Car is in otherwise fine mechanical condition, with all four new tires, new brake rotors & ceramic brake pads, new battery & cables inspection, professional throttle body cleaning, 5,000 mile full-synthetic oil change & oil filter, new engine air filter, and all features and functions in what was known as 100% condition with no Engine Fault indicator on or subsequent OBD II codes. Yet it stalls at all speeds, including street traffic speeds and cruise at 70mpg. No indication whatsoever.

The paint on the roof of my 2013 Toyota Rav4 has begun to peel (delaminate). I first noticed it in May 2025. More recently, in June, while driving I saw in my rearview mirror what was clearly a chunk of white paint fall off the blowing from the top of my vehicle behind me - which is poses a safety hazard on the road. There are two areas on the roof now peeling. One is about 8inches in diameter (which has grown since May) and the newest area peeling and flaking/flying off is 1 foot long and 4 to 5 inches wide. Areas of the roof where the paint has peeled are beginning to rust and compromise the integrity of the body. I contacted Toyota twice recently regarding my case (#250701000710) which is considered closed. Two representatives said they were sorry I was having this issue but advised that I would have to go to a dealer and take care of it myself. Please be aware that there was a prior/known paint peeling issue that affected cars in blizzard/pearl white (which my car is). They offered what was called a "goodwill service" at the time under which they only paid to repaint any areas of the car that were already peeling at the time. The body of my car had many large areas of peeled paint that were repainted. However, the "goodwill service" program has since expired but now the roof is peeling. So, even though aware that the painting was flawed and prone to significant peeling they did not repaint affected vehicles as a whole and so it appears this issue has resurfaced.

If left untreated, corrosion could affect critical safety components such as the hood latch or airbag crash sensors located behind painted surfaces. 3.Potential for Peeling Paint to Become Road Debris: While driving at highway speeds, I have noticed paint flaking off and flying away. This could become a hazard to other vehicles, especially for motorcyclists or drivers behind me who could be hit by the debris. 4.Compromised Vehicle Visibility to Other Drivers: The exposed, uneven patches of primer and metal create an irregular surface, which could affect how my vehicle is seen by others, especially in low-light conditions. This may increase the risk of rear-end collisions due to diminished contrast. Toyota Response: Toyota previously acknowledged this issue with a Warranty Enhancement Program (WEP ZKG) for peeling paint, but I was informed that my vehicle is one year past the extended coverage period. Despite the defect being well-documented, Toyota has refused to assist. This is not just a cosmetic issue—it has clear safety implications. I urge NHTSA to investigate this problem further to determine if additional action is needed.

Finally Toyota has offered me a repair for this issue, but they are charging me an extra $250 to replace the car battery, which is perfectly functional. The vehicle shudders significantly when accelerating/cruising between 35-45 mph. The shudder started slight and did not pose an issue for a while, but now its significant enough that is causes erratic steering. The vehicle waved back and forth from the vibration yesterday and went across the median line into oncoming traffic. I was able to regain control and accelerated through past 45 mph when the vibration receded. After calling Toyota I learned that Toyota knew about this issue and the a faulty torque converter was to blame. There was no recall or notification just (as the dealer explained, an extended warranty). Since I was not "notified" I did not know of this dangerous exposure and missed the time period to get the convertor replaced by Toyota.

There is a large amount of peeling paint on the roof and front side panels of the car that started on 1/9/25. Toyota says they had a recall but that it expired. I brought the car to the dealer where it was original purchased (by the previous owner) and they completed a paint inspection and found "excessive paint peeling" but they are not willing to do anything. I called Toyota customer care but they have denied and closed my case (case #250109001939). There is at least a 2 foot section on the roof where the paint just peeled off and blew away. Without the paint the car will rust and become unsafe.

During the crash, the vehicle sustained severe front-end damage but the air bags did not deploy. The contact indicated that the front driver's side door had to be removed in order to be removed from the vehicle. The contact sustained multiple cervical fractures, a fractured sternum, and injuries to the rib and chest areas. An occupant of the second vehicle also sustained a minor undisclosed injury. The vehicle was destroyed and towed away. A police report was taken at the scene. The cause of the failure was not determined however the black box was pending review. The manufacturer and local dealer were not contacted. The failure mileage was 50,200.

