ODI #11767927STEERING,VEHICLE SPEED CONTROL,UNKNOWN OR OTHERCRASH1 INJURY

Vehicle Information: 2022 Ford Mustang Date of Incident: XXX Description of Safety Defect & Crash: On XXX, this vehicle suffered a catastrophic concurrent failure of the electronic throttle control and steering systems, resulting in a violent injury crash. Prior to the incident, the vehicle was completely disabled due to electronic throttle errors, requiring us to rent a U-Haul tow dolly to physically haul the vehicle to Apple Ford in [XXX]. The dealership replaced the throttle body assembly to address a wrench/check engine light and airflow codes (DTC P050A). Exactly 30 minutes after picking up the vehicle from the dealership, while driving home at normal speed limits, the vehicle suffered a sudden, uncommanded wide-open throttle failure. The RPMs violently spiked and the supercharger engaged aggressively without driver input. Simultaneously, the steering wheel suddenly and aggressively forced itself completely to the right. The steering wheel violently locked in the rightward position; the driver actively attempted to pull the steering wheel back left, but the vehicle overrode his physical inputs completely. Because the electronic throttle and steering systems completely overrode driver control, the vehicle broke traction from the rear and veered sharply off the roadway, striking a tree. The uncommanded engine force was so severe at impact that the vehicle was launched into the air and rolled until stopped by a second tree. The driver sustained a broken fingertip and severe body bruising, requiring emergency hospital evaluation. Certified phone logs confirm the driver was on a hands-free call troubleshooting the initial mechanical instability up until the exact minute (12:19 PM) the final catastrophic failure occurred and a 911 emergency call was placed. The vehicle is currently being transferred to a Progressive Insurance facility for total loss processing. We are requesting a federal review . The contact stated while driving 60 MPH, the engine shuddered as if it was starving from fuel. The check engine warning light illuminated. The contact was able to drive to the residence, and the vehicle was parked in the garage. The contact had stopped driving the vehicle due to safety concerns.  The dealer and several other unknown dealers were contacted but were unable to advise when the parts would be available. Vehicle still does not have a remedy after nearly a year preventing use.

ODI #11730267ENGINE,FUEL/PROPULSION SYSTEM

The vehicle is a 2022 Ford Mustang EcoBoost. While driving in traffic, the engine suddenly stalled and shut off without warning. This created an immediate safety risk as the vehicle lost power in the middle of active traffic flow, requiring me to quickly maneuver to avoid blocking traffic and potential collision. The issue appears consistent with a fuel delivery or fuel pressure failure, as the engine restarted immediately after the stall. Prior to this incident, the vehicle had shown intermittent warning signs including check engine light activation and suspected misfire-related alerts. The problem has not yet been fully reproduced or confirmed by a dealer as a root cause, although the vehicle has been presented for service and was found to have no active recall tied to the VIN according to the dealership. No formal inspection or diagnosis confirming the exact failed component has been completed yet. No police, insurance representatives, or manufacturer inspectors have examined the vehicle at this time. Warning lamps (check engine light) and possible misfire-related symptoms were present prior to the failure. These symptoms appeared intermittently over the weeks leading up to the stall, particularly after periods of low fuel operation. Based on the symptoms and behavior, the incident closely matches known fuel system issues reported in similar Ford Mustang EcoBoost vehicles, including those associated with fuel pressure loss leading to engine stall while driving. The suspected system involved is the low-pressure fuel delivery system (fuel pump or related components), which is available for inspection upon request.

This recall describes a known safety-related defect which can stall the vehicle while driving, thus increasing the risk of a crash. After 8 months without developing a remedy, that danger remains every time I drive my beloved Mustang. Is Ford waiting for numerous deaths due to this issue before making the cure a reality? What is the explanation for the lack of a cure? Evidently Ford has produced a fine looking automobile which should sit in one's garage instead of confidently driving it.

The engine stalled without warning, creating a dangerous situation with surrounding traffic. I had to coast the vehicle and restart it. This vehicle has an open safety recall (NHTSA Recall Number 25V455 / Ford Recall 25S75) related to a low-pressure fuel pump failure that can cause engine stalling while driving. The recall has been open for several months, and there is currently no available remedy or repair from the manufacturer. The incident I experienced matches the recall description exactly. Despite the known safety risk, the manufacturer has not provided a fix, leaving the vehicle unsafe to operate. I am concerned for my safety and the safety of others due to the risk of repeated engine stalling at highway speeds.

On April 6th, the AC in the car began stalling and a few days after the engine started making all sorts of noise and failing and ultimately stopped working. I kept the car in my garage until I was able to obtain the money to replace the engine. I just yesterday picked up my car from the shop that replaced the engine (where I paid $13,000). The car is back in the shop and having same stalling issues.

