The contact owns a 2015 Mazda Mazda3. The contact stated that while driving at various speeds, or while attempting to reverse, the rear camera flashed on and off for approximately five minutes and then started working as intended. No warning light was illuminated. The vehicle was repaired under the NHTSA Campaign Number: 23V487000 (BACK OVER PREVENTION) by the owner of the vehicle; however, the fix failed to repair the vehicle. The contact stated that the failure had occurred after the recall repair was performed. The manufacturer was not notified of the failure. The failure mileage was 180,000.

While parked on a slight downhill grade, gasoline started leaking and created a significant puddle on the ground. No warning lamps, messages or other symptoms. An independent service center ran numerous tests over two days, including a smoke test. Diagnosis: There is a hole in top section of fuel tank and the fuel tank needs replaced. Recommendation was to not drive vehicle with fuel tank over half full. Due to the potential of leaking fuel in the presence of a hot ignition source increasing the risk of fire, driving the vehicle long distance with defective fuel tank appears to represent an unreasonable safety risk. Unfortunately, the vehicle is currently 300 miles from home. Mazda’s Mexican-made plastic fuel tank specifically required for our vehicle’s VIN is not currently available. (Mazda makes a similar metal fuel tank in [XXX] which will not fit.) Mazda has an estimated three-month backlog in [XXX] for the fuel tank. Still awaiting repair due to lack of availability of the appropriate fuel tank. Major safety recall NHTSA 16V684000 for the 2015 Mazda3 addresses a manufacturing defect where a fuel tank weld could fail, leading to fuel leaks and a potential fire. Recall is for vehicles produced between 12/4/13 – 9/18/15. Our vehicle was manufactured 02/2015 in [XXX] . However, recall is limited to three specific series of VINs and our vehicle falls in the gap between subject VIN ranges. A recent NHTSA complaint XXX (1/27/26) has the same issue: fuel tank leak, but not within range of recall VINs. It seems that the list of affected VINs included in recall 16V684000 should be expanded to include additional Mexican manufactured vehicles potentially with the same issue. Or create a new recall. Having a hole in the top of a plastic fuel tank securely mounted to the vehicle is unexpected through normal wear and tear. Any fuel leakage increases the risk of fire. Description On 12-15-2025, in 18°F weather, the vehicle was parked on a residential sloped driveway with the manual parking brake fully engaged. While the driver was outside the vehicle, the parking brake system failed to hold the car’s weight. The vehicle rolled backward, crashed through a wooden railing, teetering on a 4-foot retaining wall, and came to rest over a culvert. Two young children were inside the vehicle at the time of the failure. Component Failure & Availability: The manual parking brake system failed to provide sufficient holding force to secure the vehicle. The rear brake calipers are currently available for inspection. Safety Risk: The safety of the two children inside the vehicle was put at extreme risk due to the unexpected movement of the car. This failure matches the exact conditions and consequences described in Safety Recall 1217F (NHTSA Campaign 17V-393), which states that "unexpected movement of the vehicle if parked on a slope" increases the risk of a collision. Reproducibility & Confirmation: The failure occurred under the specific environmental triggers (cold temperature and slope) identified in the manufacturer's own safety recall documentation. However, the manufacturer has officially refused to authorize the internal teardown of the actuator shafts—the only diagnostic method capable of verifying the rust and sliding resistance that causes this loss of holding force. Inspections: The vehicle has been visually inspected by an authorized dealership and insurance representatives. The manufacturer has declined the necessary internal mechanical diagnostics to verify the internal defect. Warning Symptoms: There were no warning lamps, messages, or symptoms prior to the failure. The parking brake felt fully engaged at the time the vehicle was parked.

After coming home the following day I look closer at the documents and realized the CarFax did not match the VIN of the car I purchased. After looking up the CarFax with the VIN on the car I purchased I saw that the person who sold me the car had rolled back the odometer.

The Infotainment System will randomly change radio stations while driving, or start scanning through the various system menus, and on at least in one case, started randomly making phone calls from a connected phone. This creates a major distraction while driving. This has been looked at by the dealership and would cost $1500 for the system repair. I understand that this is a known problem.

1. Automatic transmission suffered a catastrophic failure during operation. Transmission was discarded by Bountiful Mazda, Utah. 2. Both other and I were at risk as the vehicle could not be controlled. 3. I read similar findings on Social Media posts. 4. Bountiful Mazda confirmed metal shavings in the drain trans fluid. 5. No prior warning on car instrumentation, or while driving, or on prior inspection by Cardinaleway Mazda, Gilbert, AZ 6. Neither Mazda USA (Mfr) nor Protective Asset Protection have taken responsibility for the initial equipment failure nor reimbursement through the extended service contract even with extenuating circumstances for late notification. Since the middle of 2023 I have noticed a gas fume smell both inside and outside of my 2015 Mazda Mazda3 after I fill my gas tank to full. This smell persists up until I use about a half tank of gas or slightly less. I do not top my gas tank off and stop filling when the automatic gas pump stops. There are no visible leaks on or around the gas tank or below the vehicle, and there are no noticeable cracks around the gas tank, valves, lines, etc. Additionally, there are no warning lights present on my dashboard or any codes.

