My 2025 Toyota Tacoma engaged the emergency braking/pre-collision system system while my vehicle was going under 10 mph on a steep road in winter conditions. My vehicle was completely under control before the system engaged and there was no risk of a collision with any vehicles in front of me. The emergency braking system engaged and rendered my vehicle completely out of control. I tried desperately to pump the brakes in hopes I could regain control of the vehicle, but the brakes were fully locked. Had the brakes dis-engaged, I could have easily avoided the collision. The car not only did not decelerate, it accelerated down the hill while the brakes were locked for several seconds. My vehicle eventually slid into another vehicle that had been parked on the side of the road. From doing extensive research I believe a perfect storm of events caused this malfunction. Previously on this day, several inches of snow had fallen. The road had not been plowed and cars were parked on the side of the road. There was also a curve in the road near my vehicle's collision where the other vehicle was parked. My windshield was also still defrosting in the uppermost section near the rear view mirror. It is also possible the radar block on the front of the vehicle had snow or ice on it. It was also actively snowing. From what I have read, any of the above factors can cause a malfunction in the emergency braking pre-collision system. I had never been in an accident in over 20 years of driving and my vehicle did the least safe maneuver it could have in the given situation. There is no circumstance where one would purposefully slam the brakes on a snowy/icy down hill road if they know how to drive in the snow (even if you were going under 10mph as I was). At the most basic level of failure, I can't imagine this system should ever engage if going as slow as I was. Also, the system is designed to decelerate (was accelerating) and I was pumping brakes in effort to disengage

I either have to roll down the window and open the door from the outside or I have to let passengers out from exiting from the front seat. I took it in for inspection by the dealer, West Kendall Toyota on March 17th, 2026. They had to order a new part. They tried repairing it on March 31, 2026 but failed to do so and had to reorder the part. It is still broken as of today April 1st, 2026. My front seat driver’s side door handle also broke in August of 2025 and I could only get out through opening the window and unlatching the door from the outside door handle. This is a serious safety issue as passengers could become trapped should the car be hit on the other side and if even more so if the power goes out on the car. I feel Toyota is not doing enough to fix the issue and is not making a quick effort to repair this and is showing a lack of concern for consumer safety.

Symptoms observed: Doors locking autonomously on exit, sometimes before occupants could exit the vehicle System falsely detecting the key fob as being inside the vehicle when it was in the owner's pocket outside, causing the truck to refuse to lock Touch-lock refusal at the door handle Random unprompted beeps while parked and locked with no corresponding lock movement Key card unable to lock the vehicle Dealership witness: During a January 2026 service visit, Delaney Toyota Service Advisor Ricky Carlson was personally locked inside the vehicle by this malfunction before he could exit. This was witnessed firsthand by a trained Toyota service employee and is part of the service history, though it was not captured in the official repair order. MY 2025 TACOMA is part of a recall by Toyota, I was notified of the potential issue on early November 2025 and told Toyota was working on a remedy. On December 3rd, 2025 while using 4-wheel drive during a snow storm the drive shaft (recall issue) broke. On December 4th, 2025, the local Toyota dealer checked my Tacoma and found it unsafe to drive - they kept my Tacoma and provided a loaner vehicle. Toyota reported at this time the remedy would be available by the end of 4th quarter (December). As of today, February 21, 2026 -- there is still no remedy for the issue, my Tacoma is still sitting at the dealer, and I do not have my truck. Toyota reports a remedy will be available during the 1st quarter of the year but nothing yet. I called brand engagement recently and asked them to extend my warranty, Toyota care, and other free trials that were included with my purchase. However, Toyota refuses to do anything further - as they said a loaner car is enough - to make the issue more acceptable even though my new vehicle is losing this coverage.

