OnStar Module failure. This did not trigger any type of notification. I had to make several phone calls to OnStar to determine I would need to schedule a service appointment for a technician to look at the vehicle. After inspection it was noted that the OnStar Module is not working and will need to be replaced. Upon ordering the part I was informed it is on backorder with no delivery time noted. If I were to get into a crash OnStar would not have triggered a call to first responders for my safety - which is a service that I pay for. No warning lights appeared and the the OnStar button still shows "green" as in active service.
On Monday, April 6th, I was turning into a parking lot and the braking system, specifically the anti-lock braking system (ABS) and electronic brake control module, appeared to malfunction. While driving, I received a warning indicating the ABS was off, and the brake pedal became soft with reduced stopping ability. The check engine light also came on. I had to coast the vehicle to a complete stop. The vehicle is available for inspection upon request. The loss of normal braking function significantly reduced my ability to stop the vehicle safely, increasing the risk of a collision. This created a dangerous situation for myself, passengers, and other drivers on the road. Brake failure at any speed presents a serious safety hazard.I am aware of similar complaints reported for this issue. My vehicle was towed to and inspected by a dealership where it sat overnight before being diagnosed; however, they were unable to reproduce the issue at the time of service. Despite this, the braking failure did occur and remains a safety concern. The service advisor told me to "bring it back if it happens again" and that it "lost communication with the EBCM" which sometimes happens. The vehicle has not yet been inspected by the manufacturer directly, police, or insurance representatives. There were no warning lamps or other symptoms of this problem prior to the failure.

When attempting to engage the Left turn signal, the lever has to be pushed down several times to "catch or engage" and stay engaged. Frequently it won't stay engaged and turns off before the turn is completed. (The Right turn signal works properly). This happens 8 out of 10 times on a consistent basis. The dealer shop supervisor and tech test drove it as well as another brand new lot vehicle and both did the same thing. They said that that's what they all do, and I should push down more firmly, which doesn't help. I'm in my late [XXX] , have driven new GM vehicles for decades and know how to use a turn signal. I have been in the middle of LH turns and the turn signal was not flashing to advise other drivers of my intent to turn. I believe that this is a significant and dangerous safety issue. I bought the vehicle new a little over a year ago and it currently has about 2500 miles on it. I am not sure when this started happening, but I started to notice it at about 1700 miles, 6 months ago when I started driving it regularly in place of my 2024 Chevrolet 1500 pickup. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
On four occasions while operating the vehicle down the roadway the dash screen and infotainment / vehicle information displays all have went dark off and reset causing disruption to vital information. Cruise control stayed active. It is unknown if the advanced safety features failed during the reset.
1 March, Onstar failed while utilizing the gps late at night. Stuck on side of the road until a passersby assisted me. 2 March, took truck to dealership, after 4 days said no issue found. Driving home, same issue persisted. Called dealership, told to bring back, 45 minutes away. Declined because was told no issue found. 6 March, morning truck completely dead. Called dealership and was told to have towed back to dealership. I declined because you didn’t find any issues when it was there. Following morning, 7 March, had truck towed to local dealership. After a week, was told Onstar telematics module failed and needs to be replaced. As of today, March 29, the part is on back order.
There are two issues with the headlights as it relates to the instrument panel and controls. First, the symbol on the instrument panel is the same for both headlights and running lights. This means at dusk, I cannot know whether my auto-headlights have turned or if I need to override the auto-feature to turn them on manually. Second, the headlight controls are buried in a menu, hidden behind the steering wheel. The "quick menu" is inconvenient, but the fact that it is hidden bedding the steering wheel means that I have to lean forward and around to fiddle with it, which has led to me drifting into another lane as I am distracted. From the driver's perspective, the "quick menus" are all hidden behind the wheel, and the control panel is just far enough out of reach that the driver must adjust and lean forward to reach for any controls not on the steering column.
The rim has a crack in it and it won’t hold air
Notice the windshield was cracked after leaving a restaurant!
The vehicle has experienced repeated failures of the integrated telematics system, including loss of GPS location accuracy, loss of data connectivity, navigation malfunction, and intermittent failure of voice command and steering wheel controls. On one occasion, the vehicle experienced a complete display blackout while driving, during which both the infotainment screen and digital instrument cluster went entirely black. The condition has occurred repeatedly and persisted despite multiple repair attempts. The vehicle has been out of service for approximately 82 cumulative days for diagnosis and repair of these issues. An authorized Chevrolet dealership ultimately diagnosed a failure of the telematics control module, which was replaced under warranty. Following this repair, the same data connectivity failure recurred and was documented with photo and video evidence. The vehicle was returned to the dealership for further diagnosis. The failure is intermittent and may not be present during every inspection. The telematics system supports navigation, driver information displays, and emergency communication features, including transmission of vehicle location and system data. Loss of GPS and connectivity may impair the vehicle’s ability to provide accurate location information during an emergency. The prior display blackout resulted in loss of driver information while the vehicle was in motion. The condition has been reported multiple times to authorized Chevrolet dealerships. Diagnostic procedures and programming were performed, and the telematics module replacement confirms a system-level failure. Recurrence after component replacement indicates the condition has not been permanently resolved. Prior symptoms included loss of GPS signal, incorrect location, loss of connectivity, navigation failure, and intermittent driver assistance alerts.
The lane keep assist never works properly, it vehicle doesn’t stay in lane, also gps has never worked properly, it’s been to the dealers several times and parts have been replaced with out any success
On Star module failure with severe backorder on parts
The telematic control module drains the battery. I bought the car in February 14, 2026 and it was towed to the dealer on February 20. Today is March 17 and I have had no results. I opened a case with GM directly but I receive nothing but canned automatic emails. The dealer isn't taking my calls. Please help.
21" Equinox EV RS wheel cracked at the lip even though we do not recall hitting anything. No known potholes or other impacts. This caused air loss from the tire and had to refill the air at least once a day. When the tire shop removed the tire to inspect it further we found no damage to the tire, just the wheel was cracked. Called dealership and they claimed it would not be a warranty issue. We ended up putting different wheels on the car as we didn't want to have this issue again.
My vehicle has had recurring interior water intrusion and related safety concerns. The most serious symptom was interior window fogging on the inside of the vehicle for multiple days during or following rainy, snowy, and cold conditions, which significantly reduced visibility while driving and made it difficult to see out of the windshield and side windows. To clear the fogging quickly, I have had to turn off the heated seats and defrosters and open the windows; otherwise, I have needed to pull over and wait until I could safely see through the windshield again. A strong mildew odor would also develop and could last for days after rain. Sometimes the seats felt damp and the odor became worse when the heated seats were turned on. The first repair visit for this issue was January 13 to 19, 2026 and during that visit, the dealer was unable to recreate the issue. The problem continued, and after my navigation/infotainment screen began having issues, I returned the vehicle to the dealer again on March 7, 2026. I was then informed there was water intrusion and water-damaged carpeting, and I was told there was a known issue involving a faulty or broken seal on this model and there was a recall, but I have not been able to confirm that recall by VIN online. It remains unclear whether the leak is the full cause of the repeated interior fogging condition. In addition to the water intrusion issue, the vehicle has also had other electrical and safety-related concerns, including the infotainment screen turning white for weeks, abnormal turn-signal behavior after completing turns, lane-assist steering irregularities that pulled the vehicle toward the lane line, and braking irregularities, including the brakes locking up while backing into a parking space or during slow stop-and-go traffic. It is currently unknown whether moisture intrusion contributed to any of these other issues. My vehicle is still with the dealer for repairs until around April 15.
