Vehicle went to turtle mode 3 separate times at highway speeds forcing a stop. Each time it was towed to the dealer. First two times little was done per the report and was returned. On the third time it has spent 7 weeks for a repair on the transmission not a replacement. The dealer also needed to order a replacement exhaust clamp that took 5 plus days to receive and install. Service department wanted me to take delivery prior to the clamp replacement and I refuse due to safety concerns of CO. Without our AAA membership we would have spent nearly $1000 on those 3 tows.

ODI #11742297ELECTRICAL SYSTEM,UNKNOWN OR OTHER,ENGINE

I am a full-time power wheelchair user with Multiple Sclerosis and rely on this 2024 Chrysler Pacifica wheelchair-accessible van as my primary means of transportation. Since purchase, the vehicle has repeatedly experienced dangerous loss-of-power events while being driven. Immediately before the failure occurs, warning beeps sound and multiple warning lights illuminate, including Check Engine, ETC, Park Brake, FCW, ESC, and MIL. After these warnings appear, the vehicle loses acceleration and enters a reduced-power/limp-mode condition. Pressing the accelerator does not result in normal acceleration, creating a serious safety hazard in traffic. Most recently, on June 4, 2026, this occurred approximately six times during a single trip. During one incident, the warning lights illuminated, the vehicle lost acceleration, and we were forced to pull over with emergency flashers activated in a dangerous location. A police officer came upon us, stopped to assist, and advised that the location was extremely dangerous. Because I use a power wheelchair, I cannot safely transfer into a standard vehicle, taxi, rideshare, or other transportation. We explained to the officer that if the van were to be towed, I would effectively be stranded because there was no practical way to transport me in my power wheelchair. The condition later cleared and we were able to continue. The vehicle has been inspected multiple times by Chrysler service departments and DriveMaster, the local mobility provider working with Freedom Motors. The problem has been reported repeatedly but is intermittent and has not been reproduced during inspections. Despite multiple repair attempts, the problem persists. The vehicle is available for inspection. These recurring loss-of-power events create a serious safety risk for myself, my family, and other motorists.

ODI #11739439AIR BAGS

I own a 2024 Chrysler Pacifica with open safety recall FCA 06D / NHTSA 26V-189 for side curtain airbags/ejection mitigation. I contacted the dealer approximately two months ago to schedule the free recall repair. The dealer advised there is a parts delay and the repair is still approximately two more months out. I checked for earlier repair options and was told no dealer can complete it sooner due to parts availability. This is a family vehicle used to transport children, and the recall involves side curtain airbags that may not retain sufficient pressure during deployment, increasing risk of injury or ejection. I am requesting NHTSA review because the manufacturer appears unable to provide the safety recall remedy in a timely manner. I am also seeking guidance on whether the vehicle is safe to continue driving and whether alternate transportation should be provided while waiting.

I am writing to formally express my concern and frustration regarding the unresolved recall on my vehicle. It has now been over a year since the recall was issued, and my point of purchase dealer still has not received the necessary parts to complete the repair. This extended delay has moved beyond inconvenience and has become a genuine safety issue. I rely on my vehicle daily, and continuing to drive it under an open recall, especially one involving safety components is unacceptable. I have been patient for more than a year, but the lack of progress and communication has left me with serious concerns. I expect prompt attention to this matter and a concrete plan to resolve the recall as soon as possible.

The contact stated that the vehicle failed to accelerate as intended while depressing the accelerator pedal. In addition, the vehicle stalled. The contact was able to coast to the side of the road. The contact stated that after several attempts the vehicle restarted. The contact stated that the failure persisted. The vehicle was a rental, and the contact returned the vehicle back to the rental agency. The vehicle was not taken to the dealer or an independent mechanic. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 25,500.

ODI #11731184FUEL/PROPULSION SYSTEM

2024 Pacifica Van with 66,000 miles started to run rough then quit in a McDonalds parking lot in Richmond VA 70 miles from our home residence. I had the car towed to a local Chrysler dealership on Saturday night before Easter. The tow truck drive was kind enough to take myself, spouse, daughter and Golden Retriever to the Richmond airport where we rented a car and drove home to Leonardtown, MD. The Chrysler dealership troubleshoot the vehicle and ordered a fuel pump.

ODI #11727174SERVICE BRAKES

I started to brake on Peck Road. I turned into my driveway and as I started to come to a complete stop, the brake pedal went to the floor and the brakes did not engage and I went over a landscape wall. I have a video of the event from my ring camera. I went on-line and noticed that other Pacifica owners have had an issue similar to this one.