Driving up the 15 freeway from Diamond bar CA to Victorville CA the car gave me a warning that the transmission is overheating and to pull over to cool it down keep in mind I was on a paved freeway with no trailer and nothing in the bed of the truck so no unusual weight so with the car being brand new (only 1500 miles) I played it safe and pulled to the side of the freeway for about 15 minutes to allow for cooling then the sign went away and it was driving normal. A few days later I took the car into the shop for transmission, hard shift issue and I had them look at the overheating as well and they found an overheating was caused from “harsh acceleration” i’ve had many cars and no car has ever had a transmission overheating light come on unless if I was on the track or off roading I went up to Victorville again the following weekend and the overheating light came on again but the dealership is saying it’s normal but going up a paid freeway and having to stop on the side of the road to let the transmission cool down is not safe

The contact stated that while driving approximately 75 MPH, a Semi Truck attempting to merge onto the highway crashed into the passenger side of the vehicle, causing the driver to lose control of the steering and cross three lanes of traffic before crashing into the concrete median. During the crash, no air bags in the vehicle deployed, the collision avoidance systems did not operate, and twelve error messages were activated, including a brake error, a Four Wheel Drive error, and an ABS error. The driver sustained a concussion from the head contacting the steering wheel and lacerations from shards of glass entering the vehicle during the crash. The front seat passenger sustained lacerations to the legs and arms, the rear driver side passenger sustained fractured ribs and lacerations to the arms leg, and face. The rear passenger side occupant sustained lacerations to the arms and legs. All occupants in the vehicle were transported to the hospital with injuries, along with the semi-truck driver. No further information was available. A Police report was taken, and the vehicle was towed away. The manufacturer and local dealer were notified of the incident. A representative from the manufacturer was sent to inspect the vehicle and deemed no failure had occurred. The vehicle was destroyed. The failure mileage was 4,000.

Supplemental Safety Description I am submitting this supplemental statement to provide a complete and consolidated description of the ongoing safety concern affecting my vehicle. Vehicle Behavior and Safety Symptoms During normal driving conditions, the vehicle consistently exhibits directional instability, including pulling to one side and difficulty maintaining straight-line tracking. Steering control feels unstable, and the vehicle experiences abnormal harshness and vertical movement over the road surface. The vehicle often feels difficult to control and demonstrates behavior consistent with lateral misalignment, including a crab-walking sensation. During rapid left and right steering inputs, the vehicle exhibits abnormal and asymmetric body movement. One side of the vehicle body rises excessively while the opposite side compresses, resulting in excessive body roll and uneven suspension response. This abnormal dynamic behavior creates a strong perception of instability and increases the risk of loss of control or rollover during steering maneuvers. Additionally, overall traction appears reduced. Under wet or slippery road conditions, the wheels are more prone to slipping, further compromising vehicle stability and steering control. Repair History and Dealer Involvement The vehicle has been presented to four different Toyota authorized dealerships for the same steering and suspension-related concerns. Multiple wheel alignments, steering calibrations, inspections, and road tests were performed. Despite these repeated repair attempts, the condition persists. Each dealership reported that no abnormal condition was found and stated that the vehicle was operating normally. In at least one instance, a dealership declined to continue further repair attempts while the safety-related condition remained unresolved. I was driving to work when after running across a minor pothole I heard a clunking sound. It turns out it's the top cap for the front coil-over Bilstein shocks had broken allowing the shaft and the bump stop to protrude through the upper mounting plate. The coil did its job of keeping the vehicle suspended, but the shock piston has no ability to alleviate any stresses put upon the coil. Dealer states that because there was an outside contribution, i.e. a pothole, which had been driven around and over many times before, wasn't warrantable. There was no more than standard bump and sway than when driving around as usual. There was no shudder as though I was going too fast for the defect in the road. I dont know if there was pre existing stress damage from a recent collision(opposite corner of truck) or if the was a defect with the part. The top cap is made out of aluminum rather than steel, which I find disturbing after learning this is not the first to have broken. This vehicle has only been off pavement in gravel driveways and parking lots. I hope you can investigate this thoroughly.

The vehicle was being operated in 4-wheel-drive, mode 4H, and when turning to the left at low speed, there was a grinding noise from the front of the vehicle and it would not proceed. The vehicle had to be put back in 2-wheel-drive, mode 2H, put in reverse, backed up a short distance, and then put back in drive, and then it was able to proceed. This is likely the CV joint issue reported in other 2024 and 2025 Toyota Tacomas. The dealer has been notified, and the vehicle will be taken in for service.