While pulling into a parking spot of a business and approximately 6 feet from the curb, the automatic emergency brake engaged forcing my whole body forward, slamming my head on the visor/roof of the car very forcefully/painfully. I was not in traffic, I was getting ready to park. My front seat passenger slammed against her seatbelt and I had a child in the back seat who slammed forward as well. No dash indicators or seat rumbling occurred, it was just an immediate jolt/hard stop that I was not braced for. At risk of concussion or TBI.
I purchased a 2025 Chevy Equinox EV last year out of the local Chevy dealership It is my first brand new vehicle and I’m the only owner, I have only had this vehicle for a few months and it has already showed me significant issues with the Electronic components Main issues I’ve experienced are: Randomly shutting off Randomly phantom braking On star capabilities not working No access to WiFi or any apps on the touch screen I.e. maps, music, settings, volume, radio etc. A/C/heating unit not working won’t get cold or hot just blowing air Issues with charging (almost left me stranded multiple times) not fast charging Issues with my Chevy app Took the vehicle to the dealership to get looked at they have had it for a week already and have not given me any update except that I will need a new onstar module. The vehicle is not safe to drive and I’m afraid that it will shut off while I’m driving at a higher speed I.e. on the freeway or that it will phantom brake while I’m on the highway which can cause a life threatening injury This vehicle has not been in any accident No warning lights came on except one message that said service emergency calling No warning messages prior to any of these events
The illuminated dashboard, center control console, and headlights suddenly turned off while the vehicle was in motion during a dark night. The blackout lasted approximately 10 seconds and I was unable to see to keep the vehicle on the roadway. After running onto the unpaved shoulder, I was able to fully stop the vehicle without hitting anything. All of the lights came back on and the dashboard showed a message that said "Auto shutoff". Then everything returned to normal. This problem hasn't been reported to the dealer yet, but will be discussed during an upcoming service to correct one of the other problems that keep occurring with this vehicle.
The vehicle’s high-voltage battery system and propulsion system appear to have failed, causing a complete loss of power while driving. The vehicle is currently at the dealership for inspection, The vehicle completely shut down while I was driving on Florida’s Turnpike, creating an extremely dangerous situation on a high-speed highway. There was no warning prior to the shutdown, and I am currently pregnant, which made the situation especially traumatic and heightened the risk to both myself and my unborn child, as well as other drivers on the road. The vehicle has been taken to the dealership for inspection. As of now, the problem has not been reproduced, but the shutdown event is confirmed and documented by the dealer during the incident report and intake process. The vehicle is currently at the dealership for inspection. General Motors engineers or technical assistance center (TAC) may review the vehicle once the dealership reports the incident. No police or insurance representatives have inspected the vehicle for this issue. No warning lamps, messages, or other symptoms appeared prior to the sudden battery drop and complete loss of propulsion. The failure occurred without any prior indication while driving on the highway.
When accelerating to enter a road with high speed traffic the engine dies for a few seconds resulting in on coming traffic having to brake to avoid a collision. Then, the engine suddenly accelerates wildly. This problem occurs every time the vehicle is driven. The dealer service people has "tested" by driving the vehicle for 7 miles and declared there is nothing wrong. This is dangerous because the vehicle engine dies/stalls at a critical moment and if an oncoming driver isn't paying attention this could result in a collision.
am writing again regarding serious safety concerns with my Chevrolet Equinox EV RS. This is my fourth attempt to contact Chevrolet, as my previous emails have not received a response. These issues present a significant safety risk and require immediate attention. The problems I am experiencing include: Unintended lane movement: The vehicle shifts into another lane without any steering input from me. Brake response delay: The brakes fail to respond appropriately, causing a delay in stopping the vehicle.Steering wheel vibration: The steering wheel begins to shake after the vehicle exceeds approximately 55 mph.Steering wheel vibration: The steering wheel begins to shake after the vehicle exceeds approximately 55 mph.Due to these issues, I have almost been involved in three separate accidents, which is extremely concerning. These conditions make the vehicle unsafe to operate.I am requesting the following as soon as possible:A formal case number for this safety complaint.Immediate guidance on inspection or repair through an authorized Chevrolet dealership.Confirmation that these safety concerns will be investigated and resolved promptly.
OnStar Telematics module is defective and keeps draining the battery causing the need for multiple jump starts. The problem has been confirmed by Don Hewlett Chevrolet Buick on March 5th, 2026 and was told that the part is on backorder and that there is no ETA.
After hitting a deer, the body shop fixed the damage to the front part of the vehicle. They realized the battery kept being drained even after purchasing a new one. It’s now at a dealership and they are staying it needs a new ONSTAR module, not sure when it will come in either. I’m paying for a rent car that I can’t afford to pay for anymore. I wanted them to just do away with the ONSTAR but they said it’s not possible.
Driving down the road with my mom and my panoramic sunroof randomly exploded sending glass through the car. This was a very dangerous situation. Is the glass defective ? This should not happen and is a safety concern.
2025 CHEVROLET EQUINOX HAD, ON NUMROUS OCCASIONS, COMPLETE POWER FAILURE. THE VEHICLE HAD TO BE JUMP STARTED MORE THAN ONCE. THE VEHICLE WAS BROUGHT IN TO THE DEALERSHIP, BY THE OWNER, WITH POWER FAILURE CONCERNS. THE VEHICLE WAS TOWED TO THE DEALERSHIP DUE TO COMPLETE POWER FAILURE CONCERNS. THE VEHICLE HAD BRAKE PEDAL ISSUE WHEN POWER FAILURE OCCURRED. MEANING THE BRAKE PEDAL WAS NOT ABLE TO BE PRESSED. DISPLAY SIGNALS: ERRONEOUS DATE/TIME, ABS, CHECK ENGINE, VEHICLE DRIVE MODE UNAVAILABLE YOUR VEHICLE MAY NEED SERVICE. FURTHERMORE, ON ONE OCASSION A POWER SURGE WAS DETECTED. THE VEHICLE WAS DIAGNOSED AND AN ADVISOR STATED THE TELEMETRY MODULE IS GOING OUT. HOWEVER, THE PART THAT WAS REPLACED IN THE 2025 CHEVROLET EQUINOX, SEVERAL WEEKS LATER, WAS ACTUALLY THE TELEMATIC CONTROL MODULE. THE VEHICLE DISPLAYS DRIVE MODE UNAVAILABLE YOUR VEHICLE MAY NEED SERVICE, EVEN AFTER TELEMATIC CONTROL MODULE REPLACEMENT BEGINNING THE NEXT DAY AFTER PART REPLACEMENT. THE MESSAGE DISPLAYED BY THE 2025 EQUINOX OCCURS ON A DAILY BASIS. ISSUES ARE OCTOBER 2025 TO PRESENT DAY.
