I was driving my 2024 Toyota Corolla SE gas model (with the convenience package) on the interstate in [XXX] during stop-and-go traffic. Traffic had been moving slowly for about 15 minutes, and I had Brake Hold turned on, cruise control was off, Proactive Driving Assist was on. I was behind a semi truck and was moving very slowly, getting ready to stop. I had maybe 3 or 4 car lengths between me and the semi so they could see me in their mirrors. As I was about to stop, the brake pedal suddenly went all the way to the floor, the car jerked to a stop, and the electronic/emergency parking brake applied on its own. I put the car into Park and then back into Drive, and the problem did not happen again during that drive. There were no warning lights, dash lights, or warning messages before this happened, and there were also no warning lights or dash lights afterward. The issue happened suddenly with no prior symptoms that I noticed. I believe the brake system, Brake Hold system, electronic parking brake system, or a related brake/vehicle control system malfunctioned. This put my safety and the safety of others at risk because the car unexpectedly stopped abruptly in interstate traffic. Since I was in heavy stop-and-go traffic and other vehicles, there was a risk of being rear-ended, causing a crash, or becoming disabled in traffic. I took the vehicle to a Toyota dealership after the incident. They were not able to reproduce the problem, but they did find stored code C14FE14. They told me they checked the brake system, cleared the code, tested the steering angle sensor connector, and said everything tested within specification. They also performed a multipoint inspection and said the battery, brake pads, and other inspected items tested well. Had 6+ alignments with no improvement. Recent alignment caused binding to flip sides, proving internal mechanical binding not alignment issue. RELATED: Toyota issued CSP 22TE02/TSB T-SB-0028-22 (2022) for 2018 Camry with identical "reduced return to center" caused by intermediate shaft binding. My 2024 Corolla has same defect Toyota acknowledged in 2018 models but refuses to repair. INDUSTRY PATTERN: Honda HR-V TSB 24-094 (2024), Mazda CX-90 Recall 24V-022 (26+ failed fix complaints, 2 crashes), GM Bolt TSB 17-NA-148, Ford Explorer ST owner reports - all for steering not returning to center. REQUEST: Investigate if Toyota intermediate shaft defect extends to 2024 Corolla and whether industry-wide EPS binding represents systemic safety issue. Toyota confirmed defect in writing but denies repair.

The vehicle in question is currently exhibiting transmission anomalies consistent with those observed in several other vehicles under review, specifically: slipping, jerking, surging forward, hesitation to accelerate from a complete stop, loss of power, droning sound at low speed, odd knocking noise/vibration during idle, unresponsive gear changes, shuddering upon vehicle shutdown, and decreased fuel efficiency. The owner has incurred expenses exceeding $2,000 for rental vehicles and diagnostic services in an effort to resolve these persistent issues. However, as of January 21, 2026, Toyota has not acknowledged the existence of a problem, asserting that these conditions are within normal operating parameters and that the vehicle is performing as intended. A local mechanic has advised the owner that Toyota will not diagnose the issue, but they are likely to offer a post-warranty adjustment should the transmission ultimately fail. It has become increasingly apparent that Toyota is not conducting a thorough investigation into these matters and expects the customer to bear the expense beyond the scope of the standard warranty. Beyond the immediate safety concerns for the owner, there is a significant risk of collision should the vehicle experience a loss of control or erratic behavior. The contact stated that because of the failure, the contact's vehicle rear-ended the Honda CR-V vehicle that was parked in front of the contact's vehicle. The contact stated that the vehicle's speed was only 3–4 MPH. No air bags deployed. The other vehicle sustained damage to the rear bumper. The contact's vehicle was towed to a local tow yard. The contact's vehicle was condemned as a total loss by the insurance adjuster. No police report was filed. No injuries were sustained. The manufacturer was made aware of the failure. I am reporting an ongoing safety issue involving the Skid Control ECU and loss of power break recall on my 2024 Toyota corolla hybrid. The first time, my vehicle experienced loss of power brake assist and displayed a “Hybrid System Malfunction” message on screen. I towed my car to Toyota, confirmed it was related to the Skid Control ECU recall and repaired it under the recall at no cost, they did a software update. The second time, the exact same problem reoccurred. I had the vehicle towed again to a Toyota dealership, but this time, they refused to perform diagnostics unless I paid out of pocket for a 12-volt battery replacement. The dealership stated they cannot confirm if the issue is recall-related until diagnostics are complete, but they also refuse to complete diagnostics without me paying first. My vehicle is currently undrivable and unsafe, and I rely on it for daily transportation and work. So they made my pay for this 12v battery to do the diagnostics and now they tell me it is not recall related but one of the relays not working, the repair will be cover under warrant but i had to pay for battery and rental cars . I believe Toyota is in violation of its obligations under 49 U.S.C. §30120, which requires manufacturers to correct recall-related safety defects free of charge. This issue leaves me without a vehicle and unable to verify that the recall has been properly resolved. I have opened a case with toyota brand engagement. I am requesting that NHTSA review this case and ensure that Toyota complies with recall repair requirements so my vehicle can be safely diagnosed and repaired without charge.

