Details: We purchased this vehicle used at approximately 60,000 miles. I was never notified by Ford, the dealership, or NHTSA/DMV of any requirement for interim recall repairs every 40,000 miles. During a service at Antioch Ford, I was told there was an open recall but that parts were unavailable. I was never contacted again by Ford or the dealership with instructions to remedy the defect. Ford has since confirmed in writing that the recall requires interim repairs every 40,000 miles but claims they will not assist financially since my vehicle has not had those interim repairs. As a second owner, I had no knowledge of this requirement because I never received recall letters or notices. Safety Concern: This is a serious safety defect that nearly caused catastrophic injury or death to my family. The manufacturer has acknowledged the recall but is refusing assistance, citing “policy,” even though I was never notified of the required interim repairs. Ford has also attempted to close my case as “inactive” without resolution. Requested Action: I am requesting that NHTSA investigate Ford’s handling of this recall, specifically: Whether Ford properly notified subsequent owners about interim recall repair requirements. Whether Ford should be compelled to remedy this defect at no cost, given the severity and risk to public safety.

Finally took to the dealer and had them place the part on order (backorder) to be filled when available on/or around 1-2024. Failure happened 4-18-2024 with the recall part still on backorder damaging all brake lines and fuel lines all at the same time. This part should have been available when the vehicle was taken to the dealership for repair in Jan. 2024 and fixed. Not until a Failure happens that could of cost the lives of X-people on a highway due to parts not being available for a Major recall like this one.

Instrument panel turns off on and on during driving. Problem occurs during driving (will even turn off and on many times within same driving session) but will also happen while car is sitting just with ignition on. Took to dealer and they couldnt find the problem, told me to (and I quote) “continue driving until it gets worse” so they could find the problem later on. Romano Ford in Fayetteville NY told us to continue using the vehicle while the instrument panels contiously turns off and on, disregarding the safety issues of not being able to monitor anything while driving (speed, heat, fuel, warnings, blinkers or anything!). When we scanned the vehicle for DTCs it comes up as the instrument panel is losing connection to every single module in the vehicle, and they for some reason do not see this as dangerous.

On 17 November, 2023, at approximately 12 PM, I was traveling down the highway at approximately 55 mph. I suddenly heard a loud pop coming from directly under the cab and a subsequent rattling. It lasted for about 15 seconds, went away, and then lasted for another five seconds before completely stopping. Upon parking the vehicle, I crawled underneath and noticed that the flex coupling between the transmission and drive axle was missing two big chunks, and it appeared that a part of that material was lodged into the fire wall above the transmission underneath the floorboard. Also, present was a lot of strands of fiberglass from the coupling material. I immediately searched the Internet for some information regarding this and came upon an active recall that was published in 2019 for certain Ford transits manufactured 2015 to 2017. My vehicle is a 2016 Ford transit 250. It is my belief that this coupling issue that Ford published for recalls was not caught on my vehicle, and I have a concern that my vehicle may have fallen under that safety recall. Would like to know if an inspection could be done and some research to determine if my vehicle, in fact, falls within that recall.

I asked if it could be a leak in brake booster and was told no. No lights were coming on. I took the van to Les Schwab and was told the same thing. Unsure what to do, my brother came to the rescue and took the van. He started driving it home to California from Colorado. I rented an RV and continued our road trip. 80 miles later, the engine on the van made a loud bang and ceased. No engine lights. My brother said the engine started having acceleration problems so he stopped and checked all of the fluids. Everything was fine and engine was not overheated. But, complete engine failure at 79,000 miles. This was July. Since then, we paid to have a new engine installed. My husband flew out to pick up the van and new engine started having problems. He had to fly back home. Ford has said that the new engine is bad so they are paying for rental car and getting another new engine installed. Still do not have van. It's been three months. My concern is something else is going on...just seems extremely strange. I am still worried about the braking and revving bit and wonder if anyone else has had a similar issue.

I noticed this problem with approximately 75,000 miles. The vehicle developed a loud chirping or ringing sound but no warning lights. This leaking and failed exhaust system is allowing noxious gas to build up in the engine compartment and possibly leaking into the passenger compartment.

They said they had the parts ordered etc. I left the van there for several days...Then I was notified that they put on the new replacement driveshaft,,,, assuming it was correct and BALANCED.... the service manager notified me that the replacement driveshaft installed was NOT BALANCED and was causing all kinds of vibration problems / issues... steering wheel, driving, etc.... upon taking the van back several times.... and getting a new corrected balance driveshaft, installed that. However,,, still more problems,,, humming and vibrations, etc. they looked at the DIFFERRENCIAL ,,,, and found that all the bearings were bad,,, had to wait 4 to 6 weeks to get replacements......I told them that anything that SPINS... and not balanced is causing more vibration problems through out the van,,, engine, transmission, exhaust system, brakes, name it,,, etc. I picked up the van on June 28, 2021.. note... they gave me an extended warranty until 2024 ( 3 years ) and milage of 75,000 miles... Looks like expecting more major problems, etc..... I still have the **humming noise over 35 mph... and getting louder... I have seen articles about possible exhaust / muffler baffles going bad, etc... now that they did the recall, etc.. my van is an accident waiting to happen... and with the humming noise... Now all I need to do is lose my hearing... bull ssssss... thanks

FORD SAYS THIS IS A SAFETY FEATURE WHEN A PART GOES BAD SO THE ENGINE DOESN'T GET DAMAGED. THE THROTTLE BODY HAD TO BE REPLACED. IT IS VERY DANGEROUS FOR THE VEHICLE TO LOSE ALL ACCELERATION WHEN DRIVING ON THE FREEWAY WITH NO WARNING. THIS SHOULD NOT HAPPEN.