My Onstar module is draining my battery. I have to constantly get my car jumped to get to and from. It's a danger to my job as a rideshare driver. I went to the chevy dealership today and they told me that the part I needed to order is on back order. How can a piece like that be on back order. This could potentially mess people's alternators and starters up in their cars.
Was told bad fluid pump at 35k. Replaced. Car broke down 30 seconds from dealer. New transmission to be replaced now. Parts on backorder. Will be without car for “months”. No symptoms prior to failure. No warning lights. Car just wouldn’t start.
The onstar telematics module has stopped functioning. That means I am not receiving any systems updates, cannot use onstar, and cannot use the my Chevrolet app for remote start or remote unlock. I took it to the dealer 2 weeks ago and they said the telematics module is backordered. They did do a number of system updates. After those updates my adaptive cruise control has completely stopped working. When I called about that, the service department said that one module could have caused the other module to fail and that they won’t try to look at my cruise control until after they can replace the telematics. There is currently no ETA to receive the part.
VEHICLE INFORMATION Vehicle: 2025 Chevrolet Equinox (Gas) Purchase Price: $46,000 In-Service Date: January 28, 2026 Current Mileage: 2,433 miles First Repair Attempt: Approximately 200 miles DEFECT DESCRIPTION (SAFETY-CRITICAL) The vehicle experiences intermittent but recurring safety system failures while driving, including: • Complete instrument cluster blackout (all gauges dark) • All warning lights and alerts disappear • ADAS / driver-assist systems shut down • System appears to reboot for approximately 15 seconds while in motion • Vehicle settings reset to default (vehicle systems, not radio) • Brake pedal becomes hard during these events • Stopping distance feels increased during braking events This behavior indicates a brake assist interruption and a loss of critical driver information while the vehicle is moving. REPAIR HISTORY SUMMARY • Vehicle has been in the dealership approximately 4–5 times for the same or substantially similar defect • Dealership has performed software updates • Dealership has replaced computer modules controlling the information center and related systems • The defect persists SAFETY ASSESSMENT Loss of braking assist (hard pedal with increased stopping distance) combined with full instrument cluster blackout while driving constitutes a substantial impairment of vehicle safety. This is not an infotainment issue.
Almost every time I fill up with gas ( I use top tier like the manual states), once I start driving, it feels like it misfires or jerks. It will do it one/ two times and then it’s fine… until I fill up with gas again. It’s been doing this since I got the vehicle- no indicator lights on.
Blind spot indicator lights ( both left and right side) randomly alert when there is nothing beside me. Also when I’m backing into a parking spot it also randomly blinks fast.
While driving under normal conditions, a body panel detached from my vehicle and flew off without any collision, impact, or external cause. This was not an accident. The vehicle was being operated properly, and the failure occurred spontaneously, creating a significant safety risk to myself and other motorists. Since purchasing this vehicle less than one year ago, it has been subject to multiple recalls, raising further concerns about overall build quality and safety. I am also aware of reports from other Chevrolet Equinox EV owners who have experienced similar component failures, suggesting a potential pattern of defects. When I attempted to address this issue through the dealership and insurance channels, my concerns were dismissed, and the incident was incorrectly characterized as an “accident,” despite the absence of any collision or driver error.
Vehicle telematics module defect across the board that deactivates safety features from Onstar in case of an accident. GPS and Internet connection lost.
SEE NHTSA Safety Recall Report 25V-12- It appears it should apply to both 2025 FRONT WHEEL DRIVE Chevy Equinox EVs with ADAPTIVE CRUISE CONTROL (and without Super Cruise) in addition to 2025 ALL WHEEL DRIVE Equinox EVs with ACC (also without Super Cruise). This is very dangerous. Here's what happened: My 8 month old 2025 Chevrolet Equinox EV FRONT WHEEL DRIVE w/ACC(no Super Cruise) has had 2 instances where a very large "thunk" sound and vibration emanating from the rear of the car while stopped at an intersection, was immediately followed with sudden brake failure and sudden forward acceleration. The ACC was on in both instances. The first time this happened was alarming but since the was nothing in front of me there was no damage and the problem seemed to resolve itself thereafter. The second time was while waiting at a red light about 10 feet behind another vehicle. My car shot forward and into the other car, which was then pushed into the car ahead of it. Both my car and the car ahead of me had very expensive damages, but fortunately, no injuries. What I experienced sounds exactly like the recall problem described in NHTSA Safety Recall Report 25V-12 which has thus far only been applied to the AWD version of the 2025 Equinox EV, not the FWD version that I have. I believe this needs rapid consideration due to the danger of of sudden acceleration resulting in serious property damage, injury, or death. It appears to me that other instances of this problem have been described in various online forums and YouTubes.
Just after Thanksgiving, my vehicle displayed a “Service Emergency Calling” message. I brought it to the dealership and they documented the concern, referencing GM bulletin PIP6077B, but no repairs were performed. On December 30th, while parking, I shifted into reverse and the vehicle would not reverse and instead moved forward. A “Service Transmission” warning appeared and the vehicle was towed. The dealer cleared the codes and released the vehicle without performing any diagnostics. About two weeks later, when starting the vehicle after work, several warnings appeared, including “Service Safety Restraint System,” “Forward Collision System Unavailable,” and a rear-seat alert despite no passengers. While driving, lane keep assist, lane departure warning, and blind zone steering assist were not functioning. The next morning, the vehicle suffered a complete electrical failure. It would not start, the screens stayed off, and the doors would not lock or unlock. The vehicle was towed again. The dealer replaced the battery, claimed it had simply died, and attempted to return the vehicle to me. I refused because none of the prior warnings were diagnosed, and I was told the codes “erase when the battery is changed.” After I demanded full electrical and transmission diagnostics, including a parasitic draw test, the dealership informed me the vehicle was unsafe to drive. They stated the OnStar/telematics module was draining the battery and the replacement part was backordered with no estimated arrival date. I was told multiple customers are waiting for the same part, which means this is NOT an isolated event and needs way more attention than it's being given. This defect results in loss of essential safety systems and complete vehicle shutdown, creating a significant safety risk.
The contact owns a 2025 Chevrolet Equinox. The contact stated that the vehicle failed to start up as needed. The vehicle was towed to the dealer; however, the cause of the failure was not yet determined. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 2,600.
The angle of the windshield reflects the dashboard so much that you can't see out the windshield when heading anywhere towards the sun. It also reflects the dash at night inside the car. It's a significant safety hazard and a driving hazard the car creates for itself. This is a design defect, according to all service departments at GM I have visited to address the issue. Update: The dash cover remedy GM provided did not help; in fact, it's more annoying now than before and distracting.
Onstar module isnt working and was told to report it to you guys.
