I own a 2023 Chevrolet Malibu with an open safety recall. General Motors/dealership has refused to repair the recall. The vehicle has also had multiple safety-related problems, including Service ESC warnings, jerking/poor acceleration, Service Safety Restraint System and airbag warnings, and a “Steering Assist is Reduced, Drive with Care” warning. On or about XXX, while driving, I experienced what felt like the brakes, wheel, and steering system locking up. I was unable to safely control the vehicle, resulting in a crash. During the incident, my head struck the steering wheel and I injured my hand, which has visible scars. No police were involved. I have a [XXX] daughter. She was NOT in the vehicle during the crash, but I do not feel safe transporting her in this vehicle because of the serious safety issues and what happened. The vehicle was later inspected through GM/dealership channels. I was told no diagnostic codes were found, and GM declined assistance. The dealership also stated the vehicle needs repairs related to the crash, including a passenger presence module. I am especially concerned that the open safety recall remains unrepaired. I have photographs of the vehicle damage and photographs of the scars on my hand. I can also provide my communications with GM and information regarding the recall. I am requesting that NHTSA review these safety concerns and the refusal to repair the open recall. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

My rear view camera is fuzzy and today it went out completely and the screen was black. Sometimes it shows what’s behind the car when I’m backing and then suddenly the screen goes black and I’m unable to see anything. It just started today 6/15/26z . I believe this is a safety hazard and could cause an accident while backing up putting myself and others at risk . There was no warning before this happened, I just noticed the screen was fuzzy on one side, then the fuzziness would go to the other side of the screen . I am writing to log an official safety complaint regarding my 2023 Chevrolet Malibu. This vehicle is included under active NHTSA Campaign Number 26V212000 (GM Internal Recall Number N262551720) for a defective rearview camera housing that allows moisture intrusion, causing the backup display to go blank. My vehicle has experienced a complete, permanent failure of the rearview camera screen. The backup display is entirely black/blank when the vehicle is placed in reverse, eliminating all rear visibility. This creates an immediate, active safety hazard during everyday backing maneuvers and non-compliance with Federal Motor Vehicle Safety Standards.Despite the recall being announced months ago, General Motors has failed to provide a timely remedy or allocate replacement parts to my local dealership. Because my camera has actively failed, I am being forced to operate an unsafe vehicle indefinitely due to manufacturer parts delays. I request that the NHTSA investigate GM’s failure to supply remedy components to vehicle owners experiencing active component failures, as this delay leaves a known crash risk unresolved.

Most of the time, the car will not unlock when the unlock button is pressed on the key fobs. Most of the time, when the key fob is on the console and the start button is depressed, the car will not start. It can take multiple minutes and attempts before the car will finally start. Changing the batteries in the key fob did not resolve this issue. When we took it to the dealership, they say that the Body Control Module is the cause of the issue. When we google that, we see that there is an existing GM-issued service bulletin for certain units of the same year, make and model of our car. We therefore would like it to be investigated to have this service bulletin expanded to include our vehicle. It is unacceptable for a three-year old car not to unlock, lock, or start reliably and isn't normal wear and tear and there have not been any accidents.

The rearview camera started acting funny around March 2026 and completely stopped working in April 2026. When I put the car in reverse it says “service vision system” I have been frequently checking the recall website and it says they are aware of the recall but do not have a fix. I have even called my local Chevy dealer and they had the same info. Not having a rearview camera is a big safety hazard- I live in a neighborhood with numerous children and in a small town where parking is tight and often inconvenient. I would like to have a functioning rear view camera again please!

ODI #11745186VISIBILITY/WIPER,LANE DEPARTURE,BACK OVER PREVENTION

To Whom It May Concern, I am the owner of a 2023 Chevrolet Malibu that is affected by the ongoing backup camera safety recall. I am submitting this complaint because this defect is not a minor inconvenience—it is a serious safety issue that places me, my family, pedestrians, and other motorists at risk every time the vehicle is operatesThe rearview camera system is a federally required safety feature designed to help drivers detect people, vehicles, and obstacles behind the vehicle. When the camera fails, freezes, goes blank, displays a distorted image, or does not activate properly when the vehicle is shifted into reverse, I am left without a critical safety system that I relied upon when purchasing this vehicle This defect has significantly reduced my confidence in the safety and reliability of my Chevrolet Malibu. A vehicle with a known safety recall involving visibility and accident prevention creates constant concern, especially when backing out of parking spaces, residential driveways, crowded parking lots, and areas where children or pedestrians may be present. The possibility of striking a person, vehicle, or object because a manufacturer-installed safety system fails is unacceptable. I purchased this vehicle in good faith expecting Chevrolet to provide a safe and dependable product. Instead, I am driving a vehicle that has been identified as having a safety-related defect serious enough to warrant a recall. While recalls are intended to correct safety issues, owners should not be left dealing with uncertainty regarding when the problem will be fully resolved or whether additional failures may occur in the future. Because this defect directly impacts a critical safety system and has diminished my trust in the vehicle, I respectfully request that General Motors and Chevrolet review my case for additional relief beyond the standard recall repair. Specifically, I am requesting consideration for a vehicle replacement, manufacturer-assisted trade-in, buyout

