ODI #11746247TIRESCRASH

What component or system failed or malfunctioned, and is it available for inspection upon request? I purchased four tires from Belle Tires on April 1, 2026, 14 days ago (note: mileage at accident 10,776 vs mileage at tire purchase 10,360 or 416 miles of wear). On Wednesday, April 15th, I was about to change lanes after entering the Tri-State Expressway (route 294) going southbound from Half-Day Rd. (route 22), I had crossed the first two lanes without incident, and moving into the third lane, when I experienced a blowout of my driver’s-side rear tire. The car started a fishtail counter-clockwise spin and crossed the third and the fourth lanes (i.e., the far left lanes) before smashing into the concrete median, separating my Southbound lanes from the opposing traffic lanes. I have kept both the tire and rim in my garage. How was your safety or the safety of others put at risk? As I started sliding or spinning, I cannot honestly say, whether or not I hit the brakes or can really describe the spinning that occurred. As the rim of blowout tire was evenly torn up on its outer edge, I know that the tire was turning. All I can recall is hearing the car hit the concrete median twice. I know that it had to be counterclockwise since I started going southbound, and after the car came to a rest next to the median, I was facing southbound again. Has the problem been reproduced or confirmed by a dealer or independent service center? I took the tire on it’s rim to the Belle dealer who would not acknowledge a blowout, but he kept insisting that he is not an expert on crash interpretations. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Allstate Insurance, Toyota used a Bosch representative to retrieve the Electronic Data Recording (EDR) information. I have copy of this report available for expert interpretation. No warnings were given prior to this blowout.

ODI #11739187TIRES

I heard loud sound then the tire of driver’s side front was blown out suddenly with warning alarm on the 210 east bound freeway on 10th of May, 2026 at 1:40PM, I stopped in the shoulder then called AAARoad service to change it to the spare tire. The service technician said that the tire has defect and blown out. I visited Toyota Pasadena to change the tire, the service representative suggested to claim the defect to Bridgestone personally because my mileage of car was only 33,416 and I have still warranty of this tire. I reported to Bridgestone but they didn’t do anything to follow up. Tire size is 225/55R29

ODI #11712196ELECTRICAL SYSTEM

The contact owns a 2024 Toyota Venza. The contact received notification of NHTSA Campaign Number: 25V595000 (ELECTRICAL SYSTEM). However, the part to do the recall repair was not yet available. The contact stated that while driving at an undisclosed speed, the screen displayed a black screen while the vehicle was in drive(D). The contact stated that the failure obstructed the driver from safely operating the vehicle. The contact stated that the failure was most predominant when the weather was cold. The local dealer was contacted and confirmed that the part was not yet available. The vehicle was not repaired. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue. The failure mileage was unknown.

ODI #11702659UNKNOWN OR OTHERCRASH

Last year my vehicle rear camera and sensor failed to which caused me to backup into a mailbox. I had brought it up after a car wash at the dealership and they said they didn't know about any recalls at the time. I recently saw the recall for this year and make and would like this to be investigated further. I also have an estimate for the damages that I can provide.

ODI #11690439SERVICE BRAKES,FORWARD COLLISION AVOIDANCECRASH

After applying the brakes in a sudden situation, the automatic emergency braking came on but the car didn't stop. The brake pedal was pushed all the way to the floor board but the car didn't stop and crashed into the vehicle in front. This happened once before but didn't end in a crash. This was a total brake failure.

ODI #11656854ELECTRICAL SYSTEM,BACK OVER PREVENTION,FORWARD COLLISION AVOIDANCE

-Issue with electrical system. Unable to access to connect services (APP, SOS). Contacted Toyota three times to let them know about this issue. -Collision Warning/Rear Cross Warning don't always operate. There is no sound or warning. -Occasionally, screen turns off completely when driving. -Navigation system is always confused and doesn't give proper directions. -Carplay not able to connect to phone sometimes. - -

ODI #11645248EXTERIOR LIGHTING

When driving at night using low beams a shadow is cast by the headlights. The shadow is on the driver's side approximately 100 feet in front of the vehicle, at the 11:00 position. This gives the impression that someone or something is entering the path of the vehicle causing the driver to brake or swerve to avoid a collision. This endangers the occupants of the vehicle and others on the roadway. If the driver grows accustomed to this and "learns" not to pay attention to this area of his/her path it endangers those who enter this area.

ODI #11633986ELECTRICAL SYSTEM,BACK OVER PREVENTION

The other day when I was backing out of a parking spot my car almost backed into 2 people. There was no alert from the rear cross traffic warning.