IN THE EQUINOX LT, VIN [XXX] , THE TELEMETRY MODULE HAS AN ISSUE PER THE DEALERSHIP. THE VEHICLE LOSES POWER DUE TO THE TELEMETRY MODULE ISSUE. THE PART IS ON ORDER AND AS OF JANUARY 16, 2026. IT WILL TAKE 4 TO 6 WEEKS TO ARRIVE. IN THE MEAN TIME I HAVE HAD TO CALL ROADSIDE ASSISTANCE TWICE IN TWO WEEKS, AND THE VEHICLE HAS BEEN JUMPED 3 TIMES IN THE PAST TWO WEEKS. HOWEVER, THE VEHICLE HAS BEEN TO THE DEALERSHIP 4 TIMES. ONCE PRIOR TO THE DIAGNOSIS, AND THREE TIMES SINCE THE DIAGNOSIS. THE VEHICLE HAS APPROXIMATELY 16, 600 MILES, AND IS DRIVEN TO AND FROM WORK. THE TELEMETRY MODULE IS THE CAUSE OF THE OVER ALL ISSUE, PER THE DEALERSHIP. THE BATTERY CONTINUES TO GO DEAD ON VEHICLE [XXX] , 2025 CHEVROLET EQUINOX. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
Windshield glare is horrible. It can cause distortion of objects. It can cause dizziness. Especially on bright sunlight and when many lights from oncoming traffic reflect off the windshield. I have had to pull over a few times. The dashboard shininess is a real problem. The dealerships don’t want to order the free dash mat unless they “diagnose” it even though it is a known problem and they ignored my concerns when I went in to the dealership even though a free dash mat is offered by GM The fix for this in reality is a new dashboard that will end this problem but that is too expensive for GM. They all say to wear polarized glasses to help. Wrong. That’s like putting a bandaid over a gunshot wound. If they aren’t willing to fix this error I want them to buy me out cuz it’s not a small problem that could result in injury or death if not addressed. The 2026 models have a new dashboard that corrects the problem. This problem has caused me not feel safe and avoid driving sometimes and this is not acceptable to me.
While driving my Equinoix EV I lost propulsion and the dashboard started flashing. I could coast to curb lane. When fully stopped I turned the car off and on again and propulsion resumed. I have taken it in to the dealership and they said they see no error message and I should continue driving to see if it reoccurs. They suggested I keep cloth to wipe off windshield where sensors are in case it was obscured. This happened in clear weather.
Light from recessed LED headlights becomes obscured by packed snow when traveling in a snowstorm. LED lamp does not emit enough heat to keep clear. Loss of light is gradual and can reach an unsafe level almost without notice.
My Onstar module and Telematics is not working and Chevy/GM is trying to refuse to fix it and claim it's due to not having the Onstar internet plan. If that was the case it would never work, yet it works some days then other days it shows me in another city. My Google assistance will show not connected when Google maps doesn't work. I've only had this car since April 2025. This needs to become a recall. GM can't keep doing this to their customers.
On three occasions, gas pedal went limp, and required restarting engine to reset controls to get gas pedal working again. On one occasion, brakes failed to operate leading to a collision. And forward collision warning did not activate.
GOT THE EQUINOX BRAND NEW ON MARCH 2025. FEW WEEKS LATER HAD TO TAKE IT TO SERVICE/DEALER WITH BACKUP CAMERA PROBLEMS AND IT HAD TO BE REPLACED. IT STARTED TO HAVE SPEAKERS ISSUES DUE TO A VERY LOUD HUMMING SOUND WHILE DRIVING. ALSO WINDOW FROM DRIVERS SIDE RUNS SLOWER THAN THE REST OF WINDOWS. THE VISIBILITY THROUGH FRONT GLASS IS NON EXISTENT WHEN RAINING DUE TO THE ANGLE. TRANSMITION STARTED SLIPPING AND STARTED TO HAVE A CHECK ENGINE LIGHT POP UP AND HAD TO TAKE IT TO SERVICE AGAIN... AND FINALLY AT THE BEGGINING OF NOVEMBER HAD TO TAKE IT TO SERVICE DUE TO A SUDDEN BATTERY DRAIN. SHE WOULDNT TURN ON FOR NO REASON.THE DEFECTIVE MODULE IS ON BACKORDER WITH NO ESTIMATED ARRIVAL DATE, LEAVING THE VEHICLE UNUSABLE. ITS BEEN THERE FOR 2 MONTHS NOW... THEY PROVIDED A LOANER. EVERYTHING HAS BEEN CONFIRMED BY DEALER. THEY SAID THAT HAD 3 PAGES OF CODES POP UP TO BE FIXED.
The contact's wife owns a 2025 Chevrolet Equinox. While his wife was driving approximately 40 MPH, the message "Service Emergency calling Issue" was displayed. The driver pressed the car phone button on the steering wheel, but the message "Cannot Connect" was displayed. The vehicle was taken to the residence. The contact then pressed the OnStar button, but the system failed to connect, with the message "Cannot Connect" displayed. The vehicle was taken to a dealer, where it was diagnosed, and determined that the Telematic Control Module needed to be replaced. The manufacturer was notified of the failure. The failure mileage was 10,484.
Twice, while driving in falling snow away from home, the front wipers did not work properly when switched to LO or HI. They behaved as if set to intermittent wiping. The intermittent wiper setting worked properly to adjust the wipe interval. But setting to LO or HI did not cause the wipers to go constantly. As far as I could tell, the arm pivot area was clear enough for them to wipe. The problem went away on its own without restarting the car. It happened again a few days later when parked in a driveway.
On 3 separate occasions now, my vehicle has slammed on the brakes abruptly when there were no other cars around me in close proximity. The sensors vibrated the driver seat each time right before slamming brakes on, even though no other cars were even close to me. The sensors often go off when they shouldn’t be. The first time this happened, there were no cars ahead of me or beside me on the highway but there was one car behind me about 50 feet I’d say. Thankfully he was paying attention or he would have rear ended me. The 2nd time my husband was getting off an exit ramp so he was only going about 25mph and the car slammed the brakes on. There were no cars ahead of him or behind him. The 3rd time I was pulling out of a plaza entering a stop light and when I saw no cars coming, I started to pull out and it slammed on the brakes. There was a car behind me, but none beside me, and since I was turning left to approach the stop light, there were a few cars in each lane but they were about 20 feet away from me. None of these incidents should have activated the emergency braking. It has to be faulty sensors as they go off often when they shouldn’t. My husband and I both felt like we had bruised ribs and we thank God that nobody hit us, but at some point, we won’t be so lucky! And I am forced to drive this death trap because it’s not showing any codes for the dealership AND I still have to pay on my loan to drive an unsafe car!
The Equinox is detecting objects in the rear camera that are not there and automatically applying the automatic emergency breaking. This is causing the car to completely stop and has now caused me to injury my back due to the force of the stop. There is no warning given that the breaks are being applied. I was backing up at 7 miles an hour and the Equinox just stopped dead and there was absolutely nothing near the car. The car was in for an oil change and they said nothing was wrong with it. This latest incident of 4 now happened today and caused me to almost be hit in the parking lot of my child’s school while backing into a parking spot.
