Vehicle Information Vehicle: 2024 Toyota Tacoma VIN: [XXX] ate: January 26, 2025 Repair Completed: October 1, 2025 Safety Complaint Description Beginning on or about February 5, 2025, my 2024 Toyota Tacoma developed a serious braking-related safety issue that substantially affected the safe operation of the vehicle. When I brought the vehicle to the dealership, I reported braking concerns and was advised by dealership personnel that the vehicle may not be safe to drive due to concerns that the rear brakes might not function properly under certain conditions. I was specifically told by dealership staff that they would not recommend driving the vehicle due to safety risks. Despite this warning, I was forced to continue operating the vehicle because no repair remedy was available at that time and no equivalent loaner vehicle or safe alternative transportation was provided. Over the course of several months, I repeatedly contacted the dealership regarding the issue. Each time, I was told that no repair remedy was available and that nothing could be done until Toyota released a repair solution. I was also advised that my only option was to leave the vehicle at the dealership indefinitely, which was not practical because the vehicle was my only means of transportation. As a result, I was forced to continue driving a vehicle that had been identified as unsafe. The defect remained unresolved for approximately eight months. During this time, I experienced significant safety concerns and loss of reliable vehicle use. Because of the prolonged delay in repair and lack of alternative transportation, I was ultimately forced to purchase another vehicle in order to maintain necessary transportation. The braking-related condition was not permanently corrected until approximately October 1, 2025. This situation created significant safety concerns because I was forced to operate a vehicle that dealership personnel advised may not be safe to drive, yet INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6) INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

After having Toyota Tacoma rear brake hose recall performed at Steven Toyota Harrisonburg Va I have very poor braking! This is a huge safety concern for my family and myself. Toyota is not taking this matter seriously. I was told “bring it in and let the tech have another wack at it”. No get a master tech on this problem immediately! I had reservations about having this recall performed by Toyota because in Toyotas pursuit to fix a problem they always make it worse. This has been our family’s experience over the course of many years. I have reported this issues to Toyota corporate and they are oblivious to our families safety concerns. They are more concerned about their protocol.

What component failed?
The rear brake system (rotors and pads) and the components associated with Safety Recall 25TA04 (rear brake hoses). The parts are available for inspection. * How was safety put at risk?
The dealer (Island Toyota, St. Croix) labeled my 2024 Tacoma "UNSAFE TO DRIVE" on Invoice #[XXX] but refused to perform the Federal Safety Recall (25TA04) for rear brake hoses unless I paid for an $800 brake job first. This violates federal law by making a safety recall conditional on customer-paid maintenance. I am currently driving a vehicle the dealer documented as "unsafe" because they withheld the free safety remedy. * Has the problem been confirmed?
Yes, the dealer confirmed the rear brakes are failed but refused the recall and the 36,000-mile warranty claim. * Has it been inspected by others?
Not yet. However, the dealer’s technician failed to record actual millimeter measurements for the pads or rotors on the official invoice. * Warning lamps or symptoms?
Metal on metal grinding noise from the rear brakes. No warning lamps appeared prior to the failure. * Additional Note on Records:
The dealer falsified my service records on Line C (Recall 25TC07), claiming I was "unwilling to wait" for a repair that was never even mentioned to me. The dealer also claims I denied service on Line B, but what actually happened is I refused to pay $800 for a premature brake failure that should be covered by my 36,000 mile warranty or as consequential damage from the recall. The dealer then used my refusal to pay for pads as an excuse to withhold the Federal Safety Recall (25TA04). I never denied the safety repair I denied a bill for a factory defect. I have uploaded photos of both rear wheels. The left side is healthy, but the right side is completely destroyed with deep scoring. This uneven wear at only 34,613 miles proves this is a manufacturing defect, not my driving style. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

It happened on February 17, 2026 around noon. My truck is park on the drive way, I went outside to checked the condition of the weather and I notice the driver side window glass looks like a crumpled Reynolds Aluminum wrap. I called Toyota Service Dept since the truck won't start. The battery was replaced and so is the window glass but, they told me it's not covered by the warranty. I paid the new window glass for 619.99. I called Geico my insurance and the deductible was 1,000.00. I told Geico I will not claim for the damages for now. I was wondering of all the cars and trucks park in our neighborhood, only my truck sustained this kind of damage. Would this fall under defective materials used by Toyota?