ODI #11715184ELECTRICAL SYSTEM

As prior reported I am the owner of a 2024 Chrysler Pacifica that has a known safety defect in the rear sliding doors that is impossible to disable. This issue also cases parasitic draw strain on the battery that can completely disable the vehicle. The affected system can not be isolated and triggers even without the vehicle in use even effecting the way that the engine runs when in use due to voltage issues that the door issue causes with the battery system in the vehicle. This is not an isolated issue Enterprise Rent a Car in Rome, New York also stated to me that they had the same issue with a Pacifica that they use for rentals when I mentioned why mine was in the show this was on 2/2/2026 at 4:00pm. Something should be done to fix this issue or at the very least they should have a way to completely turn off the effected system. Nye Dodge in Oneida, New York is doing the repair on my vehicle and can be reached at 315-277-8955 for more information about the cause, and the fix for this issue. I want to reiterate that this is not a one off issue it is a known issue with the Chrysler Pacifica series that should be investigated.

ODI #11714729UNKNOWN OR OTHER

The headliner AC vent cover fell out while driving striking my minor daughter. We were driving a rental so we did not attempt to correct the issue, leaving the vent cover in the front, but we have found several other incidents reported online of the same issue. We did not take it in for inspection nor were any warning lamps on.

ODI #11713523ELECTRICAL SYSTEM,STRUCTURE,UNKNOWN OR OTHER

Rear sliding doors have an electrical issue that can cause a warning that the door is open to be displayed on the drivers information screen while the vehicle is in gear, and being driven. After a moment you will hear a relay click, the error message goes away, and after a while the error message shows up again this repeats in cycles. Attempts to disable the sliding door system for safety removing fuses do not work the system will continue to indicate that the door is open even when it has been closed manually. I just had the vehicle serviced at Nye Dodge in Oneida, New York so they have better information on what caused the issue, and the fix they can be reached at (315) 277-8955 for information on the issue. The issue I would like to bring to light is that in the event of a defect it is impossible to fully disable the sliding door, and it's sensors I have attempted to pull fuses to shut the system down, and can't find anyway to totally isolate the effected door for safety. I have been reassured the door will not open however I will not be using this vehicle to transport my children until this issue is corrected. You should be able to turn the system off in the event of a failure with one of the sliding doors.

ODI #11709477AIR BAGS

I am reporting a failure by Chrysler (FCA US LLC) to provide a remedy for Recall 25V302 (55C) within a reasonable timeframe. The local dealer (Houston, TX) has confirmed a 6-week backorder for the side curtain airbag parts. The dealer refuses to ground the vehicle or provide a loaner/rental. As this defect involves a life-safety restraint system, I do not feel safe operating the vehicle, and the manufacturer is failing their federal obligation to provide a timely repair.

ODI #11706683POWER TRAIN,ELECTRICAL SYSTEM,ENGINE

The charging system light came on. My wife went to charge at public charging station due to being close to one. After plugging in the van, it said charging failed. She unplugged the charged went to back up and the van died. Had to have it towed to dealer. Dealer states even though the van is in the warranty period, the IDCM which is up under the van is scuffed and therefore isn’t warrantied. They said there is a big dent in the muffler also so we must have ran over something and caused the damage even though we live in a city with paved streets, a subdivision with paved streets, and have a concrete driveway at our home. They said they have sent me emails with the pictures which I have yet to recieve. I went to the dealer and asked them to jack up my van so I can see the damage personally and they don’t have a lift available at the time, so I told them I would be back in town at a certain time from work and to call me so I could see it and they have yet to show me the scuffed IDCM module or the dented muffler. We haven’t run over anything to cause this kind of damage to a module like this because all the hybrid batteries are under the van. If the Chrysler Pacifica Hybrid is so cheaply made for a $62,000 vehicle that something can “scuff” a module housing and shut a vehicle down, this is potentially very dangerous if someone were driving on the highway if it were to shutdown and someone (including myself or any of my family) were driving and lose all propulsion and steering (because it’s electric steering) or worse braking! It could be catastrophic with people dying. This needs to be investigated to see how fragile this component is and if they need to “toughen” it up or re-locate it. Versus trying to get by and get around paying warranty claims or hoping nothing happens until the units break down after the warranty period not knowing of the Safety hazard they have created for their vehicle owners and the driving public.

The contact owns a 2024 Chrysler Pacifica. The contact received notification of NHTSA Campaign Number: 25V302000 (AIR BAGS); however, the part to do the recall repair was unavailable. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue. The contact had not experienced a failure.