During a routine transmission fluid service on a 2024 Toyota Corolla Hybrid, I discovered that the transmission model (PA10) is completely unlabeled and cannot be identified anywhere under the hood, in the owner’s manual, or on any visible tag. The only label is buried behind the firewall and not reasonably accessible to consumers or technicians. As a result, I initially attempted to service the wrong fluid port because Toyota provides no visible or clear identification of the transmission model or fill/drain points. Additionally, the factory-installed transmission drain and fill bolts were tightened with extreme torque well beyond normal service specifications, requiring application of high heat and excessive force to loosen. This resulted in personal injury and risk of property damage. The combination of (1) inaccessible transmission identification, (2) inadequate service guidance, and (3) extreme bolt torque creates a foreseeable safety hazard to vehicle owners or technicians attempting basic maintenance. Toyota’s official service documentation (Toyota Technical Information System, “TIS”) that clarifies the correct fill ports is paywalled and not included with the vehicle or manual, making it impossible for consumers to safely perform essential maintenance without paying daily access fees. This lack of accessible labeling and over-torqued components presents an avoidable risk of physical injury, vehicle damage, and mis-servicing. I believe Toyota should be required to provide visible transmission identification and safe torque specifications for consumer maintenance.

Component/system failed: The SRS Airbag System malfunctioned. After a rear-end collision on October 3, 2025, in Gwinnett County, GA, the airbags did not deploy. The vehicle displayed an alert: “SRS Airbag System – Malfunction detected” at 16:02. Safety risk: My safety was put at risk because the airbags did not function during a significant impact. This failure could have resulted in severe injury if the collision had been more forceful or from another angle. Inspection/reproduction: On October 6, 2025, I brought the vehicle to Atlanta Toyota. Instead of documenting the diagnostic trouble code (DTC) and providing a report, the dealer cleared the fault code and recalibrated the system without my authorization, effectively erasing critical evidence of the malfunction. The invoice confirms this. Inspections by others: The vehicle was inspected only by the Toyota dealer. It has not yet been inspected by the manufacturer, police, or insurance representatives, but I have legal counsel and am preserving the evidence for further independent inspection. Warning lamps/messages: Yes. The SRS Airbag System Malfunction message appeared immediately after the collision on October 3, 2025. I have photographic evidence of this alert.

My vehicle will jerk into the left lane of the road when lane keeping assistance is activated. It appears isolated only to situations when I am travelling on roads that were recently repaved and lines haven't been painted yet. However, these roads are equipped with consistent reflective indicators marking the center of the road because these roads remain unpainted for months. There are many of these roads where I live so it is a frequent issue that I experience multiple times everyday. I have never allowed the vehicle to fully execute it's correction in fear it would drive off of the road or into oncoming traffic but it is a very scary event when my car tried to kill me.

My car was parked in my driveway on am 80 degree afternoon. I went into the car to get something. When I closed the driver door, the entire back window shattered. I am the only driver of the vehicle and there were no issues with the window prior to this- no cracks, no incidents, no rocks on the road.. The only indication there was any issue was condensation on the inside of the back window two days before this happened. I thought this was unusual, but it did not seem like something to call the dealer about. I took the car to IRA Toyota in Saco, Maine. They informed me there was no way to 'prove' a defect, as there was no longer a window to inspect. They suggested I call my insurance and lie about the incident, stating that 'nobody would believe a window just blew out' and they were just trying to help me get the repair covered by my insurance. I know there is currently a class-action suit against Honda for this same issue, and found over 20 shared instances in Toyotas (of various models) from 2021,2022, 2023, and 2024. I understand glass is not included in warranties, and I think this only makes sense for manufacturers. The fact that a window could spontaneously shatter at any moment is certainly a safety issue and should be covered under warranties or some other level of accountability. When my window combusted, all of the glass fell into the back window and back seat. If any kids or animals had been in the back seat (as they almost always are), they would have been covered in glass. Thank you for your attention to this matter.