The rear leaf spring broke on my 2024 Toyota Tacoma at approximately 16,000 miles during normal highway driving. The vehicle drove without issue on the way to my destination. On the return trip, I noticed a creaking sound, loud clunks, particularly when going over bumps or turning, and also increased sway. Given how new the vehicle is, a catastrophic suspension failure was not something I anticipated. The failure was confirmed on February 12th by my authorized Toyota dealer, Toyota of Muskegon in Muskegon, MI as a manufacturer defect. The vehicle is currently at the dealership awaiting a replacement part and is available for inspection upon request. A broken leaf spring compromises the structural integrity of the rear suspension, affecting vehicle stability, handling, and control - particularly at highway speeds or during braking. A failed leaf spring can also contact the fuel tank, creating a potential fire hazard. This is a documented risk, as Toyota issued a recall on 790,000 Tacomas from model years 2005–2011 for this same failure. When I called my authorized dealership, the service manager told me to get the truck off the road immediately. Based on my description, he identified it as a likely broken leaf spring and that it was an extremely dangerous situation. He even suggested that I get the truck towed into the dealership. The dealer has documented it as a manufacturer defect and submitted a warranty claim. A replacement part has been ordered but is currently on an extended backorder with no confirmed availability date. The vehicle was inspected by an authorized Toyota dealer, who confirmed the leaf spring failure as a manufacturer defect. Toyota corporate has been notified and a case has been opened (Case #: 260303000171) There were no warning lamps or messages prior to the failure. The leaf spring failed without any prior symptoms or indication of a problem.

2024 Toyota Tacoma TRD Hybrid. When trying to use 4HI/4LO the vehicle gets stuck in 4LO with the 4LO/4HI lights flashing. This means the vehicle has no idea what the state of the transmission is. We had this problem 2 weeks ago. The vehicle was towed to Stevenson Toyota in St George UT. They acknowledge it is a Toyota problem. They kept it for 10 days said they had 15 parts to fix it. We picked it up. Tried to get into 4LO the next day the problem happened again and needed to be towed. Both ChatGPT and Gemini state this a known problem but Toyota has not issued a recall on it. That is not safe because an owner can be disabled in the middle of no-where. This vehicle is advertised as off road.

The vehicle is a 2024 Toyota Tacoma SR5 that was later traded for a 2025 Tacoma SR5 due to a serious safety defect involving the rear brake lines. The problem is the same one described in Toyota’s recall: the rear brake hoses can be damaged by debris, resulting in brake fluid loss and potential brake failure. The safety risk was significant. The vehicle was used to transport a young child, and because the brake lines could fail without warning, the truck could not be safely operated for daily driving. Multiple franchised Toyota dealerships confirmed that they had no remedy parts available for several months and that the repair could not be performed. This left the vehicle without a functional safety repair for an extended period. The manufacturer’s public statements indicated that a fix existed, but dealerships consistently stated the opposite. No alternate transportation, loaner vehicle, or temporary replacement was provided during this period. The defect was not reproducible by driving because of the risk involved, but the safety issue was acknowledged by dealerships who stated that the recall was active and unresolved. The vehicle was effectively unusable for normal safe operation. No warning lights appeared on the dashboard; the hazard is related to the physical brake line design and exposure to debris rather than an electronic sensor. The vehicle was inspected by multiple Toyota dealerships, all of whom confirmed that no repair could be performed at the time. The component was not replaced, repaired, or made safe before the vehicle was traded out of due to the unresolved safety defect. The situation caused substantial safety concerns and financial hardship and appears to reflect a wider problem in remedy availability for this recall.