Rear camera is not working. When I put the car into reverse I get a blue screen and then an error message stating my rear camera is not available. The sensors still work, if a car/object is close to me the car does beep but I cannot see behind me using my camera. This puts my safety at risk because I cannot see behind me as well as I could with the camera and could potentially hit a car and/or object. My car is at a Ford dealership right now getting looked at to see what could be the cause of this issue. This problem has been happening for the past few months, it started as it would stop working every now and then but work for the most part and now it does not work at all.

Yes, if the NHTSA communicates with me that they need to look at my transmission, and I am either provided another equal mode of transportation or paid for my lack of ability to use my vehicle during inspection/possession/out of service by the NHTSA. 10R80 10 speed transmission was finally replaced under warranty by FORD in January. The new transmission has been problematic and getting worse since march. Transmission flairs/slips. Transmission acts as if the brakes are put on at times/acceleration is greatly decreased for no reason. Hard shifts at times… I and other others are put at risk due my car, responds erratically and this could become a safety issue by causing a crash or loss of control by the Ford MACH 1 Mustang. Transmission often is too high gear and the as a result, the engine is lugging too much. No idiot, lights or warning lights have come on this go round. The police have not inspected the transmission. I would not expect them or ask them to inspect the transmission because I would assume 99% of them would know very little to nothing about it. Same goes with the insurance company. Ford and Chevy the two manufacturers of this transmission are both aware of the multiple problems of this poorly designed and manufactured transmission. I was not sure what incident date you wanted. The first transmission or second transmission? There are multiple incidents and issues starting from my first ownership of the car when I first test drove the car and had 550 miles on it. The original and replacement transmission are both and have been problematic and unsafe.

My daughter was driving her 2022 Mustang GT when the rim snapped, vehicle went into a truck, started flipping on the highway, then burst into flames. My daughter had approximately 3 minutes before it was engulfed. The manufacturer, Wheel Pros, stated that the insurance company allowed us $400 for the rim, so we should be good. This looks like a defective rim. It put my daughter’s life in danger. In June 2023, there was another complaint about rims cracking from this same company. The Mustang window is supposed to retract approximately 1/2" when the door is opened or shut. My vehicle does this BUT the window will not return to the fully closed position when the door is shut. This is a safety Hazzard as the window tries to fully close but then goes down a 1/2" and returns to fully closed repeatedly while I am driving; this leads to driver distraction on the road, a very hazardous situation. It also leaves the vehicle vulnerable to theft as with the window 1/2" down it is very easy for a thief to stick in a wire and activate the automatic door unlocks.

ODI #11576708FORWARD COLLISION AVOIDANCE

On December 9, 2023, I was driving my 2022 Ford Mustang on Interstate 70 (westbound) and received the following notifications: "Pre-collision assist not available" and "Powertrain Malfunction/Reduced Power." This resulted in an inability for the car to rev above 4000 RPM which was problematic for driving on an interstate in the mountain. Additionally, driver assists were disabled. The message cleared after the car was turned off for ~ 2 hours, but the pre-collision assist intermittently incorrectly identified objects prompting braking when ACC was activated. However, about a week later, the car suffered a cracked windshield prompting replacement on 12/29/23. Following replacement, the car gave a message saying, "Front Camera Malfunction. Service Required." Despite recalibration attempts at both Safelite AutoGlass and my local Ford dealership, the camera has remained dysfunctional. Similarly, apart from the initial "Front Camera Malfunction" message upon starting the car, there is no indication that Pre-collision, adaptive cruise control, and automatic emergency braking are disabled for the duration of the drive. On 2/23/24, I received a call from Safelite stating that the "manufacturer of the windshield had issued a precautionary recall where it looks like the camera bracket can possibly detach and cause some issues with the advanced safety systems." Upon returning their call, it was confirmed that my vehicle, 2022 Ford Mustang, was affected by the precautionary recall. Therefore, I was scheduled to take my car in for a new part and told that I could seek reimbursement from Ford. However, upon further discussion with my Ford dealership and Ford Motor Company Customer Service team, I was told that they were unaware of a recall. Similarly, when further information was requested from Safelite, I was told that "we are not allowed to give out that information." As of this submission, the advanced safety systems remain inoperable.

The blue line shows 50 total NHTSA complaints filed over 5 years, with the worst year being 2025 (14 complaints).

The amber reliability line is declining — recent years score around 86/100, lower than earlier in the vehicle's life. This pattern indicates issues are accumulating over time rather than being resolved, which is a concern for used buyers.

The recalls (red dashed lines) followed the first complaints by 1 year — a typical response window; neither unusually fast nor a pattern of denial.

Red dots on the amber line flag 3 years (2022, 2024, 2026) where complaints included crash events — these are the highest-severity incidents in the dataset.