I purchased my 25 Equinox in December 2024 and immediately started having issues. I worked with multiple dealerships when the issues began and then reached out to GM for further help. After more than 6 months of GM giving me the run around, I've been left with an unsafe vehicle that even they don't want back. I had no intention of trading the vehicle within the first 3 years, but I don't feel safe in this vehicle. When I left the dealership the day I bought it, the vehicle drastically depreciated once I cleared their mailbox and to this point, GM hasn't offered any assistance with offsetting my trade difference. I also got 0% financing with GM that isn't currently being offered if I were willing to take the massive loss on my trade in. Here are just a few of the issues that I'm dealing with. The blind spot warning light stays on for no reason. Multiple updates have been done, and modules were changed and the issue remains. At night the warning light shines right in my face. I've tried adjusting the mirror completely outward and it's still an issue. I try avoiding driving at night all together because it makes it very unsafe. If I do have to drive in the dark, I have to use my arm to try blocking the light from my face, but my arm isn't big enough to block all of it. The entire dash has gone black leaving me with no speedometer or gauges. It did come back on, and the dealership ran updates. Most recently, while driving the car will slam on the brakes and put the red triangles on the windshield for no reason. I've had vehicles behind me that almost hit me because it is a drastic drop in speed. Like brake checking. Sometimes the adaptive cruise will start slowing me down like when a car is in front of you only there's nothing there. Dealership doesn't have a solution for these issues. I've also been places and come back out to the car to find that the rear hatch has opened on its own. The dealership ran updates to hopefully prevent this from reoccurring.
My 2025 Chevrolet Equinox (VIN: [XXX] ) was at Island Chevrolet from Thursday, June 26, 2025, through Monday, July 14, 2025, due to the transmission seizing. The vehicle was not even a year old at the time, as I purchased it in April 2025. Today, [XXX], my Equinox is at Freehold Buick because the OnStar hardware has failed. There is currently no estimated time of arrival for the replacement parts. The OnStar hardware controls critical vehicle functions, including diagnostics, automatic crash response, navigation, and remote commands such as remote start and door locking. A known issue with this OnStar failure is that the vehicle’s battery can drain as the system continuously attempts to reconnect. These types of major issues should not be occurring within the first year of ownership. At this time, there is still no date for when the OnStar hardware will be available. Transmission fluid leaking from the bolts
I am submitting this letter to formally notify the NHTSA of a safety critical defect in my 2025 Chevrolet Equinox Activ starting on 12/9/25 involving the Telematics Control Module (TCM), GM part number 85669062. This defect results in intermittent or complete failure of essential OnStar safety functions, including Automatic Crash Response, emergency communication, and accurate GPS location reporting. During normal driving, the vehicle experiences the following failures: •Loss of telematics connectivity •Inability of OnStar services to initialize or maintain a connection •GPS location drift or inaccurate positioning •Failure to reliably transmit vehicle location or contact emergency services These failures occur without user action and under normal operating conditions. The defect directly impairs systems intended to provide crash detection and emergency communication. If a collision occurs, the vehicle may not contact emergency services or may transmit an incorrect location. This represents a loss of a safety system that General Motors advertises as part of the vehicle’s crash response capability. On December 9, 2025, my Chevrolet dealer confirmed that the Telematics Control Module had failed and requires replacement. General Motors has placed this part on national backorder with no estimated availability, leaving the vehicle in an unsafe condition with no available remedy. General Motors has not issued a recall, service bulletin, or customer satisfaction program addressing this defect. The absence of available replacement parts suggests a potential supplier level issue affecting multiple vehicles. Given the safety implications, part unavailability, and potential for a broader pattern of failures, I respectfully request that NHTSA: •Review this defect as a potential systemic issue •Evaluate whether similar failures are occurring in other GM vehicles •Consider opening a Preliminary Evaluation (PE) if warranted. Thank you.
OnStar Module malfunctioned, has been diagnosed and awaits identification of specific type of module to determine if one is available or if it is one that will be on back order. The safety of myself and my family was put at risk, along with others, when vehicle braking system randomly activated and slammed on the brakes despite clear highways all around, almost causing us to be rear-ended more than once. This has happened prior to 12/01 several times randomly but we assumed the sensor was sensitive and was picking up something we couldn't see, however this was not the case in this situation. The battery keeps going dead and needing jumped and there are numerous warning messages/lamps. Dealer service department confirmed OnStar module failure and need for replacement, has not yet reproduced braking issues. Vehicle is currently at dealership service department for inspection and was deemed unsafe to drive at this time. Dealer stated they had no loaner vehicles to supply and instructed us GM would reimburse up to $45 daily for rental at Enterprise, so a rental vehicle was obtained and placed on waiting list at two different dealerships for loaner vehicle. WiFi and Navigation System originally quit working, then car battery started dying and requires being jumped multiple times if not driven within 8 hours, Received alerts off and on of "Lane Change Alert System Off", "Service Parking Break", "Adaptive Cruise Control Temp Unavailable", "Service Power Steering", "Service Electronic Stability Control", "Service Transmission Now, Unable to shift soon", and cruise control/Lane assist etc quit working. We became stranded in another town and had to purchase a portable jumper. Dealer currently has no anticipated repair date.
I am on 20 service calls.Mine is currently having onstar issues screens glitching, Apple CarPlay won’t connect, sensors on mirrors are going off nothing beside me, when I hit my breaks it keeps turning my emergency brake on and engine power reduced keeps coming on dash but the dealership is saying GM said my warranty is voided due to a aftermarket part
The windshield is a major safety issue. It is distorted, warped, and wavy. The dealership has ordered a replacement, but I’m worried it will be the same. As for the blind spot warning, it flashes when there is nothing present. The dealership cannot replicate, and I am not able to record when driving.
The car automatically brakes when nothing is in front of you and all of sudden it slams on its breaks. This is going to cause someone to rear end you. The steering is all over the road and does. It feels safe to drive. Transmission gets stuck and revs really high until it catches and they say nothing is wrong. Safety sensors go off all the time and nothing is near you. Key fob not detected, multiple awd issues
The vehicle’s display screen intermittently goes black, rendering the rearview camera inoperative. This failure significantly impairs rear visibility and creates a safety hazard while reversing
The vehicle’s heating system operates intermittently or takes an excessive amount of time to produce heat, reducing cabin visibility and comfort in cold conditions and creating a potential safety concern.
vehicle gives false blind side alerts on driver side. Happens when no objects are approaching. Happens when vehicles pass from opposite direction. System otherwise works as advertised.
Telematics unit in my Chevrolet equinox EV 2025 has failed. GPS/navigation/wifi does not work anymore. Calls to onStar for collision data / support in emergencies also do not work anymore. Also, basic features on the myChevrolet app to unlock/lock the car do not work. This happened gradually over time. Around 6 months after owning the vehicle, the GPS unit would sometimes randomly turn on/off and show my current location 1-2 miles away from my actual location. Then around 9 months after owning the vehicle, the unit as a whole failed and the whole tablet doesn’t work anymore. The dealer initially claimed that it was an issue with my onStar plan, but it is actually an issue with the telematics unit that needs to be replaced. This is a very well known problem is searched online across all newer GM vehicles that depend on Onstar.
You can't see clearly out the windshield while driving during the day or night due to dashboard reflection on the windshield. The defect is highly publicised on the internet and on Equinox EV forums. I was told it is an engineering defect due to the windshield's angle and cannot be fixed. I have asked Chevrolet to do a buyback because, even with the TSB fix for the dash cover, it doesn't fully address the safety issue. Chevrolet should not be selling vehicles with window glare that makes it impossible for the driver to see the driving field clearly 30-50% of the time, as it is for me when driving the Chevrolet Equinox EV. I don't feel safe.