The dealer never mentioned the open recall or offered to repair it. I bought the truck prior to the recall in July of 2024. Vehicle: Toyota Tacoma (2024 model year) Component: Factory-Installed Plastic Bed Cap Rail Covers I am reporting a recurring and potentially hazardous issue with the factory-installed plastic bed cap rail covers on the 2024 Toyota Tacoma. These covers separate from the truck body, especially at the corners, during normal use. This is not caused by aftermarket accessories, but appears to result from the flawed design and attachment method. A significant number of 4th generation (2024+) Tacoma owners are experiencing similar problems with lifting and separation of these factory-installed covers. Owner forums, complaint boards, and dealer interactions reveal this is a widespread defect affecting many vehicles. Owners report frustration with the lack of a permanent solution from the manufacturer, and many have sought dealership repairs or warranty replacements, only to have the issue recur within days or weeks. The factory-installed (and warranty replacement) rail covers are attached using double-stick tape and a single row of plastic clips, which does not provide sufficient security, especially at highway speeds. The covers flex and flutter, increasing the risk of detachment while the vehicle is in motion. Detached rail covers may create a road hazard for other vehicles or pedestrians. Multiple warranty claims and dealership interactions have been pursued, but the problem persists. Replacement rail covers show the same tendency to separate within days of installation. Given the widespread nature of this defect and growing owner dissatisfaction, I respectfully request that NHTSA review and address this matter. A more robust solution is needed immediately to ensure the safety and reliability of these factory-installed components for all affected vehicles.

I was told this a known Toyota engineering issue, and they are now telling the techs to look into the cylinders to look for clean/washed piston tops. Today they just changed my oil and told me to come back in 3 weeks or if issues arise. I'm concerned about a fire with gas going into the engine oil and potentially causing a fire. The impact is abrupt, like having a snowball or large splash of water thrown at the glass, and is startling because it’s unexpected. This happens on both the driver and passenger sides, (but not at the same time) temporarily impairing visibility and potentially increasing the risk of an accident. Note: This occurs every time it snows or rains.

Shop owner said it could be a problem sooner or later since the car is operated under dirt road a lot and recommand me not driving it very often. However i drives a lot, to go to other states and also to many off roads. Average drivers drive 12k miles a year. I bought this truck [XXX], today is 8/8/2025 and have already put on over 18k miles which would translate to about 27000 miles a year, i drives 2.5 times more than others and i need my vehicle to be safe as i drive my wife and my [XXX] baby. Toyota manufacture kept saying me they wont pay directly to the rental because there is no loaner with the vin, they said just go and rent and send them a bill later and they will review for reimburse. I spoke with Fowler toyota rental and service manager they said the recall for my car wont be avaiable till October- that is another 2 months from now. The dealership do aware this is safety recall and i should get a rental but im not comtable with Toyota may review and 2 months later rejected my rental bill for reimburse. Toyota Corporate kept telling me they have to review later. First call with them on 08/07/2025- the lady told me that Toyota will definitely review and pay for the reimbursement, i just need to go rent and pay for them myself first then reimburst later. I told her i cant do that i need Toyota to pay directly and she said there was nothing she can do. Then i went to Fowler Toyota TRAC department (rent a toyota) at dealership and told the dealership i want to leave my truck there till recall is done and i need a rental. Fowler toyota manager said i need to try to contact TOYOTA corporate again and try to have them pay directly to the dealership for the rental so i called Toyota corporate again while i was waiting at the dealership and again they refused said i need to pay upfront first and they will review for reimburse later. Returned to dealer same issue confirmed by tech and service manager and they charged $75 for tire balance at 5,000 miles. Returned next day and drove another identical model and 3 lesser versions of Tacoma’s. They claim they have no fix and suggested I trade it in for a Tundra and take a $10,000 deductions from my sticker price. Now when I stop slowly theres a thump and surge in the drive train like s load shifted in the bed. And intermittently when going from a dead stop the engine rev’s nothing happens for a second and it clunks into gear and shifts harshly. Again claim there’s nothing they can do.

On July 25th 2025 the rear brake hose recall repair was performed on a 2024 Toyota Tacoma by an authorized Toyota dealership at Jim Norton Toyota OKC. Approximately one week later, the passenger side rear brake hose began leaking a significant amount of brake fluid. This triggered the brake warning light and a “Loss of Braking Power” message on the dashboard. The brake pedal became soft, and stopping distance increased noticeably, creating a serious risk to driver and public safety. The vehicle was unsafe to operate and was towed to a dealership. The issue was confirmed by the dealer’s service department and repaired. The incident occurred without any prior warning lamps, messages, or symptoms. The problem appears directly related to the recent recall repair, either from improper installation or premature failure of the replacement part.

