Critical Deferred Risk
Critical deferred failure risk — 3 safety-critical or high-cost components are past their service interval. Without immediate attention, repair costs will compound significantly.
No odometer data on record. Mileage is estimated at ~96,000 mi based on average annual driving. Items marked EST. OVERDUE may already be due — verify service history with the seller. Items marked OVERDUE are confirmed past-due based on vehicle age alone.
DUE SOONCriticalTiming Chain Tensioner
Service interval: 120,000 mi · Est. ~96,000 mi (no odometer on record)
Schedule Timing Chain Tensioner service before 120,000 mi
Worn chain tensioners cause timing chain skip or snap — common on GM 5.3L/6.2L, Ford EcoBoost, VW/Audi EA888, BMW N47/N63. Listen for cold-start rattles.
EST. OVERDUECriticalWater Pump
Service interval: 8 yr · Vehicle age: 8 yr · Past due by age
Service Water Pump immediately — already past the 100,000 mi / 8 yr interval
Water pump failure causes rapid overheating. If the engine overheats even once, head gasket damage is likely — turning a $400 part into a $3,000+ repair.
EST. OVERDUEHighTransmission Fluid / Service
Service interval: 5 yr · Vehicle age: 8 yr · Past due by age
Service Transmission Fluid / Service immediately — already past the 60,000 mi / 5 yr interval
Degraded transmission fluid destroys clutch packs and solenoids. CVT transmissions are especially sensitive — neglected fluid can result in a $4,000–$8,000 replacement.
EST. OVERDUEHighCoolant Flush
Service interval: 5 yr · Vehicle age: 8 yr · Past due by age
Service Coolant Flush immediately — already past the 100,000 mi / 5 yr interval
Degraded coolant loses corrosion inhibitors and attacks aluminum engine components. Can cause head gasket failure and water pump corrosion.
EST. OVERDUEModerateSpark Plugs (iridium/platinum)
Service interval: 8 yr · Vehicle age: 8 yr · Past due by age
Service Spark Plugs (iridium/platinum) immediately — already past the 100,000 mi / 8 yr interval
Worn spark plugs cause misfires that can foul and crack catalytic converters. A $150 tune-up can prevent a $1,200 catalytic converter replacement.
DUE SOONModerateFuel Injectors
Service interval: 120,000 mi · Est. ~96,000 mi (no odometer on record)
Schedule Fuel Injectors service before 120,000 mi
Dirty or failing injectors cause rough idle, reduced fuel economy, and increased emissions. Cleaning costs $100–$200; replacement $800–$1,600.

Like many other complaints. My 2018 Mazda cx5 GT is having the same issue with the cracked cylinder head. I only have about 36-38k mileage on it ( I forgot the exact). Mazda dealership will not cover the cost. NEVER had an accident to the front and only one accident to the back when I got rear ended in 2019. If this is a wide spread issue, why is this not a recall? and why is it only the turbo engine gets extended warranty? The repair cost right now approximately $9,500. I never do any maintenance at Mazda so I doubt they will give me the good will assistance but I am still waiting for an answer. I hope NHSTA will do something because this issue will continue with all other owners that have the same year model.

My 2018 CX5 just hit 50,000 miles recently and I smelled something burning and saw smoke coming from the hood while I was stopped for a school bus. I immediately stopped driving due to fire concerns. The vehicle has a cracked cylinder which is a known issue for this vehicle. Mazda has no plans to correct this issue other than issuing a technical bulletin. I feel that Mazda should repair this at their cost because it is a defect and the vehicle only has 50,000 miles. Instead I have to pay nearly $5,000. This is hard to believe! Transcript from dealer: The technician reported an oil leak from the cylinder head, specifically indicating a crack in the number four lifter valley that is causing fluid to leak down the engine block. This issue is recognized as a known problem, and the technician will check if it is covered under warranty. Repairing the cylinder head will require significant labor, including dropping the engine, and will take some time to complete. Services being performed: - Oil change. - Inspection for oil leak issue. Recommendations: - Replacement of the cylinder head due to the identified oil leak. Multi Point Inspection $4870.22