ODI #11705111ENGINE

I was driving the vehicle as normal when all of a sudden a message appeared and the engine shut down. The message was "Stop Safely Vehicle Will Shut Off Soon". It brought my speed down to 10 mph and I had to pull off the side of the road. This is now the 3rd time it has happened. We have brought it into the dealership previously and both times they said that they were not able to find the issue and since the message went away, we could proceed with driving the vehicle as normal. I have 3 little kids and we often drive on the highway and I no longer feel safe in my car. I know there is an open investigation for model years 2017-2023, but unfortunately ours is a 2024 so is not in that investigation.

ODI #11703663WHEELS

The contact owns a 2024 Chrysler Pacifica. The contact stated that while attempting to make a left or right turn, there was an abnormal sound coming from the front driver’s side wheel. The contact stated that while attempting to depress the brake pedal, the vehicle made the same sound. A dealer was contacted. The vehicle was taken to the dealer, where it was diagnosed and determined that the wheel hub was loose and needed to be replaced. The wheel hub was replaced, and the vehicle was repaired. The manufacturer was not notified of the failure. The failure mileage was 15,000.

ODI #11700255AIR BAGS,SEAT BELTS

The vehicle was sold with a “minor accident” reported, but the condition suggests the damage may have been more significant. After purchase, I discovered the rear seatbelt did not work and required interior panels to be removed and replaced. A non-working seatbelt points to possible prior impact or structural damage affecting the restraint system. The vehicle also has an active safety recall for the side curtain airbags. The combination of a recall and a failed seatbelt raises concern that the vehicle may have been in a more severe collision than reported or repaired incorrectly. A failed seatbelt is a major safety hazard and should not be present at sale. The defect was confirmed by the dealer’s service department, and the vehicle has been in their possession since November 10, 2025. No warning lights appeared; the issue was found only when trying to use the seatbelt.

ODI #11697837ELECTRICAL SYSTEM

When I tried to start my Pacifica I got a clicking sound like the battery was almost dead. I tried again and a very loud pop came from the area above the driver side around the top left windshield or possibly the air bag area. All the electric doors failed to operate. The car did not start. I snapped pictures of the instrument cluster. The motor LED came on, the "Auto Park Not Engaged" came on. The engine start button showed RUN. All 5 gear shifter LED were on "P R N D L". Both outside mirror blind spot indicators were on. I was trying to go to a doctor's appointment so I tried lifting the battery cable under the hood to reboot the computer. That worked and all returned to to normal. This malfunction has happened twice in the past month. The second one happened 11/4/2025. Photographed images are available upon request.

Car sold at auction. Obtained Black Box & OBD info. Car owned by EAN Holdings, LLC. - Enterprise RAC. Problem not reproduced, car not drivable after crash. Did not notice any warning lamps, too busy trying to keep car on road. My wife was heading home after some late-night shopping when crash occurred on 2-lane dry & clean asphalt country road. Weather was a cool and clear moonlit night. Posted speed limit 45mph. With her headlights on she could see very well far up ahead & knew her surroundings, where the roadside became grass on the left, & a drainage ditch to the right side. She chose out of caution to drive near the center line in the roadway to give ample distance between the passenger side of the car and the soft shoulder to the right. Approx. 150 ft before she approached the turn to the left, she felt a sudden & very intense TUG from the right rear wheel. This caused the car to jerk out of control, shoving its back end to the right. It was impossible to correct the vehicle's course & what the car decided to do on its own accord. The car then proceeded to yank the steering wheel hard to the right (while she fought to keep the car turning left), and it forced her off the right shoulder of the paved road, across a 3-foot soft shoulder on the passenger side of her car. The car continued traveling off the road and over a gravel and corrugated drainpipe, landing her in a deep 5-6' irrigation ditch, and abruptly stopped on an uphill slant, making contact with a small sapling tree/weeds. The ride was so violent from all the jarring & bouncing that everything inside the car was flying around like in a front-loading washing machine. She had to exit via the back pass side door. Airbags did not deploy & car required towing. By the Grace of God, she did not die!