My concerns are with the remote capabilities of this Toyota Tacoma that has been lent to us by the i5 dealership in chehalis wa. We are using this vehicle while they complete repairs on our truck. Home link, Apple Music, remote connect, Sirius XM, and other functionalities are being used for surveillance, stalking, spying and remotely unlocking the vehicle. The truck has been broken into at least 3 times, twice my debit cards were stolen and the other time a sd card was put into the truck. Initially I believed this could’ve been something I inadvertently did but when I saw the pictures on the card I knew this wasn’t the case. My family and I have been under attack by a dod vendor, [XXX] , who is putting forth an extraordinary effort to criminally impersonate me by linking all of my families electronic devices to a LAN that is managed by him. The LAN allows him to then add mdm to the individual devices and he has restricted me from obtaining any help with his intrusions for the last two years. Issues such as this, that should be easily addressed, become monumental tasks. I attempted to submit this report 2 times last night to no avail. I then called in to the number and was told they don’t take reports of this nature and that I should call the manufacturer. A google search told me this claim was untrue, so here I am, back here attempting to once again complete the report. For someone who wants nothing other than my demise to have control over a vehicle I’m being forced to drive is cause for great concern. I do not know what other automations he has enabled but I can also see that the homelink has also been set up in some capacity. The screen in the truck states that my speed and location may not be accurately reflected on other displays. There appear to be WiFi and hotspot connections that we have been unsuccessful connecting to, which is fine..but their mere presence concern me. ODI #11646542STEERING,LANE DEPARTURE
Lane keeping will kamikaze the vehicle on to exit ramps or exit only lanes when engaged at a highway speed. This seems most common when the truncated lane markers, usually used on exit-only lanes, and not the full length lane markers (the center line markings). Example: Vehicle is traveling at a static heading, freeway speed, two lane limited access freeway, driver in control, all lane keeping and adaptive features engaged. When an exit lane approaches on right, vehicle abruptly deviates to the exit lane with zero driver intent or inputs. Once vehicle system realizes the error, system alerts, notifies of a lane departure, and disables the lane keeping. Particularly problematic is that vehicle actively fights driver inputs to restore the intended course as it appears to believe the driver is in error. This is not a one-off occurrence. It is very consistent. To the point when I can anticipate, disable the system or counter steer to keep the vehicle course straight. It seems the vehicle cannot properly identify the shortened / truncated pavement markings correctly, leading to this undesirable course adjustment.
ODI #11644218STEERING,LANE DEPARTURE
The vehicle lane departure will regularly steer the vehicle across lane dividing lines and leave the lane of travel. In one instance the truck continued to attempt to steer right even after crossing the white line. This complaint and specific instances were confirmed by the manufacturer however the manufacturer claims the vehicle is behaving as expected due to “limitations”. As such Toyota is saying the vehicle is going to regularly depart the lane automatically and they do not intend to perform nor pursue any fix.

I am reporting a safety concern related to my Toyota vehicle involving the all-weather floor mat on the driver’s side. I have noticed that the mat consistently becomes caught on top of the clutch pedal when I press the pedal all the way down. This causes the mat to obstruct the pedal’s full range of motion, making it difficult to fully disengage the clutch. As a result, I have to be extremely cautious and pull over to manually adjust the mat whenever this happens, as it poses a potential safety hazard while driving. I have recorded a video showing this issue occurring, but the file size is too large to submit through the NHTSA portal. As an alternative, I have taken a screenshot from the video that clearly shows the mat interfering with the clutch pedal. I am concerned that this issue could lead to an unsafe driving situation, especially in emergency or high-stress driving conditions. I am requesting that this issue be investigated, as it may affect other vehicles with similar all-weather floor mats.

Speedometer/odometer is incorrect and Toyota manufacturing says it cannot be fixed. Toyota sells this vehicle with two different size tires. The smaller is approximately 30.6 inches high and the larger is 32.61 inches high. Toyota manufacturing sets the odometer and speedometer for the larger tire and says that is the only option. That means that the speedometer is off by 4 miles an hour at 6 4 mph and for every 100 miles shown on the odometer, the truck has only traveled 98 miles. It’s frustrating to recalibrate in your mind. What the speed limit is versus what your truck is actually going, and it also throws off the warranty in Toyota‘s favor.