During the winters I have tried dry gas thinking that could be helpful. I had also tried higher octane gas. It's been getting considerably worse so when I made an appointment at Grappone for my oil change, I asked them to diagnose it. They came back saying I need a new $4000 Torque Converter. Once I got home I began doing research. I found T-SB-0023-15. Then an extension on it to 8 years or 150000 miles. It's a problem big enough to put a TSB out. Residents came from the street to help put the engine compartment fire out. Driver and passenger (a dog) were able to get out safely, police and fire department came to the scene and asked people to move away from the vehicle in case of explosion. Fire was extinguished. Car was towed to local Toyota dealership, Car insurance agent has done preliminary inspection as well as dealership service manager. Both state that the battery did catch on fire. Battery installed was correct size as per service technician's initial report. At this time, total damage to the car is unclear, as a full inspection of the vehicle and electrical system has not been done. two or three times sporadically over the two days prior, the lights on the dashboard flickered on momentarily as though there was some kind of electrical short circuit happening somewhere. At present the vehicle remains at the dealership awaiting full Toyota inspection, since as a result of this office's investigation of numerous battery fires, Toyota has issued a Consumer safety recall.

The pickup truck stopped in the back seat of the Rav4. The Rav4 stopped when it hit the bumper, just short of going through the windshield. It is unknown how fast the drunk driver was traveling, but my Rav4 was totaled. However, no airbags deployed. Fortunately, my daughter and granddaughter, although completely shaken, walked away from the collision.

While driving the vehicle it experiences an intermittent shutter in the drive train. On flat roads it is an annoyance; when driving on mountain roads or with a loaded vehicle it becomes more severe to the point the vehicle feels like it will lose control. At its worse the issue impacts the ability of the driver to maintain control (e.g., braking down a mountain), at a minimum it causes significant distraction in situations requiring constant focus on the road. The Toyota dealer diagnosed the issue as a torque converter flex lock up. Toyota corporate is aware is this defect and issued a Warranty Policy Bulleting (No. POL17-06) to address it. While the company issued a warranty enhancement to address the issue, they are treating it as a minor defect affecting customer satisfaction instead of the safety issue it is. Due to Toyota's approach, they limited the time period they would cover safety related repairs. This leaves 2013-2015 RAV4 vehicle owners to either not correct the issue and hope for the best, pay to correct the defect themselves (expensive due to the need for a Toyota drivetrain expert or master technician to perform the repair), or pass the problem off to the next owner. I contacted Toyota Corporate on and even though the vehicle has low mileage (95K), because the problem appeared after a Toyota established date they would not correct the defect. In my conversation with Toyota Corporate they confirmed it was a safety issue and advised me to have it repaired right away at my own cost. This manufacturer defect in 2013-2015 RAV 4s, in certain road conditions, creates an unreasonable safety risk to drivers. Toyota's approach to resolving the issue is insufficient and does not protect owners from unreasonable risk of an accident because of this safety flaw. Please start an investigation into this safety defect and initiate a safety recall compelling Toyota to correct the issue regardless of the vehicle's age. Thank you

Note: This is a fully electric vehicle. It was made by Toyota, but the drive train (electric motor, battery, etc.) is made by Tesla. Earlier this summer (Around June 2021) while starting the vehicle from a stop, the vehicle either shifted to neutral (halfway through an intersection) or stalled. The vehicle displays a message saying "Check EV System". We have since taken it to two Toyota dealerships. The first (Toyota of Tacoma in Tacoma WA) said the 12V battery needed to be replaced, but the issue happened again almost immediately after we took the vehicle off the lot and completely stalled in the middle of a busy intersection. We brought it back to the same dealership who then said nothing was wrong with the vehicle. We also took the vehicle to a local auto repair shop that specializes in electric vehicles where they were able to recreate the issue but unfortunately were unable to work on the car as Tesla had locked the codes needed to diagnose the issues. The second dealership (Titus Will Toyota in Tacoma WA) was able to reproduce the issue, but they didn't have the equipment to work on the vehicle. They said that they spoke to the Toyota engineering team who told them that the speed sensor had moisture on it, and the coolant on the right side was low which indicates that coolant was leaking into the electric motor and the vehicle needed a new drive train. Toyota said that since the drive train was made by Tesla, they are unable to work on the vehicle and we would need to figure out how to get the vehicle from Washington to California to find a shop that could service the vehicle and completely replace the entire drive train - a diagnosis made and given over the phone. We purchased the vehicle just over a year ago in Washington and now it has a dangerous safety issue that neither Toyota nor Tesla is wanting anything to do with and that local repair shops cannot address.

