park from reverse, and after jumping curb across the street. Fortunately, there were no cross traffic in either direction on the street, otherwise vehicle could have been T-boned. Tire wall on the rear passenger side busted. AAA was called to replace tire with spare. There were no warnings on the dashboard or any symptoms anywhere else, although the vehicle had all the safety features.. The next morning, vehicle was taken to Toyota of Carlsbad. Left there for diagnosis and tires change. Vehicle is left with Toyota of Carlsbad until this issue is answered/solved. Told by service manager of Toyota of Carlsbad to call Toyota USA at [XXX] to report incident by customer, and request Toyota USA to send expert from Toyota to diagnose issue. I attempted to remote start my 2022 Toyota RAV4 Hybrid from inside my home using the Toyota mobile app. I received a confirmation notification stating that the vehicle had started and would automatically shut off in 10 minutes. Approximately six minutes later, I exited my home and approached the vehicle, which was not running. After unlocking and entering the vehicle, the interior temperature confirmed that the vehicle had not been running. I attempted to start the vehicle by pressing the brake pedal and pushing the start button. No dashboard lights illuminated, no warning messages appeared, and the vehicle was completely unresponsive. I attempted this process two additional times with the same result. After exiting the vehicle and closing the driver’s door, I attempted to lock the vehicle using the touch sensor on the driver-side door handle. The vehicle would not lock. I attempted this a second time and it still would not lock, indicating a complete loss of electrical power. This failure occurred in cold weather conditions and involved the hybrid battery and associated electrical system. The loss of power created a safety risk because the vehicle could not be secured and could have left me stranded in cold weather. Later that day, I contacted my insurance company for roadside assistance. A tow truck arrived and jump-started the vehicle using a portable jump box, which allowed the vehicle to start and the battery to recharge. A Toyota service appointment is scheduled to inspect the vehicle. The issue has not yet been confirmed by a dealer. The vehicle has not been inspected by the manufacturer or other representatives. Only the roadside assistance tow truck driver responded. The vehicle and battery are available for inspection upon request.

At the car wash while slowly pulling (1mph) into a parking space to vacuum the vehicle the car unexpectedly accelerated. The brakes failed and the car continued to accelerate until forced into park and turning off the vehicle. During the acceleration there was a grinding noise. This acceleration caused the car to hit the front curb and cement pilings with poles supporting the roof and vacuum. There were no warnings or lights being presented. If not for the curb and being off the road anyone in the car would at the very least been injured and possibly hitting another auto. The dealer is claiming no none issues as described. The auto was towed to the body shop to inspect and repair. We were told by the body repair shop the front suspension failed. The auto was great prior to this incident. Afraid to drive for fear of a repeat especially due to others on the internet claiming the same issue has happened and often continued to have a repeat acceleration incident. No one has found resolution from Toyota. Approximately $7,000.00 worth of damage and our insurance has now gone up.

After driving for 35 minutes (in the rain), I pulled up to work and let someone out. While car was sitting there in park, it bucked in the back, I then put it into park and almost immediately, it shut off with this on the display "Engine Off, Steering Power Low, Do Not Drive". My lights and windshield wipers remained on. I tried to turn it back and and it shut off again. I waited a few minutes, turned it on and went to pull out onto main road and the car shut off again with the same message. Thankfully I was turning onto the main road so close to shoulder and able to keep my car pretty close to side of the road. I called the Toyota dealer who told me not to drive it and it would need to be towed. I towed it to the closest dealer and when they checked the codes (the next day), they said there were no codes and they could not replicate the problem. As a note, the rain was off and on, nothing that was worse than anything I have driven in previously. I drive main roads and did not go through any large puddles or anything similar. This is a major safety issue and I am terrified that it could happen while driving on a highway where I may not have time to get out of the way of other traffic (car literally stopped with no option of continuing to coast). I am also shocked that there were NO codes triggered as there was obviously some event and that the car is only one month outside of warranty.