Subject: Safety Complaint and Request for Assistance – 2023 Chevrolet Malibu Backup Camera Recall To Whom It May Concern, I am the owner of a 2023 Chevrolet Malibu that has been affected by the backup camera recall. The malfunctioning backup camera creates a serious safety hazard for me, my passengers, pedestrians, and other drivers. The backup camera intermittently fails to display properly when the vehicle is placed in reverse. When this occurs, I am unable to clearly see objects, vehicles, or people behind my vehicle as intended by the safety system. This significantly increases the risk of an accident, especially in parking lots, residential areas, and other locations where pedestrians may be present. I purchased this vehicle with the expectation that all safety systems would function reliably. The ongoing issue has caused me concern and has reduced my confidence in the vehicle’s ability to operate safely. A backup camera is an important safety feature, and any failure of this system places drivers and the public at unnecessary risk. Given the seriousness of this recall and the inconvenience and safety concerns it has caused, I respectfully request that Chevrolet review my situation and consider available remedies, including replacement of the vehicle, a vehicle trade-in assistance program, or another appropriate resolution that ensures my safety and restores my confidence in the product. I appreciate your attention to this matter and look forward to a prompt response regarding the options available to address this safety concern. This all started when i had my car serviced for an oil change and began to hear some clicking sounds when I would start my vehicle. The fuel sealant is defective, the fuel caps were corroded, leaks oil, and causes the turbo on the engine to lag. I found this out as I was driving and tried to make a turn. When pressing the accelerator the vehicle lagged almost causing me to be involved in an accident on more than one occassion. When conducting on-line research I am not the only owner that has experienced this issue. It would appear that Chevrolet is aware that this is happening in its turbo models. I have an extended warranty that was purchased through the dealership. This issue caused them to replace the fuel sealant, the fuel caps, and the fuel pump. However, 2 weeks later my vehicle is still leaking a significant amount of oil. When I took the vehicle to be serviced, I was told that the sealant is broken again and gaskets have to be replaced. Upon researching this online other owners have experienced the same issues. I did share my online research with the service technicians. Their reactions were dismissive.

The brake pedal on my vehicle intermittently becomes stuck in the fully raised position and will not depress when I attempt to apply the brakes. When this occurs, I am unable to slow down or stop the vehicle, creating an immediate and serious safety hazard. The problem happens without warning and has occurred multiple times while driving at various speeds. When the defect occurs, the pedal feels locked or jammed, and I have to use extreme force or pump the pedal repeatedly before it finally releases. This delay in brake response puts me, my passengers, and other drivers at risk of a crash. The issue is unpredictable and does not appear to be related to weather, temperature, or driving conditions. I have not made any modifications to the braking system. The defect has occurred both during normal city driving and while approaching intersections. Because the failure prevents the brakes from engaging at all, I believe this is a critical safety issue that requires investigation.

The dealer confirmed that the recall part was not yet available and referred the contact to the NHTSA Hotline for further assistance. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue. The contact had not experienced a failure.

ODI #11733160POWER TRAIN,ENGINE

The issue appears to involve the engine and turbocharger system. The vehicle has shown symptoms consistent with a turbo underboost condition, including loss of power and reduced acceleration. A mechanic indicated that the symptoms may be consistent with diagnostic code P0299 (turbo underboost). The vehicle is available for inspection upon request. This issue affects safe operation because the vehicle loses power unexpectedly while driving, which creates a risk when merging into traffic, accelerating, or maintaining speed in normal traffic conditions. The loss of power is inconsistent and can occur without warning, increasing the potential safety concern. The problem has been ongoing. The check engine light is currently on and has remained on. The warning light first appeared around January 2026, and the issue has continued since then. Similar turbocharger-related issues have been reported in prior model years, including 2022 Chevrolet Malibu vehicles, where coverage or corrective actions were provided, raising concern that the same or similar issue may be affecting 2023 models.

The rearview camera system fails intermittently when the vehicle is placed in reverse. The display will either show a distorted image or go completely blank without warning. This issue has occurred multiple times. This malfunction significantly reduces rear visibility and creates a dangerous condition while backing, especially in environments with pedestrians or other vehicles. The failure increases the risk of a collision because the driver cannot rely on the camera system to safely monitor the area behind the vehicle. There were no warning lights, alerts, or messages prior to or during the failure. The issue appears to occur randomly and without any clear triggering condition. The vehicle has not yet been repaired because the manufacturer has issued a recall, but the remedy is not yet available. The condition has not yet been inspected by a dealer, but it matches the known recall involving rearview camera failure. The vehicle and camera system are available for inspection upon request.