The vehicle became completely disabled due to a sudden battery drain at approximately 5482 miles. I purchased the car with 2553 on it in May. The vehicle would not start a few times and had to be towed to the dealership on Friday Dec. 26, 2025. The dealer confirmed that the OnStar/telematics module is malfunctioning and continuously draining the battery. The defective module is on backorder with no estimated arrival date, leaving the vehicle unusable. They also do not know if they have a loaner for me. This creates a safety risk because the vehicle can become inoperable without warning, potentially stranding the driver. The issue has been reproduced and confirmed by the dealership. Prior to the failure, the OnStar system stopped functioning, but no warning lights appeared on the dashboard. My Chevy App stopped working 26 days ago as well. I now have a Loan payment to make for a car that isn't even working and brand new. Dealership doesn't know a timeframe either.
While driving in snowy conditions the headlights become packed with snow and slush. Lights become dim and I’ve had to pull over in unsafe conditions to clear them.
I bought this vehicle brand new on January 18th 2025 and it has been into the dealership 3 times now for a backup camera malfunction. My rear traffic alert or any of my other safety features when backing up are not working. On several occasions the entire instrument display including my speedometer and everything has completely blacked out.
My vehicle randomly turns off the foward collision system causing its supercruise system and adaptive cruise control to turn off while driving on the highway potentially causing and accident. I took it to the dealer in marlton NJ with the foward collision fault light on and they cant find anything wrong with the system as it resets itself after thr vehicle is turned off for two minutes. On our Facebook page I am not the only one who is experiencing this I will attach some documents of similar issues apparently GM has been aware of this for a while and no fix yet.
My Equinox EV telematics module does not work. Prevents the use of navigation features and Onstar navigation features.
This complaint is to clarify and supplement a previous report. The issues began in August 2025, not September. Initially, the vehicle presented abnormal fuel range fluctuations, including while stationary, as well as intermittent “Key Missing” warnings while driving and display black spots. These issues were reported to dealerships and GM without resolution. Over time, the condition has progressively worsened, and additional symptoms have developed, including abnormal noises and, more recently, transmission hesitation during the initial gears, requiring me to reduce speed for the vehicle to respond properly. The escalation of these issues over time raises ongoing safety concerns.
I am the owner of a 2025 Chevrolet Equinox EV and have experienced ongoing and unresolved issues with the vehicle’s connectivity systems — including OnStar, internet access, GPS functionality, and over-the-air (OTA) updates. The problem first began in late July 2025, when I suddenly lost both internet connectivity and GPS navigation. During that episode, I was unable to use the OnStar system or navigation tools. Although the issue self-corrected after a few days, the instability raised concerns. The issue returned later in the year, and I visited my local Chevrolet dealer on November 24th, 2025, where a reset was attempted. While temporarily effective, the problem recurred. A follow-up service appointment on December 8th, 2025, confirmed that the Telematics Communication Control Module had failed — the same hardware component that controls OnStar access, vehicle geo-location, navigation, and software updates. At that time, I was informed by the service advisor that several other EVs were experiencing the same issue and that the replacement part was on national backorder, with some customers waiting more than two months. I was not given a clear estimate of when the part would arrive. This issue has effectively disabled: All OnStar connectivity (a paid subscription service), The vehicle’s ability to receive manufacturer-recommended OTA software updates, GPS navigation and location services, including vehicle tracking — a feature relied upon by my insurance company. Given the widespread nature of this issue, the lack of available parts, and the critical role of these systems in vehicle safety and functionality, I believe this constitutes a serious safety and consumer-protection concern.
Check engine light was on after just sitting in the snow for two days. Dealer is saying that it is a fuel injector and something with the gas. I hope this fixes it.
My car threw on the check engine light and a note flashed on the screen and car just slowed down immediately. I had to press on the brakes really hard to get it home and then the transmission wouldn’t go in reverse. I took it to the dealer and they couldn’t duplicate the problem but said the battery contacts weren’t tight.
My daughter was driving and she said the check engine light started blinking and the car slowed down immediately. She had to press the gas to get it into a gas station.
- The telematics module is bad, resulting in vehicle having no connectivity (GPS, entertaintment system, updates) - The car has no other means of hands-free operation for navigation software (no CarPlay or similar phone projection technology) - GM is unable or unwilling to supply the parts for warranty repair. - It is illegal to use the phone while operating vehicle in my state (Florida)
Telematics Module failure. Vehicle is no longer able to provide GPS location. This is rendered all safety features and OnStar features for accident detection useless. Dealer states parts are on backorder with no ETA. Calls the GM result in no estimated ETA or part availability.
The vehicle has ongoing and worsening electronic and safety-related issues. The fuel range and fuel economy fluctuate abnormally and frequently, sometimes even while the vehicle is stopped. The infotainment screen has experienced intermittent black areas/partial blackout while driving, affecting system reliability. Recently, the automatic transmission began hesitating during the initial gears, behaving abnormally and requiring me to reduce speed for the vehicle to respond properly. This occurs consistently. These issues have been reported multiple times to the dealership and GM with no resolution, and communication has stopped. I am concerned the vehicle is unsafe to drive and could fail unexpectedly.
No component failures at this time. My dealer stated that the continued on/off of my check engine light is that of a " GM engineering flaw/ issue" without any remedy at this time. I have been advised to continue to drive the vehicle with check engine light on as no remedy is available at this time. The flaw was disclosed by GM in 4/2025, bought my vehicle in 6/25 - No remedy as of 12/12/2025 - This is more of an FYI than formal complaint.
The contact owns a 2025 Chevrolet Equinox. The contact stated that while depressing the accelerator pedal from a stop, the power steering became inoperable. Several warning lights were illuminated. A dealer was contacted. The vehicle was towed to a dealer, where it was diagnosed, and determined that the front camera had failed and caused the power steering failure. The vehicle was not repaired. The manufacturer was notified of the failure and informed the contact to speak with management and to file a claim. The approximate failure mileage was 8,300.
I placed my foot on the brake to start my vehicle and when I pushed the start button my brake pedal went to the floor and warning came up stating brake failure ect. I contacted the dealership and was told to drive it to them so the could put it on the scope and tell me if it is safe to drive. I did and then was told it was a master brake cylinder failure and not safe to drive. In researching this is a know failure by gm service notice N252494220
Telematic Module System is malfunctions with a nationwide recall on the part. Chevy representatives have no date for when the part will be in. The computer system in the car is not working. I can't reach OnStar the internet I pay for doesn't work. My car has been in the service department for a month this year for the same problem and it's out again. Engineers from Chevy have worked on it as well. I need help with this situation. It's possible that the this failure could drain my battery causing it stop while driving. The service department and Chevy customer service just keep apologizing with no real dates on when this part will be in.