After pressing the close button at tailgate or driver's seat with tailgate open, it will lower an inch then beep several times and lift open again. Toyota dealer claims this is a known issue and the solution is to "reset" the tailgate, which they would do for me. Resetting immediately failed, and the issue persisted. Toyota service department said, "there is a button on the hatch that will reset it. you will hold that button until it does a sequence of beeps (beeeeeeeeepp, beep beep beep) then let go an press it again like you would want to close it and you should be good to go! :)" (sic). This solution worked once and immediately failed the second time I attempted to close the tailgate. This is a daily issue that was replicated at the dealer but not fixed.

It’s been raining for about 4-5 hours by now, I drove to church (1 hour drive) left my car outside in the parking lot in the rain while I was there. Then drove home (another 1 hour) and when I came home just now I sat in my car parked with the engine on for about 5 minutes when suddenly the car started shaking and then it turned off with a message “Engine Stopped - Power Steering Low, ignition on” on my 2022 Toyota RAV4 XLE. I think this issue needs to be investigated ASAP as it is a big safety concern, what if it happened when I was driving on the highway in the rain, at night? My Google and Reddit search revealed that a lot of people are experiencing this issue and this is a place to help start an investigation. I had a full inspection on my car at the Toyota dealership just a few weeks ago and everything including the battery was fine.

On [XXX], I opened the unblemished retractable glass roof of my 2022 RAV4 XSE. The rear windows were rolled down approximately 4 inches. No other cars/people were in front, next to, or behind me. As I accelerated to approximately 35 mph, I heard an explosive blast and peered upward. I noticed that the retractable glass roof panel had completely shattered. Shards of the roof rained on me as I drove home. Once home, I closed the roof just enough to secure my vehicle. The motor /closing mechanisms were operable. Glass shards were present in the tracks as well as on the top of the intact remainder of the glass roof. Glass shards were present in both the front and back seats of the vehicle. Upon inspection, there were NO visible pits/holes in the shattered roof; however, the roof was almost completely displaced from the edges of the moonroof’s seal/frame (see pictures). I contacted my Insurance to file a claim, and my SUV was towed to Toyota. 6/24 Toyota’s estimator left a voicemail stating that the glass roof was being assessed. I left a message asking for the cause of the damage. On 6/25, Toyota called and stated that the roof panel was being replaced. The estimator was unable to determine the cause of the breakage and mentioned that this was the first time they had to replace a panoramic sunroof. No other outside agencies (only Toyota Carlsbad Collision) inspected and/or addressed the damaged glass roof. A faulty design and /or improper manufacturing of the glass was never addressed or considered. 6/27 Toyota billed my insurance $1971.52 minus the $500 deductible I was required to pay per the terms of my insurance. *** It should be noted that there are a number of RAV4 cases involving sudden glass roof explosions. These lawsuits allege that glass sunroofs of RAV4s shatter under normal driving conditions. In fact, on May 3, 2024, a class action suit brought before the US District Court in California, rejected Toyota’s rebuttal, and the lawsuit is moving forward. I had another incident this week where I was stranded in a parking lot and had to call Triple A to get my car running properly again. They tested the battery and belts and all were good. 6/15/23 @ 18,357 miles the car was given a four wheel alignment at the owners request. Due to loud tire noise and visible cupping on the right rear inside edge, it was clear the car's wheel alignment was wrong. The dealer's before and after alignment is attached. When presented the before and after, I noted RR Camber was.-2.3 vs. a range of -1.8 to -0.3 degrees. The reading was unchanged from initial. They advised me the camber cannot be adjusted and is "fixed" from the Factory. The form also read "One of more value are not within specification. Tire wear, handling and safety problems may result". But they had no remedy. I escalated at Dealer. Dealer opened a ticket with Toyota to look at my car. Of note, my Sister in Law's RAV4car has the same issue. I felt I had made my concerns clear and Toyota Corp would address. 2/1/24 At 30,345 the problem is still unresolved and dealer gave me Toyota Corp number and I opened case 240201000061. 4 calls later I am told to bring my car to dealer 2/20/24. Research shows TSB 0042-14 for earlier RAV4. My safety problem is not new to Toyota, it is just a later model. When I told Toyota Corp their response from 6/15/23 to 2/1/24 is unacceptable, they took no ownership. When I asked if I should drive an unsafe car they said it is my choice. When I advised them of the language from the dealer said the tires were unsafe, they said the Dealer is independent from Toyota. In short, the dealer is "Independent". Toyota has a problem with their design using a static camber strut which causes premature wear which can lead to a blow out as shown by forums. Toyota is only interested in getting the car to 36,000 miles and little focus on safe design. I was surprised not to see this already listed. Perhaps it exists under another year? A company Hard Race has made adjustable rear Toe Arms for the RAV4. The dealer confirmed same.