We purchased this vehicle used from Mazda of Orange in July 2025 with 52,399 miles on it. After 6 months of ownership and approximately 3,000 miles there was a burning smell. We took it to our local mechanic and the following repairs were required: Replacement of cracked belts, Replacement of cracked axle boots, Replacement of an excessively loose front axle, Replacement of valve cover gasket. Cost was $3,000 and the dealer refused to reimburse us. Now the car is leaking oil and needs a new head gasket and cylinder head surface machined. Quote was approximately $3,500. We have requested Mazda perform this repair. Will you please look into these vehicles? These are significant defects on a car with such low miles. We also believe that some of these defects were know to the dealer but are unable to prove it. We believe our vehicle is part of a batch that has signficant mechanical defects. Current mileage of the vehicle is 56,865.

Unintended acceleration incident. I entered the parking lot of a business where the Salvation Army is located to drop off a donation. The parking spaces were on the left side. As I pulled into a parking space and was in the process of parking, the vehicle suddenly lurched/accelerated forward unexpectedly and struck the concrete wall of the building. The airbags did not deploy despite the frontal impact. No one was seriously injured, but my husband, daughter, and I experienced soreness the following day from the impact. Thankfully, I had chosen a parking space in front of a concrete wall rather than the glass storefront windows, which could have resulted in far more serious injuries or damage. I contacted my insurance company, and the vehicle has since been repaired.

Sunroof blew out while driving down interstate highway. There was no impact that we are aware of, just a complete failure of the sunroof glass. The sunroof was closed, with the shade also closed, so no glass fell down into the vehicle. The glass blew out with a very loud explosion that startled and disoriented all within the vehicle causing us to swerve and look around for what had happened. The glass has been inspected and is being repaired by a local auto glass shop today. No warning lamps or otherwise. I have pictures of the glass if needed.

I am experiencing “ghost touch” from the faulty laminate on the touch screen. Can literally see it degrading. It is a widely known issue and is dangerous cause I’m cruising along at 70 on the highway then all of a sudden my car randomly changes to different screens or changes sound settings. Very distracting and disconcerting. Unbelievable this hasn’t been a recall - government is certainly complicit in this now.

My CX-5 has 86,120 miles. I noticed a burnt oil smell coming into the car and later found oil droplets on my garage floor. I took it in to my neighborhood garage and they found an oil leak coming from the cylinder head. They recommended I take to my Mazda dealer. I had it towed to Pearson Mazda, Richmond,VA. They inspected it and stated I had a cracked cylinder head. I have a 100K extended warranty that will cover most of the cost to repair it. However, I found the Mazda CX-5 2018-2021 models have a cylinder head defect. I found a Mazda Service Alert, #SA-031/21 and on Reddit a Mazda CX-5 owners page that has many stories of the same problem, cracked cylinder head. Some have had Mazda cover the repair cost or given a major discount and provided an extended warranty after repair. I reviewed this with Pearson Mazda and they stated that my car does not fall in the vehicle numbers that they will cover. This is a known issue and they will not acknowledge it for my car. I have contacted Mazda Corporate and waiting for a callback. Litigation has been filed for the CX-5 Turbo customers and they have received restitution. This is also a major issue on non-Turbo customer’s SUVs. I have read that the 2018-2021 manufacturing (casting) of the cylinder head was defective. However, Mazda is not admitting it because a RECALL would cost them millions to fix the cylinder head issue on all the affected vehicles. But, they are at fault and should work with all customers that have been affected by this issue. Therefore, I am requesting NHTSA to make this a public safety matter and have Mazda be accountable for the issue. Thank you!

My 2018 Mazda CX five experience catastrophic engine failure at approximately 119,000 miles despite proper maintenance. The failure resulted in a repair estimate exceeding $9500. This type of failure raises concerns regarding potential engine defects or premature mechanical breakdown. Mazda acknowledged the issue by offering a minimal Goodwill payments, but refused meaningful assistance. I am reporting the issue to determine whether similar failures are occurring with Mazda and further investigation is warranted.