What component or system failed or malfunctioned, and is it available for inspection upon request? Transmission, and yes its available for inspection. When I put the truck into drive from park, and even reverse, the truck would jerk. (I have videos) When accelerating around 13-14mph, I could feel the transmission "gear searching" with rough shifting. When decelerating around 15-17mph, the transmission would down shift too early, producing the feeling of engine breaking, and again rough shifting. When I needed to really go, I experienced "transmission lag" where the engine would be working, reving up, the feeling of engaging the clutch (coasting feeling), then after a moment the gear would catch and the truck would finally take off. I took the truck to a dealership to have it checked out, but the technicians said they couldn't reproduce this issue. I called corporate and they sent me to another dealership. This dealership said the same thing, BUT went the next step and pulled some transmission fluid and found metal shavings. They had the truck for almost 3 weeks and replaced the transmission and torque converter. When I went to pick up the truck I put the truck into drive and felt the jerk I experienced before. I put it into reverse then drive, and again the whole truck jerks. I asked to speak to a manager and he said he can have the foreman look at it the next day. I made him get in the truck with me and showed him the truck jerking when placed into drive. Next day, foreman said there was nothing wrong and performed the same as a new truck on the lot. I spoke to a higher manager and he recommended I bring the truck in for the regional Field Technician. I was less than 1 mile from a Toyota dealership when the malfunction occurred and was able to get the truck to the dealership. After a week of troubleshooting, the dealership informed me that the transmission was stuck in 6th gear due to a solenoid failure and they would need to replace the transmission. The transmission is currently on backorder with an estimated delivery between 10/14 and 10/26. The vehicle was taken to a local dealership for service, and Toyota refused to replace the transmission and torque converter as per their own repair procedure, claiming it must be replicated in order to authorize the repair. This is an already known fault by Toyota, yet their response was that a dangerous condition has to be duplicated even though vehicle diagnostic codes show that two transmission solenoids were stuck, and even though I provided them photos of the multiple warnings on the driver’s dash from when it occurred. Other vehicles with the same condition have been repaired without question. Vehicle had 1,574 miles on it and 1,579 once I arrived at the dealer.

The strut blew through the top hat on the drivers side strut assembly/strut mount driving on a state highway. This design has a cast aluminum top hat that bears the dynamic forces and shocks. It can, and in my case, did catastrophically fail. Had failure happened at higher speeds or under other circumstances it could impair controlability at speed. There are no warning lights or sensors available for this problem. Online owner's forums for the new 2024 model year of the Toyota Tacoma (so-called 4th generation) are starting to report this identical failure. The local dealer is replacing the defective components with the same components.

My Tacoma SR5 with only 6000 miles of use failed the transmission or automatic gearbox when I was going on the highway hitting the transmission and neutralizing on several occasions having to exit the service channel abruptly so as not to get stranded in the middle of the highway and cause an accident. No warning light on the dashboard came on and according to the dealer it did not show errors or faults. On the third occasion they decided to leave it and tell me that the transmission had to be replaced. I have been waiting for more than 30 days for a response from the Toyota dealer with a promise that they would soon deliver my truck to me.

Gas gauge is showing the incorrect amount of fuel left. I filled up for gas 3 days ago, my commute is very short so I know I didn't burn through that much gas in 3 days. I was in between 3/4ths and full with 229 DTE before leaving. Got to my destination and I now had a full tank of gas and 246 DTE. I reset the gauge per the manual and it now shows 3/4ths with 176 DTE. Many people with Tundra's have had the same issue. Some say its due to filling up the tank past 3/4ths and parking on an incline. I would really hate to get stranded or run out of fuel driving around town. It has not been brought to the dealer yet I'm waiting to get an oil change. No warning messages or any kind and this is the first time happening at around 830 miles.