I called for a pickup instead and was not able to start the car until the next day. It was so terrifying, and I was fortunate that the accident did not occur. What would happen if it were to happen as I or other RAV4 owners were in the middle of the road or nowhere for help? On 11/15/23 I went to Byers Toyota Dealership and asked them to check my car due to the gas smell inside of the car when driven and outside of my car when parked for a longer period of time. The smell is coming from the rear. The car also has a very strong gas smell when the tank is open to pump gas. I did not notice any leaks. On the visit on 11/1523, they fixed the leaking evaporator according to your bulletin from May 2023 to alleviate the burning oil smell coming from my car’s HVAC heater. They also checked the fuel pump and told me that I have the correct one and it should function as designed. The service advisor told me that the gas smell is exhaust fumes and it is normal for Toyota type of engine. They did not have any explanation for the fuel smell when my car is parked. Since the smell was still present, I scheduled another appointment at the dealership on 1/3/24, they kept my car overnight but they were not able to duplicate any of my concerns. I did not contact Toyota Customer Care because they can be very unpleasant if the dealership cannot duplicate the problem.

I decided that I would open up a claim with Toyota, they have not even tried to advocate for my side; in fact no one asked for my side of the story until I said no one had asked me. It is not under warranty because its the moon roof and it is from "ware and tare" but no where does it say glass is not covered, and then they said that it would be covered only if proven from a manufacture/car fault or malfunction. My moon roof has no point of impact, hence the thought it was caused by pressure. Then I looked it up, this has happened to 100s of people on their rav4 2021, some of which got it covered by Toyota. I have been talking to a case manager (after weeks of trying to reach one) and he basically said that even if I send them these reports of it previously happening he won't bother looking at them; and that the specialist team are bias to Toyota so they will also say its outside causes. I have been pushed around, looked down on, and not advocated for. They also won't do ANY repair on my car until Toyota says okay, or I file an insurance claim, so they have had my car for 2 weeks doing nothing on it. Not only that they upsold me on the moon roof, encouraged me to do it, my $500 upgrade is now costing me $5000 dollars to fix. This is a dangerous reoccurring problem, what if my shade wasn't closed and the glass hit myself or passengers, of if cars behind me ran over glass chunks and caused flat tires or window chips etc. In addition the way the situation has been handled makes me and my Toyota loyal family not want to buy a Toyota again!!