The ITO (Indium Tin Oxide) film of the touch screen may be corroded due to moisture, which may adhere on the lens of the touch screen during the cleaning process at the plant. The corrosion may allow the electrostatic capacity to vary without any input, causing the malfunction of the touch screen.

However, the manufacturer continues to state that the remedy is not yet available.

The problem I have is with my rearview camera, as described in Manufacturer Recall Number 6023G / NHTSA Recall Number 23V-487. This problem has existed since I purchased the car in November 2022. The recall was announced on 7/14/23. Mazda has failed to provide a remedy. Dealers report that they don't have the parts needed to make the repair and that no remedy has been provided by Mazda. It is meaningless to have a recall if the manufacturer doesn't fix the problem. This is a serious safety issue as when the problem occurs, it is not possible to see behind the car via the camera/screen.

This forces my attention and hands away from the core task of driving while the car is in motion.

While I am listing the most recent occurrence in this form, this has been occurring on nearly every drive for the last 2 years. This typically happens in stop-and-go traffic. The vehicle has a manual transmission and the movement in the center console caused by shifting may be causing loose electrical connections to be disturbed and contribute to the behavior.

I was driving 90km and it suddenly stopped, I almost got into it, I put the 4 flashers and I let myself go on the shoulder, then call a towing after a machine test the error code P1092 appears , when I turn on my car, it's so miserable, it starts to bubble, it doesn't want to move forward anymore, lack of strength, All the rest is marked as PASS After googling around, I may have found an answer. Denso, who makes low pressure fuel pumps for a lot of different manufacturers, recalled several million of them a couple of years ago. They determined that there was a manufacturing defect that causes the pump impellers to crack and deform. Then they don't move enough gas to the front of the engine. I guess there is no pressure sensor between the tank and the high pressure fuel pump, so the problem isn't recognized by the car until the sensor in the fuel rail sees it. Several auto makers have issued recalls, including Mazda. But Mazda only went back as far as 2018. Toyota recalled cars as far back as 2013. There are now several lawsuits against various auto makers arguing that Denso knew about the problem in 2015, and the recalls should go further back in time. The infotainment center is "ghosting". This is a problem already recalled by Mazda but my car was not included. This started about 10 months ago. The result is that the gps system will shut down, the car, if linked to my phone will automatically dial random numbers, the radio stations will randomly change or be deleted. Having this random activity while driving or stopped creates a hazard as it requires frequent adjustments using my hands to correct the problems. The lack of a functioning gps is also problematic. This problem was brought to the attention of a Mazda dealer about 6 months ago and I was told no warranty repair was available. I also contacted Mazda USA by phone and was told no service would be provided. I have also sent a certified letter to Mazda USA with no response.

In February, 2022, my 2015 Mazda 3 experienced a major failure, while I was driving on a local road, going 35 miles per hour. All of a sudden, the engine lost power, three warning lights came on, and the car was struggling to go 20 miles per hour. Being near a friend's house, I drove there. From there, her husband followed me home. The next day, my car was towed to the Mazda dealership. The mechanic diagnosed the problem as “found TBS NUMBER: 01-014/16 and inspected coils found they were bad according to lot numbers”. He replaced all four engine coils and all four spark plugs. The service manager told me that the parts were not under warranty. My research revealed that engine coils are supposed to last the lifetime of the car, which is 150,000 to 200,000 miles for a Mazda 3. My car only had 43,048 miles. The spark plugs should have lasted about five more years. I looked into the Bulletin issued by Mazda USA. Mazda USA was first aware of a problem January, 2016. On 17 March 2016, I bought the car. Mazda USA updated the Bulletin to include my car on 29 July 2016. The car was under full warranty at this time and could have been fixed, if I had been notified of the defects. The faulty coils can cause a catastrophic failure, resulting in serious accidents and injuries. My car was a “ticking bomb”, waiting to go off. I was on a local road, with light traffic, not a secondary road or highway, with heavy traffic. Rather than notify customers of the danger of the engine malfunctioning, Mazda USA chose to remain silent. The Bulletin directs that nothing be done until a customer complains. The customer does not know to complain until the car malfunctions! The customer must complain within the normal warranty period. Mazda is hoping that the bad coils fail after the warranty period.