While driving on a dry, clearly marked two-lane highway in Montana, I had Lane Tracing Assist and Active Lane Keep/Track Assist turned on. My hands were on the steering wheel, speed was within the limit, and visibility was clear. The truck unexpectedly steered itself out of the lane toward the right-side guard rail. There were no audible or visual lane-departure alerts, no “steering assist canceled” message, and the Pre-Collision System did not apply braking before impact. The vehicle struck the guard rail. The system failed on multiple safety layers — no lane warning, no steering correction, and no automatic braking — under normal conditions (dry pavement, well-painted lane lines, daylight). I believe this represents a malfunction or defect in the driver-assist software or sensor calibration for Toyota Safety Sense 3.0. I request NHTSA investigate similar incidents for 2024–2025 Toyota Tacoma models equipped with LTA/LDA and Pre-Collision System to determine whether a systemic defect exists.

Many other 4th-generation Tacoma owners (2024–2025) are reporting the same issue online — CarPlay will drop connection, the system freezes, or CarPlay will not appear as an available input for long periods. Because CarPlay is a key part of the hands-free system for navigation, calls, and messaging, these disconnections cause drivers to interact manually with their phones, which creates a potential safety hazard. Toyota dealerships acknowledge the problem but currently have no permanent fix. It appears to be a system-wide defect in the Audio Multimedia 6.0 system used in the new Tacomas. I’m submitting this report so NHTSA can investigate and, if appropriate, prompt Toyota to issue a software or hardware recall to address the defect.

This has impacted my safety and the safety of others by not properly shifting into gear. When accelerating it engages harshly from 2nd to 3rd gear. I can’t properly accelerate whenever the vehicle is idle; thus causing safety concerns when operating this vehicle. •Has the problem been reproduced or confirmed by a dealer or independent service center? Yes, on [XXX], I took the vehicle into to Carver Toyota Service Center with only 10,639. My warranty covers 100,000 miles bumper to bumper. •Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The Service Member with Carver Toyota “test drove” and confirmed that the vehicle shifted harshly from park to reverse and from 2nd to 3rd gear. The vehicle fluids were checked and were found to be low. The fluids were “topped off” and the vehicle was returned back to me within one day of my delivering it to them. •Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? At the time of my reporting this issue with Carver, there were no warnings or error codes indicted by the vehicle. However, this is a well known issue with this make and model of the Toyota Tacoma. While driving, the sunroof on my vehicle shattered spontaneously without any external impact. I have video evidence clearly showing the glass cracking, with no rock strikes or other outside causes. This failure I feel is a defect in materials, not outside damage, and therefore falls squarely under Toyota’s New Vehicle Limited Warranty. The warranty specifically excludes glass damage only when caused by external objects; it does not exclude spontaneous failures like this one. For my safety and the integrity of the vehicle, I am requesting that this repair be processed under warranty without delay. I’m risking water to enter the vehicle which will cause more damage. Please document and submit this claim to Toyota Corporate. This has been mentioned numerous times by customers. My truck is rated to tow 6,400 with no way to add a trailer brake controller. I don’t see how Toyota can legally sell this truck without at least giving an option to install an aftermarket controller, even though Toyota doesn’t recommend it.

I purchased a brand-new 2025 Toyota Tacoma TRD Off-Road. From the first day, the steering wheel was off-center and the vehicle vibrated while driving. At around 500 miles, I noticed severe instability and abnormal tire wear. A wheel alignment report showed: Thrust angle: +0.15° (spec: 0° ± 0.05°) Rear toe: Left 0.23°, Right -0.10° (difference: 0.33°, spec ≤ 0.15°) Front caster: Both sides out of specification (shown in red on report) Driving symptoms include: Cannot maintain straight-line driving; requires constant steering correction. Excessive body roll during lane changes or turns. Continuous vibration of the vehicle and steering wheel on smooth roads. Vehicle bounces and momentarily loses control on uneven surfaces. Steering wheel rotation does not match front wheel steering angle. When stationary, turning the steering to full lock causes the body to rise; releasing it causes it to drop. Attempts to repair at Toyota of Manhattan (June 2025 – August 2025) were unsuccessful. The dealership repeatedly refused to perform proper inspections, provide reports, or acknowledge the defect, despite multiple appointments and a case opened with Toyota corporate. The defect presents a serious safety risk: reduced vehicle stability, increased stopping distance, loss of steering control during maneuvers, excessive and uneven tire wear, and abnormal fuel consumption (13.5 MPG with no load). The problem exists from new and has not been corrected.