The Engine Coolant Bypass Valve is a known issue across the 2019-Present RAV4 generation. We have taken our RAV4 to the dealership numerous time for the “Maintenance Required Soon” message that comes up when the car starts. The dealer always says that is is just a reminder for us to get our vehicle serviced at 20,000. The service interval message is supposed to only come on at every 5,000, not every 3,000. We took it by our local auto parts store, and the man that works there has a 2019 RAV4 that has the same problem. They had to take theirs to the dealer and have the Coolant Bypass Valve replaced. If not addressed soon, it could cause the engine to overheat. The man at the auto parts store said it doesn’t hurt to drive it but it is best not to prolong it. I was driving on I-91 in stop-and-go traffic when, all of a sudden, my RAV4 stalled. A warning chime sounded and the battery light, check engine light, and power steering light all illuminated. In the 4.2 inch screen in the gauge cluster, the following message displayed: "Steering Power Low"..."Shift to P". A second message displayed and said: "Engine Stopped"..."Shift to P". I attempted to restart the vehicle up to 8 times and it finally restarted. I quickly drove the car to the shoulder and out of the way of traffic. There were three of us in the car (myself and my two kids) and we almost got hit two times because visibility was poor due to rainy conditions and mist from passing cars. Throughout all this, I had the Auto Start/Stop feature disabled, so the car should not have shut off on its own. We had to get the car towed because we were worried that the problem would reoccur. We hope the problem could be rectified by our local Toyota dealer, because we do not want to drive it again in fear of the problem reoccurring while on the roadway. We are reporting this to bring awareness to this issue, and to make sure nobody else will have to face this situation and potentially be put in harm's way. (The attached photo is a general picture from somebody else, but my car had the same EXACT warning lights and messages. We could not take a picture because we were focusing on trying to get the car to turn on and move it to the shoulder by any means possible).

I purchased the car last July. Up to December, the car performed fine. Starting in December when I used the car heater repeatedly due to subzero temperatures, I noticed a burning smell coming from the vents. I was afraid that the engine or car would catch fire. I called the Tansky Toyota dealership and was told to bring the car in. They checked the car and I was told the smell was identified as engine coating which is burning off and it may take up to 4-6 month for the smell to go away. When I asked about the effects of the burning smell on my health, the assigned service adviser told me that “if Toyota put it there, it is fine’. In February, the smell in the enclosed car cabin got so bad that I felt nauseous, my eyes were burning, and I had metallic after taste in my mouth. When I took my car to the Tansky dealership, I was told that what I am smelling is a new car smell. Just to prove it, the assigned service advisor asked random dealership employees to sit in my car and to confirm that the car has a so called “new car smell”. When I asked if they could do something about the smell I was told to take the car to an independent car shop. The dealership was dismissive of my concerns. I had new cars before and none of them produced the so called “new car smell” when I used the heater. I contacted Toyota and requested an independent inspection. The inspection was scheduled at the same dealership and when I received the report, it basically said that the inspector could not duplicate the problem so my concern was dismissed. Unfortunately, during independent inspection, my car was not tested in subzero temperature to replicate the problem. My main concern is the effect of the burning chemicals on my health and possibly ability to operate the car safely. No one was able to tell me what kind of chemicals are in the engine coating. At the time of purchase, I was not told that the car has engine coating which will gradually burn off via heater vent into the cabin.

Toyota (and Toyota of Boardman) is purposely overstating fuel capacity on their vehicles. So this is unsafe as drivers don’t the actual amount of gas before the run out and could lead to unexpected stops. This makes it impossible to determine the actual amount of gasoline used, which would also then make it impossible to know the total amount of air pollutants their cars omit. This could easily mean that Toyota is over stating their EPA estimated miles per gallon, the document attach shows that Toyota is aware that their gas tanks have known engineering defects however, they will only address the issue if a user owns a 2019 through 2021 model (RAV4 Hybrid). Also, they said they will only address the problem if the person cannot put more than 9.8 gallons that would mean that they will only address this issue if over 32% of the gas tank is unusable. If I would of known the truth I would have reconsidered buying my 2022 Toyota RAV4 XSE Hybrid.

Tire size:225 65/17 While driving the side of a brand new tire blew out. We had three passengers in the vehicle that could have been in danger if we were going at a greater speed. We took the new tire in and was told by the auto shop that it could not be repaired. We had to purchase a new one and I have the invoice. I try to resolve this with Firestone but according to them, the tire was not under warranty. The new tire only had 5000 miles and it is a tread of at least 60,000 miles.