DUE TO THE LAYOUT OF THE ROAD, THE CONTACT'S VEHICLE ENDED UP IN A CREEK OFF THE ROAD NEARBY. THE AIR BAGS DID NOT DEPLOY. THE VEHICLE WAS TOWED AND TOTALED. A POLICE REPORT WAS FILED. THE CONTACT SUSTAINED A CONCUSSION AS WELL AS INJURIES TO THE HEAD, ARMS, AND CHEST. MEDICAL ATTENTION WAS NEEDED. THE CONTACT STATED THAT THE VEHICLE RECEIVED SEVERE DAMAGE TO THE FRONT END AND INTERIOR DUE TO THE WATER THAT ENTERED THE VEHICLE. THE CONTACT HAD NOT TAKEN THE VEHICLE TO THE DEALER AND WAS WAITING FOR AN OFFICIAL DAMAGE REPORT FROM THE INSURANCE COMPANY. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND STATED THAT THEY WOULD NOT BE ABLE TO DO ANYTHING UNTIL IT WAS DETERMINED IF THE SENSORS WERE HIT AT THE TIME OF THE CRASH. I APPLIED THE BRAKES; THE CAR STARTED SPINNING AROUND. I PULLED THE EMERGENCY BRAKE AND THE CAR STOPPED. THE SECOND EVENT OCCURRED ON JANUARY 4, 2018. I WAS BACKING OUT OF A PARKING SPACE; I STOPPED SO I COULD RENTER THE SPACE. WHEN I SHIFTED FROM REVERSE TO DRIVE, THE CAR ROARED OFF AND BECAME AIRBORNE. I TALKED TO TOYOTA SERVICE MANAGER TODAY. HE SAYS THERE'S NO WAY TO DIAGNOSE THE PROBLEM UNLESS THEY CAN DUPLICATE IT. THAT MIGHT TAKE 100S OF MILES AS IT"S SO INTERMITANT. HE CLAIMS THEIR COMPUTER WON'T SHOW A CODE FOR THE PROBLEM. I AM AT A LOSS AS TO WHAT TO DO. IF IT LOSES POWER ON A CROWED FREEWAY IT COULD BE QUITE DANGEROUS. THE VEHICLE HAS 44000 MILES.