The head gasket on the engine is cracked and is leaking oil and causing the oil to burn when car is driven. The problem was identified by a local mazda dealership and it is a well documented and known issue among SkyActive non-turbo engines. There were no warning lights or messages on the car, other than the smell of burning oil and smoke coming from the engine after driving. I have a toddler and the smoke and burning smell coming into my car every time I drive is concerning to our health. Mazda is well aware that the cracked head gasket issue extends beyond their turbo engines but did not include the non turbo engines in their extended warranty.

I am reporting a safety issue involving engine failure and fumes entering the vehicle cabin as well as unintended braking post major engine repair at 75,000 miles on my 2018 Mazda CX5. My vehicle developed engine problems consistent with a cylinder head/coolant defect and began allowing fumes/smoke to enter the cabin while driving. This created a health and safety concern for occupants. I reported this to the dealership and was told the vehicle was safe to drive despite the presence of fumes in the cabin. A major engine repair was later performed. A few days following the repair, additional warning lights and system malfunctions appeared, including brake system warnings, and the vehicle later braked unexpectedly while driving nearly causing me to be rear-ended. The vehicle has been inspected by the dealership, but the root cause of the fumes and subsequent safety issues has not been clearly explained. This situation created a safety risk due to exposure to fumes inside the cabin and unexpected vehicle behavior while driving. Smoke/fumes video and any additional documentation can be provided upon request. Attaching the costly invoice as well.

Vehicle Information Year/Make/Model: 2018 Mazda CX-5 Engine: 2.5L Mileage at Failure: Approximately 115,000 miles Component: Engine Complaint Summary: I am reporting a sudden and unsafe engine failure involving a cracked cylinder head in my 2018 Mazda CX-5. This failure occurred without warning at approximately 115,000 miles and resulted in significant oil leakage from the engine. Description of the Failure and Safety Risk: While operating the vehicle, I detected a strong burning oil odor followed by visible smoke coming from the engine compartment. Upon inspection, it was determined that the cylinder head had cracked, allowing engine oil to leak onto hot engine and exhaust components. This condition presents a serious safety hazard, including the risk of fire due to oil contacting high-temperature surfaces. Notably, there were no warning lights, messages, or prior indicators of any issue before the failure occurred. The vehicle has been properly maintained and has not experienced overheating or misuse that would reasonably lead to this type of failure. Repair Information: The vehicle was diagnosed by an authorized Mazda dealership, which confirmed a cracked cylinder head. The estimated cost of repair is approximately $5,130. Additional Information / Defect Pattern: Through independent research, I have identified multiple reports of similar failures involving Mazda 2.5L engines in comparable vehicles. These reports describe nearly identical symptoms, including sudden oil leakage, smoke, and cracked cylinder heads at relatively low mileage. This pattern suggests a potential manufacturing or material defect rather than normal wear and tear. Manufacturer Response: I have contacted Mazda Customer Experience and escalated the matter. Mazda has declined to provide any assistance, citing the vehicle being out of warranty, despite the nature of the failure and associated safety concerns. Summary: This issue involves a critical engine component failure that can le

My 2018 Mazda CX-5 AWD (2.5L SKYACTIV-G, non-turbo) was diagnosed by a Mazda dealer with a cracked/leaking cylinder head, a known manufacturing defect in the casting of the aluminum cylinder head. Engine oil is leaking from the rear of the cylinder head onto the exhaust manifold, creating a burning oil smell and visible oil accumulation at the rear of the engine. This is a documented fire hazard. The Mazda dealer confirmed the diagnosis matches Technical Service Bulletin SA-031-21, which Mazda issued in May 2021 specifically for this defective cylinder head design on 2018–2019 CX-5 models. The dealer additionally informed me that Mazda has internally acknowledged this as a growing, widespread problem across the 2018–2021 CX-5 model years, and that a recall may be forthcoming. Mazda customer service denied my goodwill repair claim despite acknowledging the TSB. The estimated repair cost is $7,000. The vehicle is currently not being driven due to the fire risk of oil contacting the exhaust manifold. This defect appears to affect a significant number of 2018–2021 Mazda CX-5 vehicles. I am requesting NHTSA open a formal safety defect investigation and compel Mazda to issue a recall. Multiple other owners have reported the same issue on NHTSA and in owner forums. This is a manufacturing defect not normal wear and tear and poses a serious fire safety risk to consumers.