At 2:05 pm on March 21, 2023 my car suddenly lurched forward and to the left as I was pulling into a parallel parking place. The vehicle was operating in electric mode on "ECO" with the the gearshift in "D" at the time, but I heard the starter motor kick in and the engine rev up simultaneous with the sudden lurching. The lurching was so fast that it threw my head against the head rest. The vehicle jumped an 8 inch concrete curb leaving a skid mark on the right side and traveled 10-12 feet across a side walk and into dirt landscaping. Upon slamming on the brake, the car came to rest and the engine died. There were no prior incidents. The odometer read 4648 at time of incident. The vehicle has not been inspected by the dealer or police. No injury to the driver or pedestrians occurred.The incident has not recurred since yesterday's incident. After the incident, the tire pressure indicator light appeared on the dashboard. The weather at time of the incident was 72 degrees and cloudy. The vehicle had just completed a short 1 mile drive at low speed. There were no electrical transformers nearby but a ReSound GN hearing aid recharge battery case was sitting on the passenger seat at the time of the incident.

The glass busted completely, and those glass particles causes inconvenience to the other vehicle on the high way. I reported this incident to the car dealer but they were so rude to me, and said that car is not designed for the 75mph speed. Ended up convincing myself that RAV4 is not safe to drive anymore.

Hello, I am concerned about the windshield on our new RAV4. It was chipped and the glass cracked from the passenger side to the drivers side of the car. Currently we are waiting for the car insurance to cover the cost of repair for the windshield. For safety reasons you cannot clearly see out of the windshield because of the crack. The problem cannot be reproduced currently. the problem was inspected by the insurance company representative (SAFELITE) and we are still waiting on the new windshield because apparently there is a hug backlog there was no warning when the windshield cracked. But seemingly cracked when there was no warning, my wife was driving the car when something hit the corner of the windshield (some rock or road debris)

When breaking on dry roads no matter my speed, or how hard I press the breaks, the ABS system kicks in momentarily making my vehicle feel like it’s accelerating vs. stopping. This is very intermittent. Tends to happen more on rough city roads, however it really doesn’t matter. I understand ABS is really suppose to work for wet or snow roads in slippery conditions. In my experience ABS has always worked with a rapid pulse. This is certainly not happening. The breaks literally feel like one big skip.

I purchased a brand new Toyota RAV4 Hybrid XLE on June 30, 2022 and on my first drive, 6.4 miles from the dealership to my home, the the windshield developed a crack on the driver's side. It is available for inspection. Both Toyota and Safelite say they are having supply issues with the windshield and of Sep 1, 2022 neither Toyota or Safelite have replacement windshields for the RAV4. The crack affects vision for both the driver and those outside the vehicle. The problem has been confirmed by the local dealer, Toyota of Cedar Park, TX and by Safelite of Austin Texas. The windshield has NOT been inspected by the manufacturer, police, Amica insurance, or any others There were no warning lamps, messages or symptoms of the problem prior to failure or after the failure.

The vehicle without warning will lose all power; "Accelerator System Malfunction Output Power Reduced Visit Your Dealer" and "Pre-Collision System Malfunction - Visit Your Dealer" are the warning messages that appeared when power was lost. The "Check Engine" light also illuminated as well as a collision error light appeared. The vehicle has done this twice; the first time, the dealer found a recall which was a software update. However, today the vehicle became unresponsive again with the same error messages as before; we were lucky no vehicle hit us from behind since we were turning towards the frontage road to access the highway. The first time this happened, my wife was entering the ramp towards the expressway when the vehicle lost all power and she had to go to the side. Vehicle is now again at the dealer but cannot be serviced until Monday; however, this needs to be addressed since it is a safety concern. On the Toyota App, it is also listing a hybrid malfunction; this needs immediate attention to avoid a catastrophic event. Losing all power with no warning is completely unacceptable.

I am certain that it is a defect of the windshield in the RAV4's. I am not the first person to complain about the faulty windshield on these models and truly believe that is very unsafe. This needs to be investigated as it can cause a car accident if cracked on the highway.