WHILE OPENING THE TRUNK, WHICH IS ELECTRIC, THE RIGHT LIFT GATE PISTON BLEW AND SEPARATED FROM THE BALL MOUNT. AT THAT POINT THE TRUNK/HATCH WOULD NOT WORK ELECTRICALLY OR BY HAND. IN FACT THE MALFUNCTION OF THE PISTON WAS SO INTENSE IT CAUSED SIGNIFICANT DAMAGE TO THE BODY OF MY CAR, BUCKLING THE BACK ROOF, SHIFTING THE DOOR AND BUCKLING THE HINGES. THE GRAND TOTAL OF DAMAGE IS WELL OVER $5000. TOYOTA WAS WELL AWARE OF THIS DEFECT. IN FACT I HAVE THEIR TECHNICAL SERVICE BULLETIN FROM TOYOTA THAT DISCUSSES THE PROBLEM IN DETAIL (T-SB-0168-13 FOR WHICH I AM SENDING YOU A COPY). THE PISTON, KNOWN AS AN ACTUATOR HAS BEEN CHANGED AFTER A CERTAIN VIN # (SEE PAGE #2 OF ATTACHED BULLETIN). BECAUSE OF THE DEFECT I NOW HAVE TO REPLACE THE UNAFFECTED PISTON ALSO. EACH PISTON COSTS $686.16 FOR THE PART ALONE. IF THE CAR IS UNDER WARRANTY THEY COVER REPAIR, AS I HAVE OWNED MY CAR FOR ALMOST 4 YEARS, IT IS NOT COVERED. TOYOTA NOT ONLY KNEW ABOUT THE DEFECT, THEY CHOSE NOT TO NOTIFY THE CAR OWNERS'. IF THE PISTONS HAD BEEN REPLACED, ALL OF THE OTHER DAMAGE WOULD NOT HAVE OCCURRED. SEVERAL MECHANICS I SPOKE WITH COULD NOT BELIEVE THIS WAS NOT A RECALL. IF AN ANIMAL HAD BEEN IN THE BACK OR WORSE YET, IF IT OCCURRED AS IT WAS OPEN, IT COULD POTENTIALLY CAUSE SIGNIFICANT INJURY OR WORSE. APPARENTLY TOYOTA DOES NOT HAVE AN OBLIGATION TO REPAIR THIS AS IT IS NO LONGER UNDER WARRANTY. IF DEEMED A RECALL THEY WOULD NEED TO ADDRESS THIS AT NO COST TO THE CONSUMER. WE CONTACTED TOYOTA; THEY HAVE OFFERED US $2500 AS 'GOODWILL', WHICH IS LESS THAN 50%. AT THIS POINT I AM HAVING THEM DO THE ACTUAL MECHANICAL WORK, AND WILL ALSO HAVE TO PAY OVER $1100 OUT OF POCKET TO COVER THE FULL COST OF THE MECHANICAL REPAIR. THIS STILL WILL NOT ADDRESS THE BODY WORK. THEREFORE THIS WILL DEVALUE MY CAR, AND ALSO RUNS THE RISK OF RUSTING AS THE PAINT IS DAMAGED.