On several occasions in the previous two months, I have been driving during heavy rain. On the first occasion, while high speed wipers were on, they suddenly stopped working. I slowed down, but was able to get them going again prior to pulling to the shoulder of the highway. I had to switch to the slow wiper speed to get them going. I switched back to the high speed wipers due to heavy rain, and kept stopping each time. It continued this cycle and stayed on after about 15 attempts. Today, on [XXX], I drove down [XXX] from Flint, MI towards Detroit at approximately [XXX]. The wipers again quit working and I had to switch the wiper speed to get it working again while at full highway speed during a deluge of rain and nearly had to stop in a construction zone with no shoulder and heavy traffic, nearly causing a collision until I was able to get the wipers to begin working again. On a previous occasion, I turned them on. When the rain stopped, I was unable to stop the wipers, but was eventually able to stop them by carefully moving the intermittent delay wheel to a position between on and off. It appeared that there may be a short in the wiper circuit. This was a definite safety issue in that my attention was taken away from driving to try to fix the wiper situation. In speaking with my service adviser at Randy Wise Toyota in Grand Blanc, MI, he went out to their lot and tried to duplicate the problem, which he confirmed happened on that same Toyota Tacoma model. I attempted to get this repaired under warranty. Their response was that there was nothing they could do since it appeared to be in the design of the wiper system. The XM radio fades out when ever you go under a tree and on its own when going down the road. It says that it is trying to locate a satellite! The dealership has replaced the radio and the antenna and checked the antenna cable, with no improvement! They said that they can’t do anything else about the radio problem. Toyota said to file a complaint with NHTSA. Toyota's case number is 250707000269. The truck is 3 weeks old! With 453 miles on it.

Hood prop up rod comes loose in windy conditions. The was captured on my CCTV cameras and is also duplicatable. Simply shake the hood when it’s open and the rod comes loose Toyota was notified and instructed me to show the dealer. Dealer was notified and we duplicated the problem in their presence. They informed me that there was nothing they can do and they noted in in the maintenance report. This complaint is about the rear passenger side window on the vehicle. It is available for inspection upon request. This was a huge safety risk for me today. I had the rear passenger side door open (with the window rolled all the way up) gathering my things from the back seat. I closed the door with a normal amount of force and the window completely shattered causing me to be covered in glass shards. Luckily, no one was sitting in the back seat and at least I didn't get glass in my eyes. However, this is quite concerning. I don't believe the glass was under any stress or pressure. The temperature was 78 degrees and cloudy outside today, which doesn't seem extreme to me. The window was in perfect condition as I just bought the truck 2 months ago. After the window broke, I went directly to the dealership I bought it from, where they inspected it, and taped a trash bag over it. They told me there was a recall on windows for the new Toyota Tundra models, however, not yet the Tacoma. I believe this is a potential danger to many people and it should investigated.

Vehicle was set in tow/haul mode. Tow/haul mode provides higher acceleration by changing the operation of transmission. Vehicle was towing 2500lb cargo trailer on interstate. Driver stopped at rest area and turned off engine and left vehicle. Driver returned to vehicle 5 minutes later. Driver started engine and proceeded to leave the rest area. Driver was on highway entrance ramp. Entrance ramp merges with right lanr Driver attempted to accelerate to enter highway in such a way that there would be safe distance between end of trailer and car on highway approaching end of trailer. Vehicle would not accelerate as quickly as it was accelerating for the past 2 hours. Driver was surprised and distracted. Margin of safety between rear end of trailer and car on highway was greatly reduced. Some time after the incident Driver realized that car forgot it was in tow haul mode whenever engine is turned off. There is no warning that mode changed. This means that car does not behave as reasonably expected after an ignition cycle