2018 Mazda CX-5, 58,182 miles. Report date is 3/7/2026, low mileage for it's age. Vehicle has been exclusively purchased and serviced at the same authorized Mazda dealership for the lifetime of the vehicle, with all maintenance records on file. I began experiencing a strong burning oil smell inside the passenger cabin while driving. The fumes were noticeable to both the driver and passengers, creating a potential carbon monoxide/combustion fume exposure hazard that posed a safety risk to all occupants. Upon bringing the vehicle to the dealership, the technician diagnosed a cracked cylinder head requiring full replacement. This is a significant structural engine defect. Despite the vehicle being dealer-serviced its entire life, the repair cost was initially placed entirely on the owner. Mazda eventually provided partial financial consideration, though the root cause of why a cylinder head cracked on a well-maintained 58,182-mile vehicle was never adequately explained. I am filing this complaint because fumes entering the passenger cabin represent a serious safety defect, and I believe this issue may affect other 2018 CX-5 owners.

All of a sudden, my engine started making this ticking noise. I brought it to a mechanic, and he said the noise is coming from inside the engine. He checked for codes and everything, but nothing came up. The only way to look at it is to take the engine apart from what he told me. I started researching for solutions online before doing this and I came across other Mazda CX5 owners complaining of the same thing. I listened to a few of these videos of the sound it makes and sounds just like my car. Apparently, it is a common issue connected with the Hydraulic Lash Adjusters, and I am amazed that Mazda has not reached out to people affected by this. I am not sure what the long-term issue is with this noise, but it can't be good if it's in the engine. Who knows what kind of damage it could cause.

Noticed the smell of burning oil and smoke from the engine while driving on the interstate with my husband and young children in the vehicle. Pulled over at a gas station to inspect and discovered large amounts of oil leaking. While at the gas station the low oil indicator came on. There were no warning or check engine lights prior to the incident. The dealership diagnosed cracked cylinder head. This was an extensive and costly repair. I have now learned that Mazda is aware of how common this issue is with this year/make/model. This should be a covered repair or recalled part.

Turned car on when leaving for work. Smelled a strong burning smell. Fluid all under car on the driveway. Took the vehicle to repair shop and they stated that it was the cylinder head and that there was a possible recall or warranty extension for this particular issue. While driving, my 2018 Mazda CX-5 experienced a sudden and complete loss of engine power without warning. The engine shut down while the vehicle was in motion; however, electrical systems such as the radio, heat, and electronics remained operational. The vehicle lost propulsion, creating a serious safety risk in traffic. The vehicle had recently been serviced at an authorized Mazda dealer for completion of PCM and fuel-system recalls. Shortly after recall service, the engine failure occurred. No warning lights or prior symptoms were present. An independent mechanic suggested possible engine seizure, but a full diagnostic evaluation (including compression testing or internal inspection) has not been completed. The cause of the sudden power loss remains undetermined. This incident represents a potential safety defect involving unexpected engine shutdown and loss of propulsion.

Cracked cylinder head due to defect in the casting process. Oil leaking thru the crack landing on the exhaust manifold causing an odor and possibly toxic fumes in the cabin of the vehicle.

ODI #11705314ENGINE

Mazda has inspected the car and determined that there is a crack in the cylinder head. This is apparently a known defect in 2018-2021 Mazdas with 2.5L engines, including, without limitation, the 2018 Mazda CX-5. The cracked cylinder head has caused leaking coolant, which can drain into the engine and create serious safety issues. Mazda is aware of the issue, however, they are claiming that it is not covered by warranty.

The contact owns a 2018 Mazda CX-5. The contact stated that while driving at 45 MPH, there was smoke coming from the engine, and the contact smelled an oil-burning odor. No warning lights were illuminated. A dealer was contacted. The vehicle was towed to the dealer where it was diagnosed and determined that the cylinder head was damaged, causing oil to leak into the cylinders, and that the engine needed to be replaced. The manufacturer was notified of the failure, and promised to open a case and give the contact a callback within 24 to 48 hours. The approximate failure mileage was 59,000.