ODI #11633987ELECTRICAL SYSTEM

I don’t have the specific date that this happened because it was several times. When I first got the car the Toyota app stopped working after a few days. I brought it back a couple times and they replaced the headpiece because the app system crashed. Then the CarPlay stopped working not long after the head piece was replaced and the app stopped working. After going to the dealership for eight or nine times they finally ordered me a DCM. While waiting for that part, the auto start via key fog was not heating up the car. So with presetting the defrost to be on for auto start, the hazards came on when I auto started it. I happened to have had it at the service department when it happened and the mechanic said he didn’t know how to shut it off so he had to disconnect the battery. While I was speaking with Toyota about it on the phone, they asked me to try it out to see if the problem was gone and it wasn’t and I couldn’t shut the hazards off. So I drove with the hazards on and eventually the hazards turned off, but the CarPlay screen completely shut off when the hazards turned off. Also, when the hazards were on the car didn’t shut off when I opened the door. Also, another incident was that there was an acceleration issue when going to accelerate from a stopped position at a light. When I pressed on the gas the car started to move and then it went to a complete stop as if I was pushing on the break, but I wasn’t on the break. I was on the gas. I even looked down to see that my foot was on the gas. After pumping the gas pedal, it finally started to go. All of the above issues so far seem to be resolved after replacing the DCM. Like I said there’s too many incidences above for me to give a date to which all of this happened but it’s within the last few months I got this car in August.

During the commute on the way home on [XXX] between Harrisburg and Lebanon PA a stone kicked up in a construction zone and impacted the driver side windshield. At that time there was no visible damage until later in the drive there was a 3in long crack in the driver side line of sight. As the drive continued the crack continued across the line of sight. Over the course of 2 days the crack went from 3inches to halfway across the windshield. The stone crack was no bigger than a size of an eraser on a pencil. I believe this needs investigated just like the 2021 model's with defective windshields. Toyota care (ref [XXX]) was called, they declined my incident. My purchased extended warranty for chips, cracks, dents was also denied because its bigger than 3inches. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

The contact owns a 2024 Toyota Venza. The contact stated that while his wife was driving at 10-20 making a left turn, a Land Rover veered in her path, and she crashed. The auto collision system did not engage.  The brakes were applied but failed to respond. There were no warning lights illuminated. There were no reported injuries, fire or air bag deployment, or police reports filed. The vehicle was taken to the local dealer but was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 2,500. The VIN was not available.

The contact stated that while attempting to make a left turn, the brake pedal was depressed; however, the vehicle failed to respond, and the brake pedal became inoperable. The vehicle was not diagnosed or repaired. The VIN was not included in NHTSA Campaign Number: 24V708000 (SERVICE BRAKES, ELECTRIC, SERVICE BRAKES, HYDRAULIC). The multimedia display, which shows the gps mapping being sync'd from my phone has a pop up advertisement ""experience drive connect"" for a subscription service. According to multiple dealers this can not be disabled or turned off except by subscribing to Toyota Drive Connect. So, without a subscription, the multimedia display is disabled since the first screen which is an advertisement can not be removed or disabled in order to utilize the multimedia display. This has created a safety hazard for many posters on ToyotaNation forums and other sites. I believe this problem started with the 2023 version of their display and was not present on 2022 or earlier models.

Was driving the speed of traffic eastbound on [XXX] near [XXX] merge just east of Des Moines in heavy, heavy semi traffic when tire went flat. I had been driving for several hours since my previous stop and was in the far left lane when noise a loss of control made me pull off the road. I have pictures & a video that appears to show a tire defect. Neither Toyota and Bridgestone paid for my replacement tire, nor admit to a defective tire. I am concerned my other factory installed tires are also defective, and neither company are interested in my pics/video. Since I already purchased a replacement tire, they have no interest in my concerns. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I am YU Sun. I met a car accident. I drive from south to north. I drove my new Toyota Venza( less than one month) entering into an intersection at 30/35miles on green light. The other car turned left with yellow and striked my left corner of my bumper. The speed is very fast. The other car is Lexus and its’ air bag all came out. My can was serious damaged. I am trying to locate any information and updates on my second key fob that was not received at the time of the purchase of my car. Every month I talk with Toyota they say by the end of this month you should receive your letter, so the end of the month passes and I reach out to toyota and they state it was pushed back to the end of this month and so on. I am getting very upset especially when you purchase a $50,000.00 automobile. I can't find any discussions on the Venza key fob shortage but there are discussions on every other model of Toyota. I am leaving on a 7 week trip and god forbid something happens to my one and only key what am I going to do then?

ODI #11586431STRUCTURE,UNKNOWN OR OTHER

Front windshield randomly cracked, with number and size of cracks growing as vehicle is driven. Cracked windshield is not a result of any rocks or debris strikes.

ODI #11578800EXTERIOR LIGHTING

When driving at night with headlights on low beam the driver’s side headlight casts a distracting shadow on the road. Not only does this shadow prevent full illumination of the roadway, limiting visibility on the driver’s side, but the shadow can also make it look like an obstacle might be in the road. This shadow is not there when the bright lights are on. I have had this vehicle since Dec. 15, 2023 & I have had two different Toyota service departments check this out. They both told me there is no fix for the shadow. I also spoke with Toyota Corporation by phone to report the problem. They told me there is no program in place to address the issue. I have seen multiple reports of this problem on the Toyota Nation website stating this problem has existed for three years.