While driving today at about 21 mph, when a warning came on that my car had brake assist failure. It said the speed would be limited and to break hard to enable the car to stop.. I was able to get to a parking lot safely, but my car did not break at the stoplight and went straight through. Luckily there was no one coming.. my car is at the dealership now and they are finding no issue and nothing showed up on my Chevy app. From the articles I have read it said this issue is something that happens on cars that do not have super drive. I drive a 2005 Chevy equinox RS with super Dr. so clearly the brake problem is happening on this vehicle as well.. it took all my strength to get the vehicle to stop.. I called OnStar and was going to have it towed and turned the car off. Once the car came back on all of the warning lights were gone, and then the car drove fine.. I got lucky today, but this is an issue that still needs to be addressed.. The issue was supposed to be taken care of in 2024. I just purchased my 2005 in September and I have 3100 miles on the car.. I was driving in a rural area with many hills and was scared to death. I had to keep my foot all the way on the brakes the entire time to keep myself at a slow speed..
On November 30, 2025, while driving my 2025 Chevy Equinox EV LS, the vehicle displayed a “Service High Voltage System” warning message on the center screen indicating an issue with the vehicle’s high‑voltage electrical system and that some driving and/or charging functionality may not be available. The high‑voltage battery and related components are still in the vehicle and available for inspection upon request. This fault could cause sudden loss or limitation of driving or charging capability, which may increase the risk of being stranded in traffic or in unsafe locations. Beginning in the first week of November 2025, the myChevrolet app has continuously shown a “Lithium‑Ion Battery – Schedule Service Now” alert under Diagnostics, even though no corresponding warning appeared in the vehicle until November 30. I presented the in‑app lithium‑ion battery warning to the Chevrolet dealer earlier in November; they declined to act on it, stated they do not rely on the app, and asked me to reschedule and return later, so the issue has not yet been reproduced or confirmed by a dealer or independent shop. The vehicle has not been inspected by the manufacturer, police, insurance, or others specifically for this high‑voltage/battery issue. The only prior symptoms were the persistent lithium‑ion battery service notification in the myChevrolet app since early November and, as of November 30, the in‑vehicle “Service High Voltage System” warning message.
The headlights are recessed. When driving in the snow they catch the snow and block the light from coming out. I was driving home last night in a snow storm and couldn’t see very far ahead of me because my headlights were packed full of snow. The LED lights don’t produce heat to melt the snow, and there’s nowhere for the snow to fall out from so the snow and road grime just keeps getting packed into the recessed light. Very poor design.
The dashboard causes a serious glare that prevents visibility. GM has only offered a cheap dash mat as a fix and my local dealer says they don’t know anything about it. They should really replace the dash pad with the updated model and not just lay a piece of cheap carpet over the dashboard of my 50k dollar car. The attached photo dosent even do the problem justice. Due to the way camera lenses work it looks less severe in photos.
Our OnStar Control Module on our 2025 Equinox started having connectivity issues. It failed to access apps and showed our location 100 miles from where we actually were, and drains the battery. We could not access OnStar to correct the issue. It continued to go off and on, until it completely failed. Researching this, hundreds of 2025 Equinox owners are experiencing the same issues. It is an extreme safety hazard as affected Control modules would send emergency personnel to an incorrect location in the driver is incapacitated in an accident. The module is constantly on back order for weeks. This needs to be looked at
There is a button on the top of the steering wheel that puts the car in low gear. We have been going down the interstate twice and my husband accidentally hit the button slowing down the car immediately. This is very dangerous and could cause a rear end collision. We have reported it to Chevrolet several times, to the service department twice and the corporation also. Low gear has normally been on the gear shift with all the other gears, but for some reason, they put it on the top left side of the steering wheel. That is a very strange place to have that since the steering wheel also has buttons for cruise control, Bluetooth, stereo, etc. We think this button needs to be disengaged and put else where, but Chevrolet offered us $150 for the inconvenience. We are not concerned with convenience, but with safety. Also, the vertical buttons on the dash below the navigation screen are not visible during the day. At night, when it is dark, the light illuminate the buttons so they can be seen, but they are not visible during the day. However, you do need to be able to see the buttons. My husband used a label maker to put the names on the buttons just to prevent a wreck, but Chevrolet needs to address the issue. We have mentioned this to the service department and corporate office, but they do no seem to be concerned. We consider both items to be an issue that needs to be corrected, but no one seems to care. Hope you can help get these items addressed before something serious happens. If you nee to contact us, our names are [XXX] and [XXX] , [XXX] . I drive 20 miles each way to work every day. Much of this drive is on highways, and currently the bridge connecting Ohio and Kentucky is down to three lanes with no berm. On a windy road with low visibility and a steep hill near my home, the car completely lost power and acceleration without any warning. I wasn't able to drive it completely off the road before it came to a stop and due to the loss of power was unable to move into neutral to push it and the hazards would not work. Thank God that this didn't take place on the bridge or with my family in the car. I made the decision to exit the car while it was still on the road and find a safe area in the grass where I could call for help. When I exited the vehicle, it locked me out and the only way to get in was through the driver's side door, which would put me back on the road in traffic and at risk. The electrical failure prevented me from opening the other doors. The car's complete system failure put my safety/life at risk and could jeopardize the lives of others if it wasn't investigated. To make matters worse when the tow truck came via OnStar they weren't equipped to move a car stuck in park or drive due to the electronic gear shift so they drug it with the tires locked onto the bed. After several days of testing the electrical system, the dealership found a loose wire and informed me that it had been fixed. I picked up the car and after a mile or so, the check engine light came on. I contacted the dealership to come pick it up and that's where it stands at the moment. My intention was to pass this on to my boys when they start driving, but this experience will alter that plan.
The contact owns a 2025 Chevrolet Equinox EV. The contact stated that while driving at various speeds and pushing down the turn signal lever, the lever rose back unintendedly, causing the turn signal to unintendedly disengage. There was no warning light illuminated. The contact stated that the failure was intermittent. The vehicle was taken to the dealer; however, the failure could not be duplicated. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 100.
The contact owns a 2025 Chevrolet Equinox. The contact stated that while reversing, the transmission remained in drive. The contact depressed the shifter to move. The contact stated that the gear shifter was sticking. The vehicle jerked, hesitated, and then stalled. The contact stated that the doors were locking and unlocking without driver input after an update was performed, and the keys became locked inside the vehicle. The service transmission warning light was illuminated. The telematics module, GPS, and OnStar were not working as designed. The contact stated that the OnStar went into red and lost connectivity and was unable to locate the vehicle. The contact was concerned about driving the vehicle. The vehicle was taken to the local dealer, where it was diagnosed, and it was determined after further inspection that the battery was drained due to the telematics module draining the battery. The dealer informed the contact that parts were on back order. The vehicle was not repaired. The manufacturer was contacted and opened a case. The approximate failure mileage was 28,000.
check engine light keeps coming on for emissions system. When I take it to Bergstrom Dealership they tell me they are not sure how to fix the problem.
The dashboard on my 2025 Chevrolet Equinox EV RS reflects strongly on the windshield, creating glare that severely reduces forward visibility in daylight. The reflection makes it difficult to see the road clearly and poses a serious safety hazard. I have taken the vehicle to the local dealers various times for this issue, and they acknowledge the problem but have not repaired it, saying they are waiting for a part. The visibility issue remains unresolved and appears to be a design defect that could affect other vehicles of the same model. It poses a serous risk of accident and injury to pedestrians.