The white exclamation warning icon with the "Shift to Park" message, the check engine warning light was illuminated, and the “Engine Power Reduced message was displayed. Additionally, the power train and ESC warning lights were illuminated. The vehicle was pulled over to the side of the road and towed to the residence. The vehicle was taken to a dealer, but the failure was not replicated; an aftermarket tracker was disconnected. The failure persisted, and the vehicle was taken to the same dealer, where it was diagnosed and determined that the battery needed to be replaced. The vehicle was repaired, but the failure recurred the following week. The vehicle was then taken back to the same dealer, but the failure was not duplicated. ODI #11707356ELECTRICAL SYSTEM,AIR BAGS,STRUCTURECRASH1 INJURY

On December 24, 2025, the contact's 2023 Chevrolet Malibu was involved in a severe side-impact collision. The vehicle was struck on the rear passenger side by a full-size pickup truck traveling at high speed (est. 60 mph) that failed to stop at a red light. The impact caused the contact's vehicle to spin approximately 180 degrees and strike a concrete median sideways, generating high lateral G-forces. FAILURE 1: AIRBAG NON-DEPLOYMENT. Despite the severity of the impact and the violent spin into a curb (rollover trip point), the side curtain airbags and front seat-mounted side impact airbags failed to deploy. The failure of the restraint system caused the driver to be thrown against the driver-side door structure, striking their head on the window and B-pillar. The driver sustained a concussion, cervical strain, and lumbar strain. Medical records confirm the injuries are consistent with impact against the vehicle interior due to non-deployment. FAILURE 2: EMERGENCY TELEMATICS MALFUNCTION. Immediately post-collision, the driver attempted to manually activate the OnStar/SOS emergency buttons. The system failed to connect to an advisor. Instead, the vehicle's infotainment screen malfunctioned, opening a voice-to-text transcription application. The system then bypassed the vehicle's internal telematics module and attempted to route the emergency call to the owner's personal cell phone, indicating a complete failure of the vehicle's emergency communication hardware. This resulted in a 34-minute delay in coordinating professional assistance. Contact notes; while this vehicle currently shows 0 open recalls, the failure mechanism (sensor non-response during side impact) mimics the defect described in NHTSA Campaign 22V923000 (GM Recall N222392300), where improper welding of the front impact bar causes crash sensors to fail. The vehicle is available for inspection. Police and medical reports confirm the incident details. No warning lights were illuminated prior to crash.

At that point the car steered itself back and forth for our five times. I was finally able to regain control once it slowed down enough, and continued on my drive. I’m guessing that some accident-avoidance system kicked in when I swerved left in front of the other car, and the car swerved itself back to the right. But then it over-corrected back to the left. I was driving the car from home to work during morning rush hour in downtown Chicago. I was stopped at a red light and the auto-stop feature engaged as it is intended to do. When the light turned green I put my foot on the gas to go but the car wouldn't move. Cars behind me started honking & going around me. I tried various things but couldn't get the car to move although the ignition light was green. I finally put my emergency flashers on, shifted the car to park, pressed the ignition button to turn the car to off position, then depressed the brake pedal and the ignition button to turn the car on again, then shifted to drive and the car then moved forward as normal. No warning lamps have ever appeared re this. I immediately contacted my dealer to report this problem & soon after took the car there, where they confirmed this malfunction & communicated with GM Tech Line to try to resolve the problem while I sat at dealer all day but at end of day there was no resolution & they had no loaner car to give me & I needed the car so I left with it but thereafter always drove it only w/auto-stop feature turned off as I feared if it happened again I could be rear-ended. Dealer has now had my car for a week trying to ascertain how to fix this but because they say this feature is now not acting up they don't know how to fix it & they say GM Tech Line isn't giving them advice on how to resolve it. I cannot leave my car w/dealer indefinitely so now am forced to drive the car in this condition & need to always try to remember to turn off the auto-stop feature (or risk driving it in unsafe condition), which is not the right solution for a brand new car that had less than 2,000 miles on it when this started. Chart Analysis

The blue line shows 70 total NHTSA complaints filed over 4 years, with the worst year being 2026 (49 complaints, including 1 crash report).

The amber reliability line is declining — recent years score around 81/100, lower than earlier in the vehicle's life. This pattern indicates issues are accumulating over time rather than being resolved, which is a concern for used buyers.

The red dashed line marks recall issuance — the first recall came 3 years after owners began filing complaints, a lag that indicates the manufacturer was slow to formally acknowledge these problems.

Red dots on the amber line flag 2 years (2025, 2026) where complaints included crash events — these are the highest-severity incidents in the dataset.