2018 Mazda CX-5, 2.5L Naturally Aspirated Engine The vehicle has been diagnosed with a leak from the cylinder head cover, resulting in engine oil dripping directly onto the exhaust manifold. This issue causes noticeable smoke and a strong burning oil odor inside the cabin while the engine is running. This defect presents multiple serious safety concerns: 1) Health hazard due to prolonged exposure to burning oil fumes inside the vehicle. 2) Fire risk from oil contacting hot engine components. 3) Potential engine failure, which could lead to vehicle stalling or loss of control, increasing the risk of a crash and possible loss of life. Mazda issued Technical Service Bulletin TSB #SA-031-21, which introduced a redesigned cylinder head for 2018–2019 CX-5 models. The release of this TSB indicates Mazda’s awareness of the underlying design flaw. There is substantial evidence, including numerous owner reports, that this is a widespread issue affecting these model years. The defect is not related to vehicle mileage or user negligence. Mazda should be held fully accountable and take corrective action by issuing a formal safety recall or offering an extended warranty to affected owners. Immediate attention to this matter is necessary to protect consumer safety and prevent future incidents.

I own a CX5 2018 Mazda with around 31,000 miles on it. Two days ago I experienced a smoke/fume event in my car while driving. The cabin filled with burnt oil fumes and a pungent smell. I happened to be near a Mazda dealership and was able to reach it with windows down and slow driving. The dealership inspected my car and confirmed a large oil leak caused by a cracked cylinder head. I was told this is a known issue with Mazda CX5/CX9 series and that it would be covered under warranty. Today I was informed by the same dealership that my standard powertrain warranty will not cover it, as it is 60,000 miles or 5 years. My car only has 31,000 miles on it, but it’s over the 5 years by little over a year. I am looking at paying around $6,500 out of pocket to fix my vehicle. I asked if there is an extended warranty from Mazda specifically regarding cylinder heads, due to this being a known defect, however the service person said there isn’t for my car. After some online research I did come across an extended powertrain warranty applying to 2019-2020 CX5 models due to the cylinder head cracking issues. I ask that this is being looked into and that the warranty needs to be extended to the 2018 model as well.

A former Mazda mechanic posted he would see a couple vehicles a week at his dealership that experienced this problem. The odor of the burning oil onto the manifold is overpowering when sitting in the car. Some people with the same problem have posted they actually had small fires / burning under the car. In May, 2021, Mazda issued a technical service bulletin TSB # SA-031-21 with a cylinder head redesign for 2018/2019 CX5 models. this is telling of their awareness of the problem. The cylinder head on my 2018 Mazda CX-5 developed cracks on the exhaust side, leading to significant oil leakage onto the exhaust manifold. This has caused engine damage and safety concerns. The vehicle is currently at a Mazda dealership, where the issue has been inspected and documented. The component is available for further inspection upon request. Additionally, the vehicle experienced burning smells and visible smoke coming through the air vents, which could impair visibility or cause distraction while driving. These conditions place the safety of myself, my passengers, and others on the road at risk. The issue has been confirmed by a Mazda dealership. They provided a video showing oil actively dripping from the cracked cylinder head and stated that the engine requires a full replacement. Mazda dealership video: [XXX] This video contains the engine inspection findings conducted at the dealership during my recent visit. It clearly shows the damage and highlights the issues identified. The cylinder head cracked and broke resulting in a massive oil leak that covered all the parts under my car and under the hood and engine which in result started to smoke. This is not only a major issue of extremely costly repairs but more importantly also serves as a major safety issue. This could had caused fire to the vehicle and also engine misfire resulting in engine failure while in drive and potentially lead to critical injury or death for me and my family. Mazda was aware of their poorly designed engineering for this critical and extremely important component and it is known Mazda recognized the issue by issuing a TSB. After further research there are numerous TSB issued for multiple engine failures with Mazda. It is also known that certain VIN number vehicles manufactured with this defect are commonly reported and have experienced this exact same issue within only a few years and or very low mileage. This is a major mechanical failure to happen to vehicles within a short time span of purchase and results in loss of customer trust and loyalty. As a first time car buyer I am deeply dissatisfied and disappointed with Mazda. I would not had purchased a brand new car if I would had known I would experience this only at 5 years and 71 thousand miles. My vehicle was inspected and diagnosed by a certified Mazda dealership. I was advised and informed that this is a huge issue that has happened with all customers. Mazda knowingly neglected the proper manufacturing and engineering for the cylinder head and other engine parts, which is the most critical component to any vehicle to function and operate. Lastly what is more concerning was that there was no warning signs such as lights or sensors that were triggered as warning of the issue and vehicle failure posing as another safety concern for drivers.