THE VEHICLE WILL STOP ACCEPTING ACCELERATOR COMMANDS RANDOMLY. IT WILL REFUSE TO ACCELERATE WHEN THE ACCELERATOR IS DEPRESSED. THERE HAVE BEEN 4 OF THESE VEHICLES IN OUR FLEET DO THIS. FORD IS AWARE OF THE PROBLEM BUT REFUSES TO DO ANYTHING ABOUT IT TO ACCOMMODATE THE DRIVERS EVEN GOING SO FAR AS TO STATE IT COULD BE WEEKS BEFORE EVEN DIAGNOSING THE PROBLEM. THIS IS TO COVER THE FACT HAT THEY HAVE THE THROTTLE BODIES NEEDED TO REPAIR THIS COMPLAINT ON BACKORDER. IF ON A FREEWAY THIS IS A DANGEROUS FAILURE THAT WOULD COMPROMISE THE SAFETY OF ALL ON BOARD. THE VEHICLE WAS IN MOTION AT THE TIME OF THE FAILURE ON A CITY STREET.

TL* THE CONTACT OWNS A 2016 FORD TRANSIT. WHILE DRIVING APPROXIMATELY 15 MPH, A BURNING ODOR WAS PRESENT INSIDE THE CABIN OF THE VEHICLE. WHENEVER THE VEHICLE WAS DRIVEN, THE BURNING ODOR WAS PRESENT AND THE CONTACT'S BREATHING BECAME DIFFICULT. THE VEHICLE WAS TAKEN TO THE DEALER WHERE IT WAS KEPT FOR APPROXIMATELY ONE WEEK. THE DEALER DIAGNOSED THAT THE EXHAUST PIPE WAS DESIGNED NOT TO EXTEND ALL THE WAY TO THE REAR BUMPER. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE. THE FAILURE OCCURRED FOR THE LAST TEN DAYS. THE APPROXIMATE FAILURE MILEAGE WAS 16.

UNSAFE SWAYING AND POSSIBLE LOSS OF CONTROL WHEN AFFECTED BY HIGH WINDS, PASSING TRUCKS OR UNEVEN ROAD CONDITIONS. NOT SAFE TO DRIVE OVER 55 MPH ON THE HIGHWAY DUE TO EXCESSIVE SWAYING THAT COULD LEAD TO LOSS OF CONTROL. HAD FORD DEALER CONFIRM THAT THERE IS NO MECHANICAL PROBLEM AND THAT IT WAS PERFORMING AS DESIGNED. FORD ENGINEERS HAVE NO FIX AVAILABLE AT THIS TIME. IT IS NOT A INTERMITTENT PROBLEM BUT RATHER A PROBLEM THAT OCCURS EVERY TIME IT IS DRIVEN IN THE ABOVE CONDITIONS. IT IS A PROBLEM THAT BECAME EVIDENT RIGHT AFTER TAKING POSSESSION OF THE VEHICLE (IT DID NOT DEVELOP OR WORSEN WITH TIME BUT RATHER SEEMS TO BE A DESIGN FLAW).

SEAT BELT LATCH FAILURE. THE PLASTIC COVER ON THE SEAT BELT LATCH RECEIVER DETACHES FROM THE ASSEMBLY DURING NORMAL USE. THIS RENDERS THE SEAT BELT UNABLE TO LATCH COMPLETELY, AND RENDERS THE SEAT BELT INOPERABLE. THE PASSENGER MAY NOT BE AWARE THAT THE LATCH IS NOT SECURED. THIS WAS NOTED ON TWO SEPARATE BELTS IN THE REAR SEATS OF A RENTED 12-PASSENGER TRANSIT. BASED ON MY INTERVIEW WITH THE PASSENGER, IT MOST LIKELY OCCURRED BY SITTING ON THE LOWER RECEIVER WHILE HAVING A MOBILE PHONE IN THEIR BACK PANTS POCKET. ON ONE OCCASION, THE PASSENGER STATED SHE HEARD A "CRACK" OR "POP." IT SHOULD BE NOTED THAT BOTH OCCUPANTS WERE LESS THAN 150 LBS. THE PLASTIC COVERS DID NOT COMPLETELY FAIL, BUT WERE SHIFTED TOWARDS THE ANCHOR OF THE LOWER ASSEMBLY. THIS DID CREATE A SITUATION WHERE THE PASSENGER WAS UNPROTECTED.