TORQUE CONVERTER FAILED SUDDENLY WHILE CROSSING A HIGHWAY BRIDGE WITH NO BREAKDOWN LANE. NO PRIOR WARNING, JUST SUDDEN RPM JUMP TO REDLINE AND NO ABILITY TO ACCELERATE. WAS ABLE TO COAST ACROSS THE BRIDGE TO AN EXIT RAMP. DEALER DIAGNOSED THE PROBLEM AS A TRANSMISSION FAILURE. SINCE CAR WAS OUT OF WARRANTY, WE BROUGHT IT TO AN INDEPENDENT SHOP WHO FOUND IT WAS ACTUALLY THE TORQUE CONVERTER. THE SPLINE CONNECTION TO THE STATOR WAS BROKEN, EITHER THE WELD BROKE OR THE OVERRUNNING CLUTCH GAVE OUT. THIS IS THE SECOND TORQUE CONVERTER IN THE CAR. THE FIRST WAS REPLACED DUE TO A SHUDDERING ISSUE 2 YEARS AGO AT 45,000 MILES. THIS FAILURE OCCURRED AT 78,000 MILES. TOYOTA DENIES ANY RESPONSIBILITY SINCE THE SECOND FAILURE OCCURED OUTSIDE THE 1 YEAR REPAIR WARRANTY. MY DEALERSHIP CLAIMS THAT MY RAV4 HAD A CLOGGED SUNROOF DRAINAGE TUBE, BUT WHEN I TOOK IT TO ANOTHER DEALERSHIP WE FOUND ALL EVIDENCE TO THE CONTRARY. THE SERVICE MANAGER REMOVED TWO PIECES OF TRIM: ONE THAT SITS DIRECTLY BENEATH THE A-PILLAR (WHERE THE DEALERSHIP CLAIMS THE WATER TRAVELLED) AND ANOTHER THAT SITS BACK UNDERNEATH MY DASH AND GLOVE BOX. BOTH TRIM PIECES HAD PADDING (JUST LIKE THE CARPET PADDING) ON THE BACK SIDE. THE PADDING BENEATH THE A-PILLAR WAS BONE DRY. THE PADDING UNDERNEATH THE DASH WAS DAMP. PLEASE EXPLAIN TO ME HOW WATER TRAVELING DOWN THE A-PILLAR MANAGED TO MAKE A 90 TURN??? THE DEALERSHIP DID NOTHING BUT CLEAR THE "SUPPOSED" CLOG AND ASK IF I WANTED TO HAVE THE CARPETS AIR-DRIED FOR 24 HOURS. THEY MADE IT SOUND LIKE THEY DID ME SUCH A GREAT FAVOR ON THE INVOICE AND ALSO LIKE I WAS LEAVING THE HOSPITAL AGAINST DOCTORS ORDERS. WHAT THEY FAIL TO MENTION IS THAT A) I HAD ALREADY HAD THE CARPET PADDING AIR-DRIED THE DAY BEFORE AND B) I REQUESTED THAT THEY REPLACE MY CARPET PADDING AND REAPPLY THE BLACK RUBBER SOUNDPROOFING MATERIAL ATTACHED TO MY FLOORBOARD (THEY DECLINED). IN HINDSIGHT, NEITHER WOULD HAVE FIXED THE DAMAGE ALREADY INFLICTED UPON MY VEHICLE. BOTH TOYOTA OF NORTH AMERICA AND NCDS DECLINED MY CASE CLAIMING THAT MY "VEHICLE IS OPERATING AS DESIGNED." I THEN SCRAPED AS MUCH OF THE ICE OFF THE WIPER AS YOU CAN REASONABLY DO WITHOUT DAMAGING THE WIPER AND ICE AND SNOW OFF THAT PART OF THE WINDSHIELD WHERE THE BLADES REST. I STARTED TO DRIVE BUT ICE AND SNOW IMMEDIATELY STARTED ACCUMULATING ON THE LOWER PART WHERE DEFROST DOES NOT HEAT AND THE BLADES STARTED TO ICE UP AND THEY SCRAP ACROSS THE UNHEATED AREA. HAD TOTALLY UNACCEPTABLE VISION. WOULD STOP EVERY SO OFTEN AND SCRAP EVERYTHING BEST AS I COULD. START DRIVING AND A TOTAL UNACCEPTABLE VISION WINDOW ALMOST IMMEDIATELY. WHILE WE WERE NOT AWARE OF THIS ISSUE WHEN PURCHASING THIS VEHICLE NEW, SAFETY SHOULD NOT BE AN ADD ON YOU HAVE TO PAY FOR. TOYOTA PLACED THE BLADES WHERE THEY ARE SO THEY SHOULD DE-ICE. WITH A MODERATE BRAKE APPLICATION, IT FELT AS THOUGH THE PEDAL WAS ALMOST TO THE END OF ITS STROKE. I MEASURED THE PEDAL TRAVEL STATICALLY IN MY GARAGE = WITH 25 POUNDS EFFORT, THE TRAVEL WAS 2.5 INCHES. BUT WITH 100 POUNDS EFFORT, THE PEDAL WAS ABOUT 1/2" FROM THE FLOOR, MEANING THAT IN THE ADVENT OF A FAILED BRAKE CIRCUIT, THERE WOULD NOT BE ENOUGH PEDAL TRAVEL TO SAFELY STOP THE VEHICLE. ALSO, THE PEDAL EFFORT V. TRAVEL FELT LINEAR, AND NOT ASYMPTOTIC AS ONE WOULD EXPECT FROM A BRAKE PEDAL. I TOOK THE RAV4 TO THE DEALER SERVICE DEPT. THEY REPORTED THAT THEY FOUND NO ISSUES AND NO SERVICE WAS DONE. AFTER THE VISIT TO THE DEALERSHIP, THE BRAKE PEDAL FEEL WAS COMPLETELY DIFFERENT. THE PEDAL TRAVEL AT 25 POUNDS EFFORT WAS NOW 1.5", WHICH FELT NORMAL. ALSO, THE PEDAL FEEL WAS NOW ASYMPTOTIC AND PLENTY OF TRAVEL WAS LEFT AVAILABLE TO ALLOW FOR A SAFE STOP WITH A FAILED CIRCUIT.