Mazda informed me that they would cover roughly 37% of the repair costs that the dealership quoted me. I've read online on various Mazda forums that many owners are dealing with the same issue and Mazda admitted that a batch of heads have a manufacturing defect. Also, my wife's newer model, '21, could also be a lemon for the TSB. We're looking for Mazda to step up for it's (loyal) clients! The work was completed and paid for on [XXX]. Mazda reviewed our case ([XXX]) and declined to assist. A reliable vehicle, Mazda, with an engine at 54,000 miles should be cared for by the manufacturer. The reason the time expired and the mileage was low was due to the lack of driving during the Covid era. We have researched online the number of cases for Mazda and the CX-5's issues with the cylinder heads. We are looking for a government interaction to assist consumers like us. We own a 2018 Mazda CX-5 which developed a cracked cylinder head after slightly over 53,000 miles. We had oil leaking in our driveway and our dealership advised us not to drive it as this would be an extreme safety issue. I was advised to contact you as this seems to be an ongoing issue with Mazda CX-5's from 2018-2021. There is a safety bulletin TSB #SA-031-21. I am hoping this will help with resolving this with Mazda costumer service.

ENGINE HEAD HAS CRACK ON IT, CAUSING OIL TO LEAK OUT AND BURN AND SMOKE. IN ORDER TO REPLACE THE ENGINE/CRACK, MANIFOLD NEED TO BE REMOVED. LEAKAGE IS IN THE BACK OF THE ENGINE. THIS MUST BE MANUFACTURED DEFECT. MY MAZDA HAS LESS THAN 45000 MILE ON IT. UNFORTUNATELY WARRANTLY IS NO LONGER VALID. UNLESS A RECALLED IS ISSUE, A REPAIR WILL BE FREE OF CHARGE TO CURRENT OWNER. OTHER WISE OPERATOR HAS TO CONSTANTLY KEEP ADDING OIL.

Cracked cylinder head oil drips onto exhaust manifold smoke gets into cabin and could cause fire on exhaust manifold car was purchased used 5/22 and with 46732 miles and now has 74000 in 2/24 it is now at central Maine Chrysler jeep where we purchased it having repairs done under extend warrantee that we bought with the car.we spoke with the service department at a mazda dealer and they said it is very common i feel that mazda should recall it passengers should not have to breath oil fumes or risk being in a car fire we are lucky to have purchased the warrantee but without it the repairs would be to cost prohibitive for the average person thanks

-cylinder head cracked resulting in leaking and burning oil from rear of cylinder head by exhaust manifold. Car has since been repaired by Mazda, but pictures are available from the incident. -Car was unsafe to drive due to leaking oil all over road (Environmental risks and hazard to others), black smoke and burning smell circulated into cabin via AC (health risks to passengers). Prolonged driving in this condition would have resulted in loss of oil and further damage to the car and others. -Car was towed to independent service center then subsequently towed to Mazda dealership -Mazda dealership inspected and confirmed that it was a manufactured defect that they have seen with other cars. Mazda has a Technical Service Bulletin (TSB) issued as well. -No warning lamps appeared prior to observed smoke and leak, though it was noticed that the car did jerk slightly at times when stopped at a traffic light

Defective cylinder head resulting in oil leak. Confirmed and inspected by dealer. No warning messages or symptoms. Just smell of burning oil.