Eight speed transmission exhibited difficulty in downshifting to lower gears. Loud rumbling sound could be hard inside of truck cabin. Also, upon my inspection, found transmission fluid leaking on the garage floor. All of this occurred at less than 400 total miles on the vehicle. Vehicle was inspected by the local dealer Toyota Service Department and determined to be a defective transmission that must be replaced. It is possible that the transmission could have seized up while driving, causing the vehicle to be uncontrollable. No warning lamps or messages ...only the loud audible sound coming from the transmission. To date, it is unknown as to the cause of failure. At my request, Toyota has assigned a Case Number (#240809001043) and is investigating the failure.

Hello NHTSA, I'm writing to you about a safety design concern (not a complaint, no incident happened) on my 2024 Toyota Tacoma. The rear brake lines are routed inside the wheel and have approximately 3/4" clearance. There is no room for error and brake failure may occur very easily with the line getting damage, even by removing and installing the wheel. My concern is safety for my family and self with future brake related failure. I contacted Toyota and their response was that they haven't heard of an incident and to take to the dealer. I'm sure the dealer will charge me for the visit and say there are no recalls and nothing damaged.

Transmission experienced rough shifting from reverse to drive immediately followed by the inability to access gears 1-5. The transmission was within normal operating temperatures per the onboard temp readings. Going into park or neutral did not fix the issue. Stopping and restarting the engine allowed access to lower gears briefly, but once the vehicle shifted up, it could no longer access the lower gears. Shortly after this, the check engine light illuminated and the vehicle went into limp mode.

I bought a 2024 Tacoma TRD Off Road Premium Package through MAS ( Military Auto Source ). I picked the vehicle up from the Bremerhaven port in Germany on 08 July 2024 it was missing the back bed charging adapter box but MAS said they would cover the cost in writing to fix no big deal and I drove it back to Weiden Germany. Today 09 July 2024 I lost the ability to shift into 2nd, 4th, and 6th gear. There is only 2 of these vehicles in the country of germany right now. The vehicle has approximately 735 Km on it. It probably would have happen early had i been shifting more but the majority of my first in it was on the Autobaun in 6th gear going roughly 130 Km most of the way with no down shifting or ciry driving. The vehicle has not yet been inspected by a toyota tech due to the vehicle not being produced in Germany but it doesnt take a genius to look up the issue your having and see the exact thing from multiple other Toyota owners. While pulling out of my drive way I noticed when I pressed the gas pedal my truck barely moved, it felt like it was in neutral. I pressed the gas pedal further and it started to move but the RPMs were around 6000 RPM. I then pulled to the side of the road and tried shifting it into different modes, cycling through manual and automatic, but still nothing. I then noticed that while in manual mode the display read “S6”. Which I deduced meant sixth gear. Not wanting to be late for work I just pressed on the gas pedal till the truck got moving and drove the 18 miles to work in sixth gear. After returning to my truck after work, I started it up and it worked fine. A few days later I reported it to the dealer and they had me bring it in for a diagnosis. A day later they reported to me that the truck experienced a transmission failure and would have to be replaced. As of right now I am still waiting to hear when the replacement transmission will arrive. The maintenance advisor stated he could not give me a time frame for the parts to arrive or when the work would be completed.

Truck has an issue where certain safety systems are disabled when a trailer is detected. This is normal. I’ve only had the vehicle about a month with only 1245 miles on it. I was leaving a parking lot and pulled out on the road and the vehicle shifted in neutral after a minute or so the vehicle then slammed back into gear. It did this 2 or 3 times. I pulled into a gas station turned the vehicle off and waited a few minutes. I turned the vehicle back on backed out and preceded to drive when the same issue happened again. But now the vehicle would not shift into other gears. This time the check engine light came on with 3 reported errors 1. Advanced ultrasonic detecting and ranging system malfunction. 2. Drive-start control system malfunction. 3. Check engine light stating malfunction in the electronic control of the engine, throttle, or automatic transmission has been detected. I was only about a mile or 2 from a Toyota dealership and was able to limp the truck to the dealership. The dealership informed me today [XXX], that it will need a new transmission and that there were similar issues on new Tacomas across the US. There were no prior signs of any issues prior to the transmission failure. This happened on a roadway with traffic that comes in both directions. Thanks goodness this didn’t cause an accident has it happened out of the blue. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