My car has 5358 miles. Started car. DRIVE START CONTROL MALFUNCTION LIGHT; CHECK ENGINE LIGHT; SEE DEALER LIGHT all came on. Car would hardly move forward or reverse. Felt like the brakes were on. Heard subtle clicking sounds from engine. Looked this up as I never heard of this. Toyota says you MUST STOP DRIVING ASAP, assuming you "can" drive. You can LOOSE CONTROL of the car. There are many things that can cause the light to come on. Dealer "says" they never heard of this. My car had to be towed. Mechanic found nothing wrong and reset computer modules. I got my car back after 3 days. Dealer says NO GUARANTEE it won't happen again WITHOUT warning. So; anywhere, anytime, my car will just stop driving? HOW IS THIS SAFE? Internet searches show it's NOT UNCOMMON. Toyota makes it hard to file a complaint.

There was a recall on my vehicle for the skid and emergency braking and was told it was taken care of Feb 2025 at my service appointment. I was involved in a car accident on Aug 11, 2025, everything that was supposed to be fixed in the recall did not operate during my crash. I reconfirmed with the dealership that they did the recall repair and they again said yes but if they did then the repair completely failed. There was no emergency braking available, the car skid a long distance, no airbags deployed even though all sensors in the front were engaged and collision center confirmed they should have gone off. Vehicle is currently at collision center and is available to be inspected.

When exiting the freeway 3 miles later, the vehicle suddenly swerved uncontrollably without any steering input from me. This was an extremely dangerous incident that could have caused a serious accident injuring myself and others.

This is my fourth service visit for the same unresolved safety issue on my 2024 Toyota Corolla Hybrid. The problem involves a cracked fuel line and fuel vapor leak in the same area as recall 24TC08 (Fuel Tank Filler Clamp and Vapor Leak Detection). The dealership initially failed to detect this during recall work and later claimed “rodent damage” after two prior visits. I have since used a professional-grade MultiRAE Lite gas detector and documented VOC (volatile organic compounds) levels up to 123 ppm inside the vehicle cabin with the windows up. These readings indicate hazardous fuel vapor and exhaust gas intrusion, which pose serious health and fire risks. I have driven over 200 miles in these conditions), exposing myself to harmful fumes. Manual control over the steering wheel was not possible. Luckily with this occurring both times I was going a slow speed. The Lane Tracing Assist (LTA) system functions smoothly, keeping the car centered. However, immediately after disengaging LTA, the manual steering defect reappears, indicating an underlying mechanical problem, not an electronic one. Safety Concern: This defect creates a serious safety hazard. It demands constant driver attention and effort, leading to distraction and fatigue. My vehicle has remained undrivable since. Toyota conducted a diagnostic inspection on May 29 under Case #[XXX]. I’ve repeatedly requested access to the raw data from this inspection to understand the defect, but Toyota has not responded. The noise is related to an inadequate spring inside of the intake camshaft gear which will not push a small cylinder in place to lock the intake camshaft gear in place. If the gear rattles against its housing too hard overtime, it can break and cause catastrophic engine failure by sending metal pieces into the engine, including pistons and rods.