We purchased our 2024 Pacifica Pinnacle AWD new in July of 2024. The vehicle currently has 9,600 miles on it, and the issue has been present since it was new. We have experienced continuous issues while driving the vehicle at highway speeds, with the steering exhibiting a gap in responsiveness. The steering acts like there is a dead zone between 11 and 1 o'clock when turning the wheel, where nothing happens. Once the vehicle finally reacts to the driver's inputs, it causes the car to jerk from side to side. It is bad enough that it causes my entire family to become carsick during even short trips. Still, my primary concern is that the steering is not responsive, and if the issue worsens or changes, it could lead to a loss of control over the vehicle. In addition, the jerking caused by this may cause loss of control when traveling in inclement weather or icy conditions. We took the vehicle in for diagnosis, assuming an alignment issue, but the dealership verified the alignment was within the manufacturer's requirements. In addition, the dealership told us, "This is just how the Pacifica drives. All of them have this issue with the steering". Today, I came across an investigation into a similar issue with older model Pacificas, and I believe this is not only the same issue but also warrants investigation of all model year Pacificas. There were multiple comments within the forum post of owners experiencing this issue in newer vehicles than the current investigation. Please consider expanding your investigation to all model years. Thank you.

Vehicle loses power and goes into limp mode. This has happened three times same fault code. The last two times we were going 80 on the expressway. Very dangerous luckily we haven't caused a accident. The manufacturer hasn't fixed the problem. Even though we keep taking it to the dealer. They clear the code and try to recreate the problem with no success. Tell us there is nothing they can do. There are recalls snd repairs for previous years of this vehicle with the same issue. They haven't solved the problem in the 2024s. The car was bought new in November 2024 and has 18,000 miles. It is unreliable and unsafe. See attached photos showing he dash and warning.

I would like to bring up a serious safety issue with this vehicle (2024 Chrysler Pacifica Select PHEV) WITH 6997 miles on the odometer. As I was driving, at approximately 11:15AM, I received a message on the dashboard that read, “Stop Safely!! Vehicle will shut off in < 2 minutes”. For my own safety as well as other drivers on the road, I tried maneuvering the vehicle to the side of the road, however the vehicle shut off within 15-30 seconds. The vehicle did not move at all. The serious safety concern I have is “What if the vehicle had stopped running on the major highways full of traffic”? Please note, this is also a well-known issue starting from the 2021 Chrysler Pacifica models. Additionally, a similar incident occurred to another similar make model in Sept. 2024 with VIN # [XXX] . It seemed like, when I received the message, the vehicle's computer system interlocks fuel supply and chokes up the brakes to the wheels so the vehicle is completed locked up in one position and it does not do rolling stop to safely pull over to the side of the road. This is very dangerous because if a semi-truck with 80,000 lbs. load is coming behind you, you will NOT be able to move your vehicle to the side. This can potentially kill all the occupants in the vehicle. This vehicle has been towed to the dealer to troubleshoot/resolve the issue. Thank you so much in advance for looking into this issue urgently. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

9-SPEED TRANSMISSION:ZF9 HP-9, considered an "improved" transmission as compared to the previous 6-speed versions, prior to 2017. According to more than one FCA class action lawsuit, Fiat Chrysler knew the transmission had problems that could make FCA vehicles unsafe to drive. Problems: Lurching away from a stop, either when shifting into Drive or Reverse, then often accelerating into traffic, or into a crowded parking lot, or in an after-school setting, with children around. Rough shifting into Reverse or Drive from Park. Delayed shifting into Reverse or Drive, from Park. Intermittent "sounds" such as clunking or low-level grinding when attempting to shift from one gear to another. Refusal to downshift once a speed of 35-45 mph is reached, creating "gear-hunting", during which time the vehicle begins to accelerate, then decelerate, then accelerate again, often forcing abrupt, potentially-dangerous braking. Refusal to downshift allows the vehicle to continue unabated in gears 2-3, then 3-4. Intermittent refusal to upshift or downshift when engaging gears 6-9. At speeds between 50-70 mph, the vehicle will independently begin to slowly downshift, for no apparent reason, requiring a soft braking action, first, then sometimes followed by pulling over, coming to a complete stop, waiting then resuming. then getting back to speed. On one occasion, the transmission "slipped" into neutral at approximately 70 mph, causing the driver to pull off onto a major interstate's shoulder, shutting the engine off, then allowing the transmission to "cool" for several minutes, before starting the vehicle and continuing. This can be very dangerous if the transmission completely fails at 2 AM in the morning in a somewhat "dicey" area, anywhere, on any highway. All of the above complaints and transmission dysfunction/defects are sudden/acute and intermittent creating a sense of unnecessary anxiety and frustration when driving. As aresult, the owner has begun not allowing other drivers

The vehicle, without warning, flashed the message “Stop Safely, Vehicle Will Shut Off Soon” and suddenly went totally unresponsive. It had shut down within 60 seconds. This was on day 4 of ownership and less than 1000 miles on the vehicle. We were unable to restart, shift, or get the vehicle to respond in any way. Upon reading further, this is the exact issue that over 67,000 of these models were recalled for in years 2017-2023 with transmission replacement as the required repair.