The multifunction / infotainment display in my Mazda went haywire last weekend--it loops through bootup sequence over and over but never loads. Backup camera still works, sort of, but the image jumps and it goes blank sometimes. I can't unplug the display or pull the fuse, or would lose backup camera altogether. So I have no Nav system, can't change radio station, etc.--and am stuck with the 7" screen flashing in my face as I drive. Local Mazda dealer advises that this is caused by cell phone transition to 5G. They took my contact info but did not offer any solution. There have been a couple of local news stories this week about other Mazda owners experiencing this issue. Interestingly, those all seem to report that owners' radios are stuck on a local NPR station. ON 8/20/16 THE DEALER TOLD ME THEY WERE UNABLE TO DIAGNOSE THE PROBLEM. IT SEEMED THAT MORE THAN 60 ERROR CODES HAD GONE ON A ONCE, BUT THEY COULD NOT FIND A CAUSE. I HAVE EMAILED AND WRITTEN TO MAZDA ASKING IF IT IS SAFE TO DRIVE THE CAR ON THE HIGHWAY, ASKING IF IT WERE TO HAPPEN AGAIN WOULD THE AND WHAT I SHOULD DO IF IT HAPPENS. WHILE ITS CONCEIVABLE THAT IN AN EMERGENCY THE DRIVER MAY WANT TO TURN OFF THE VEHICLE WHILE IN GEAR, THIS WOULD BE VERY RARE, AND COULD BE ACCOMMODATED BY PROGRAMING THE OFF SWITCH TO REQUIRE MULTIPLE PUSHES OR HOLDING IN THE BUTTON FOR AN EXTENDED PERIOD. EXITING THE CAR WHEN IT IS NOT IN PARK IS A DANGEROUS SITUATION, JUST AS DANGEROUS AS TRYING TO START THE CAR WHILE IN GEAR, RESULTING IN UNEXPECTED MOVEMENT OF THE CAR. SINCE THE CAR REQUIRES THE GEARSHIFT TO BE IN PARK AND THE BRAKE ON BEFORE STARTING, ONE WOULD THINK A SIMILAR PROCESS WOULD BE USED FOR TURNING OFF THE VEHICLE.

THE DAY WAS HOT AND I WAS IN STOP AND GO TRAFFIC WITH THE AC SET TO 72 DEGREES IN MY 2015 MAZDA 3 WITH JUST 21K MILES ON IT. MY CAR UNEXPECTEDLY STALLED AND THE CHECK ENGINE AND CHARGING SYSTEM/BATTERY LIGHTS CAME ON BRIEFLY IN THE DASH. I HAD TO SHIFT INTO PARK AND PUSH THE BUTTON TO RESTART MY VEHICLE. THE CAR'S ECU/PCM DID NOT SET ANY CODES AND THE PROBLEM HAS NOT OCCURRED AGAIN SINCE THAT HOT DAY. THIS IS A DANGER BECAUSE THE SUDDEN LOSS OF POWER COULD RESULT IN A REAR END COLLISION LUCKILY I WAS IN STOP AND GO TRAFFIC. DEALER AND MAZDA HAVE BEEN SLOW TO INVESTIGATE THIS THOROUGHLY. FEELS LIKE THEY JUST WANT TO SEND ME ON MY WAY WITHOUT DOING A THOROUGH INVESTIGATION OF THIS ISSUE.

OTHER TIME, WAS ON CALL, AND SYSTEM RESTARTED BUT CALL DID NOT DISCONNECT, USER ON OTHER END COULD NOT HEAR ME AND I COULD NOT HEAR THEM. WHEN SYSTEM RESTARTED, CALL CONTINUED. CALLED DEALER AND WAS INFORMED THAT THERE MAY BE A SYSTEM FIRMWARE UPDATE NEEDED. BROUGHT VEHICLE IN, WAS TOLD THAT NO FIRMWARE UPDATE WAS AVAILABLE, AND THAT THEY COULD NOT REPRODUCE ISSUE.

THE DRIVER HAD FALLEN ASLEEP AT THE WHEEL, SO DID NOT BRAKE PRIOR TO THE COLLISION. DURING THE COLLISION, THE AIR BAGS DID NOT DEPLOY, NOR DID THE SEAT BELT CATCH ME, AND I HIT THE STEERING WHEEL WITH FULL STRENGTH, CAUSING DAMAGE TO THE RIGHT SIDE OF MY FACE WHERE I HAD HIT WITH EYE DAMAGE, AND SOFT TISSUE DAMAGE ALL OVER THE FACE WITH NERVE NUMBNESS ON THE WHOLE LEFT SIDE.