We have visited the dealership and were instructed how to maximize the brightness, but it is still not sufficient. The small icons that appear at the top of the center section of the instrument panel cannot be viewed while we are driving, unless we place our head close to the screen. To do this our head must be at the level of the bottom of the front window. In that position we cannot see the highway sufficiently to be “safe.” We both have good vision. In the past, we have owned over thirty automobiles and have never had a problem seeing the instrument panel.I have discovered that occassionally the illumination of the center screen of my instrument panel is fine. The car’s computer apparently dim’s it without my consent. I drove 60 miles (about an hour) on a California freeway, and for the first 20 minutes I had good illumination. Then it suddenly got to its usual dim state. I have asked several people at my Toyora dealership, and they cannot help me. I, therefore, am requesting someone at your offices to look into this problem, and call me by phone to help me solve this important problem. This is an issue I have noticed with my car shortly after I bought it. The problem is that the car has a noticable delay in acceleration when accelerating from a complete stop. After doing some research online, I found that the cause was the car's CVT transmission, and that a delay in acceleration happens with other cars with CVT transmissions as well. This could be potentially dangerous especially when making right turns on a red light or when making unprotected lefts on a green, as the window of opportunity to go is sometimes short in those situations, and delaying for even a second could mean the difference between a collision or a safe turn (especially since there are some fast drivers that go much past the speed limit and will appear from behind a hill all of a sudden). What is especially problematic is that the car will rarely accelerate normally when I press the pedal, but I would say around 80-90% of the time, the car will have a delay about 1 second before it actually starts moving. To my knowledge, this problem has not been reproduced or confirmed by a dealer or independent service center. I bought the car brand-new about half a year ago so it shouldn't have any problems. The vehicle has not been inspected by anyone, and there were no warning lights or any other symptoms of the problem prior to the failure. I didn't really notice that this car had this problem when I first drove it home from the dealership because I was not used to so many other aspects of the car, but after I got more used to driving it, I realized that the delay in acceleration wasn't normal.

Ever since my vehicle was new, the driver side blind spot monitor will periodically not work. This issue occurs primarily when starting the vehicle using the Toyota app, but also will have the issue when starting the vehicle normally. When all conditions are met, the driver side blind spot monitor will fail to illuminate during an entire trip unless the driver either: turns the vehicle off and back on, or: goes into the vehicle settings while driving and turns the blind spot monitor off and then on again. Upon doing either of these things, the driver side blind spot will resume normal operation. The issue has been confirmed by a Toyota dealership and is on file for my vehicle, and I also have video footage of the issue. While the dealership was able to witness the issue, the vehicle produces no codes and so the Toyota engineer advised it was a condition issue and not a vehicle issue. But this issue has been happening to me randomly since I got the car, and all of the facts point to this NOT being a condition issue. There are also SEVERAL other complaints on Toyota and Lexus forums for this exact same issue for varying models such as the Corolla Cross. This is a safety risk and can increase the possibility of a crash. While blind spot monitoring is intended to be an aid and should not be solely relied upon, inevitably there are drivers who will rely on that blind spot monitoring, not know it's broken, and potentially get into a collision due to the system leading them to believe the coast is clear, when it indeed is not. The first time I noticed it, which was within one week of owning the car back in January 2024, I was trying to get over to the left, saw that the blind spot was not illuminated, and almost made the turn except I checked manually first and saw there was a vehicle in my blind spot that the car did not pick up on. The fact that all it takes it turning the car off and on again leads me to believe this is a safety defect that needs addressed.

The system that is problematic for this vehicle is the Pre Collision System (PCS). This system is way too sensitive even on the lowest sensitivity setting, and I have experienced the car braking in error on multiple occasions when driving up or down a windy road. When I approach a bend in the road and there is a car parked on the side of the street, my Corolla will think that I am about to collide with the parked vehicle and make a loud beeping noise and automatically hit the brakes. This has frightened me on a number of occasions, and I am concerned that one day the car will brake in error while there is another vehicle behind me, resulting in a read-end collision. The only consistent method of avoiding this is to drive past the median in the road when approaching bends, which I very much dislike doing, as driving in the middle of the road has its own obvious risks. I have tried contacting the dealership about this, and was told to contact the service department. The service department was no help, saying that they aren't able to do anything about it and that I could try lowering the sensitivity of the PCS, which I had already done. This wouldn't be such a huge issue if there was a way to permanently turn it off. The process for turning it off takes about a minute, as it requires navigating a menu and finding the PCS option to turn off. Unfortunately, the PCS automatically reactivates every time the car is turned off and on again, and even if I use the shortcut method of holding the lane assist button for several seconds to disable the PCS, the car's screen will constantly flash, telling me that the PCS is turned off and annoying the driver to turn it back on. I know that Toyota does not have this issue with their PCS systems on their other vehicles, as I have driven a 2019 Camry before and the PCS system never went off when I drove on the same road, which led me to believe that the system on this 2024 Corolla is somehow defective.

This is very hazardous and only remedy is to switch to high beams. This issue causes me to either not be able to see the roadway adequately or blind oncoming traffic with high beams. This is an issue on my 2023 as well. Toyota has stated it is normal and as designed. I disagree. Not being able to adequately see the road with low beams is not normal. The problem is better or worse depending on orientation of the road. Shadows on Curves are much worse.