1st time: I was exiting my driveway and dash presented with no torque and a picture of a turtle came up saying power issues. Running on hybrid engine at this time. Pushed down on gas and no power. I turned van off waited a few minutes, restarted, gas engine came on along with the engine light on dashboard. I was able to return van to the garage. Came in and called the dealership. Took car to dealership next day and they plugged it into the computer to check. They returned the van to me saying they gave it a reset and said I should be OK to go. I was leaving on vacation in a couple of days and gave me the OK to drive. Drove to SC from Ohio and all went well until the third day is when it happened once again...... 2nd time while in SC: Dash board gave warning, stop safely vehicle will shut off soon....engine shut down as I was turning left in traffic. Van was running on the batteries hybrid system with 26% left of charge. I took the van to a dealership to make sure it would be safe for us to drive home. They too plugged it into a computer and information came up about the battery. The tech told me it would be OK to drive home. They did not reset as they told me to take it to my home dealership to have them document the issue. Made it home safely, thank goodness. Dealership had the car for a week. They were told by Chrysler to open a case for the battery issues before they would OK ordering a new battery. They had my car for a week and Chrysler told them to reset and give the van back to me and have me drive the vehicle to see if it would happen again. My dealership shared with them it had happened twice but Chrysler refused to allow them to order a new battery and repeated to give the vehicle back to the owner after a reset and have the owner drive and see if it happens again. My feeling is Chrysler has no regard for my or my families safety. I did have the same experience with another hybrid Chrysler van I rented going 70 MPH on PA turnpike.

The vehicle was purchased on 1/8/2025. On 1/13/2025, the vehicle had to be towed to the dealer because the transmission wouldn't shift to park. Dealer performed a software update to correct the issue. On 1/21/2025, the vehicle had to be towed to a dealer because the transmission wouldn't shift to park. Dealer replaced the gear selector. On 1/27/2025, the vehicle had to be towed to the dealer because it wouldn't shift to park. Dealer stated the battery failed. On 3/13/2025, the vehicle was taken to a dealer because the rear hatch opened on its own while driving and wouldn't close. Transmissions that won't shift to park present a serious roll away hazard that could injure or kill a person.

The contact owns a 2024 Chrysler Pacifica. The contact stated while driving approximately 55 MPH, the braking system erroneously activated causing the vehicle to abruptly stop. The failure had occurred on several occasions while driving on the same highway. The contact stated that during the failure, the Forward Collision Avoidance: Adaptive Cruise Control was not activated. The vehicle was taken to the local dealer who was able to duplicate the failure, but the cause of the failure was not yet determined. The manufacturer was notified of the failure. The contact stated that the vehicle had been modified to accommodate a disabled driver and did not believe that the failure was related to the modification. The failure mileage was approximately 800.

Adaptive cruise control slammed on brakes at least five times when nothing there in front or on the sides. First time I ended up with whiplash…now I hear it start to stop and I override system by speeding up. It also sometimes gets so close to car in front without braking so I have to manually brake. I’m concerned about being rear-ended if it slams on brakes for no reason. Five times there were no deer, people, etc.

We were rear ended on the highway, the driver seat broke a part came off and the back of the seat went back instead of staying upright. I’m not sure where the vehicle is after we released it to the insurance company on 09-11-24 The driver has back pain and the back of the seat didn’t stay where it should have. I’m not sure we haven’t talked to the dealership just a mechanic. The insurance company had an adjuster look at it and deemed it a total loss. No there was no warnings or any.

ODI #11609848SERVICE BRAKES

On a brand new vehicle with less than a few hundred miles, the electronic parking brake engaged and would not disengage until the battery was disconnected. This left an error code and the dealership replaced brake actuator. Then, the SafeHold feature started to randomly activate when vehicle started to move from a stop light or stop sign. An error message would display on the screen, the vehicle shifted itself into park and activated the parking brake. The message indicated the vehicle had to be turned off and restarted. After restarting, the parking brake would disengage and the transmission could be shifted back into drive. This happened in the middle of traffic. This did not leave any kind of computer error code and therefore the dealership was not able to address it. Separately, the parking brake would refuse to disengage after repeated attempts, again without an error code, and the dealership was not able to address it. The vehicle has currently been under inspection for over forty days in total, and over twenty days since the last incident. The "SafeHold" safety feature is an accident waiting to happen.