I just had my vehicle in for routine maintenance and oil change and three days later it started leaking oil, staining my driveway, with the leak getting progressively worse then my check oil light went on, and the day I brought it in, and it was smoking under the hood. My safety and the safety of others on the road was at risk. At the routine maintenance visit they found no issues with the vehicle. After bringing it into the dealership they diagnosed it as a cylinder head crack needing replacement. We had another dealership look at the diagnostic video and explain that this is poor manufacturing. It’s just over 60,000 miles but is still within our warranty from a timeline aspect. Upon research we found this is happening to many people with a Mazda’s from year, 18 to 21. There are many forums about this. Upon an online search we found this problem documented as tsb SA-031/21. I am disappointed that the Mazda company is aware of the faulty cylinder heads in this vehicle, and now I am responsible for the maintenance charge.

I own a Mazda that I purchased new in 2018. I have kept up with the maintenance to a “T”. I have never abused it. About a month ago I started smelling a strange smell. Like smoke or burnt oil. A couple of weeks later, the engine oil level light came on and the engine was low on oil and that’s when I started seeing that the motor was leaking oil, and I just found out that the engine head cylinder has a crack. I found out that these vehicles have a defect from the factory on this part that cracked, “Engine Head cylinder”. And now to repair it, it is going to cost me around $6,000. The warranty has run out.

Oil leak. Smoke from the engine. Inspected by the local body shop, we were told that the leakage would probably come from cylinder head. [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Engine oil started leaking after I noticed smoke coming from the hood of the car. No warning signs, no check engine light, nothing to indicate any issues until this occurred. Took the car to the dealership who confirmed it was a cracked cylinder head which is now being replaced.

ODI #11524541ENGINE

Driving down the freeway smoke started coming out of the hood of the car and into the cabin, no warnings or warning lights of any kind, got the car to the Mazda dealer to be inspected. The dealer told me it was 5 months out of warranty and would not be covered its a cracked cylinder head and oil had dripped/leaked onto the manifold causing the smoke. No smoke was coming from the exhaust so the crack only leaked oil to the outside of the cylinder head. There is no oil in my driveway so it happened that day on the road. This is a manufacturing error that Mazda won't take responsibility for.

ODI #11519930ENGINE

When driving I started to smell a burning oil or rubber smell inside the cabin of the car. When I stopped I could see smoke coming from under the hood. When I was able to park, I opened the hood and could see smoke coming from the engine. There were no warning lights displaying on the dash, no leaking fluid from under the car. I did not feel safe continuing to drive the car without having it seen, so I took it to the closest Mazda dealer service. After inspecting the issue I was told there is a cracked cylinder head that is leaking fluid into the engine and the exhaust and I would need a brand new engine. The service manager said this is a known issue with the 2018-2021 Mazda CX-5. Unfortunately, the warranty that would cover this expired in April 2023 (at the time of writing this, it's May 1, 2023). It should be noted that my CX-5 is only 5 years old with ~37,000 miles on it. I was told by the Mazda service manager that the car is not safe to be driven in this condition and would have to stay in their possession on their lot until I decide if I want to have it fixed.

ODI #11518839ENGINE

Crack in cylinder head at 25k caused oil to leak and engine to smoke while driving. No warning lights came on. Dealership replaced the part but how do I know it won't happen again? Mazda knows about this but has not issued a recall or extended the powertrain warranty to cover this known manufacturing defect.

Chart Analysis

The blue line shows 104 total NHTSA complaints filed over 4 years, with the worst year being 2024 (36 complaints).

The amber reliability line is declining — recent years score around 10/100, lower than earlier in the vehicle's life. This pattern indicates issues are accumulating over time rather than being resolved, which is a concern for used buyers.

Notably, despite 104 complaints on file, no recall has been issued — either the defect pattern has not met NHTSA's threshold for a formal campaign, or a recall may be forthcoming.

Red dots on the amber line flag 2026, a year when complaints included crash events — these are the highest-severity incidents in the dataset.