There is EXTREME glare from the black dashboard reflecting back up onto the windshield in partly sunny to sunny conditions. The glare effectively blocks my vision and I've now had TWO (2) instances, in parking lots, where pedestrians were near my vehicle and my vision was blocked - almost causing me to hit / run over these pedestrians. I've contacted GM and the Dealer (Loyalty Chevrolet Williamsburg , VA) for over three months trying to get this resolved. They contend they've issued a "safety fix" involving a new dash mat to reduce glare, but the parts are not available / on backorder
On Th, Nov 6 ~1700, the 25 Equinox EV being driven encountered an electric drive failure and brake failure, including friction brakes. This Equinox EV is essentially still brand new with 3900 miles and no accident history. The vehicle was driven off the EB SR 528 exit ramp to Narcoossee Rd in Orlando and driven to the stop light at the WB on ramp. After the light changed and began driving, the vehicle console reported an exception with drive assist. At this time the driver attempted to slow down to take stock of the situation and the vehicle accelerated, at which time the driver took their foot off the brake pedal and attempted to identify a place to take the vehicle off the roadways. Thankfully a stoplight at McCoy Rd was green and the driver continued through that light and while attempting to brake, the vehicle accelerated instead. The driver then coasted, maneuvering the vehicle was into the Vista Palms commercial center. The vehicle, unresponsive to braking, propelled through a stop sign into the Kohl's parking lot at which time the driver began looping around the lot until the weight of the vehicle brought the motion down. While the vehicle was stopped, it rotated through warning messages indicating..forward collision, brake assist is reduced, steering assist is reduced, then finally Service Transmission Now Unable to Shift Soon. The vehicle was towed by AAA to AutoNation Chevrolet Airport in Orlando. Technicians found codes U1610 and U1639, and found K160 connecter unseated. It was reseated and greased. The concern is that this should have been locked and unable to be unseated. Is this an indication of manufacturing problem or operating problem? All braking was dysfunctional. How can we be certain that this was not a one off event?
I was driving my 2025 Chevrolet Equinox EV on the highway with Adaptive Cruise Control (ACC) engaged. The vehicle was also set to "One-Pedal Driving" (High regenerative braking). The vehicle in front of me slowed down abruptly. My vehicle's ACC began to slow the car, but it was not decelerating quickly enough to avoid a collision. I immediately intervened by pressing the brake pedal as hard as I could, pushing it completely to the floor. Critical Safety Failure: The vehicle's physical brakes failed to engage. Despite my foot being pressed hard to the floor, the vehicle did not slow down. It felt as though only the regenerative braking was (slowly) working and that my manual brake application had zero effect. The vehicle continued to glide forward with no significant braking power, and I was unable to stop the car in time, resulting in a collision with the rear of the vehicle ahead. Summary of Defect: The primary safety failure was the complete lack of response from the manual brake pedal. My attempt to override the vehicle's automated systems in an emergency was unsuccessful, as the brake pedal was unresponsive. Reporting History: I have already reported this incident to my auto insurance, my local Chevrolet dealership, and the Chevrolet Customer Assistance Center. A representative from Chevrolet recommended that I file this report with the NHTSA to help identify and track similar safety-critical incidents.
Unknown. The roof is separating along the groove line causing rust on my 2025 Chevy Equinox
One time (so far) the screen behind the steering wheel that shows the gear and speedometer did not turn on when we started the car. We were able to back the car out of the garage and park it in our driveway. This is a safety concern because we could have driven the car in that condition - without a working speedometer. We are worried that the screen will turn off while we are driving.
when I started my car there was a very loud vibratory sound that caused my car to shake. When I accelarted the noise quite down but it was still there. When I stopped in traffic the sound reappeared. This occured for 2 hours ths length of my commute. When I made it to my destination. I lifted the hood. Idk what i was looking for. this morning the trembling is now light but the noise was not.
Our 2025 Equinox EV was leased new in March of 2025. Unknowingly, it had a water intrusion in the front passenger floor due to a manufacturing defect that GM is aware of. This is related to TSB 25-NA-149. The water intrusion was covered up and went unnoticed for some time due to the factory GM All-Weather floor liners. Once discovered due to the musky smell in the vehicle, black mold was found after pulling up the floor liner. The vehicle was inspected by a Chevrolet dealer, who confirmed it was due to a poor body weld that has now been resealed. Replacement carpet is on backorder due to many experiencing this defect. Our safety and the safety of others is put at risk since GM is not recalling the vehicle for this defect. We have been unknowingly driving around breathing in black mold. As someone who is immunocompromised with an autoimmune disease, this is especially troubling to discover. My autoimmune disease was sucessfully managed with medication since 2017, however, shortly after leasing the vehicle in March I began to experience flare ups. I have been unable to determine why, but this discovery makes me believe it was due to the mold in the vehicle. From discussing with my doctor, mold exposure is a potential trigger of my condition.
The contact owns a 2025 Chevrolet Equinox. The contact stated that the vehicle was experiencing rough idling and sputtering at start-up. The check engine warning light was illuminated. The vehicle was taken to Auto Zone, where it was diagnosed with rough idling and fuel injector failure. The dealer was notified of the failure. The vehicle was not repaired. The failure mileage was approximately 8,000.
The contact owns a 2025 Chevrolet Equinox. The contact stated that while stopped at a traffic sign, the accelerator pedal was depressed; however, the vehicle stalled. The contact stated that there was no warning light illuminated. The contact stated that the transmission automatically shifted to park(P). The vehicle was towed to a service station and the vehicle was refilled. The contact was able to start the vehicle, and the message "Auto Shut Down" was displayed and a 5-minute countdown was started, and the vehicle turned off. The contact was able to restart the vehicle and drove to the residence. The contact had not taken the vehicle to a local dealer or independent mechanic. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 13,000.
Returning home on I-40 on Sunday, the engine just quit working. Fortunately it WAS Sunday and the traffic was light. Had there been more traffic, someone probably would have hit me from the rear. We managed to get it almost all of the way off the road before it stopped. According to the dealership that worked on it, it was the fuel supply service module, $46.09 part. The service technician said they had been having a lot of problems like this and knew what to do. All of the code were lost because to tow truck driving had to take the leads off of the battery. That was the only way he could get the car out of park in order to tow the vehicle.
When the vehicle was stopped in an intersection, the vehicle would shift from D to P by itself. The check engine lamp was subsequently lit. I would not able to shift it out of P until I restart the vehicle several times. During such incidents, I was stuck in the middle of the intersection for a few minutes, impeding traffics and endangering myself and other motorists on the road. This problem first happened on September 20, 2024. The vehicle's odometer was at 1824 miles. The dealer made a repair. This problem happened again on October 14, 2025. The odometer was at 22065 miles. The vehicle is current still in the dealership. The vehicle was normal before such incidents. There was no warning light or any other noticeable anomaly before such incidents.
This car is equipped with a telematics module that feeds information to navigation system, Blue Cruise hands-off driving, and provides speed limit information in a heads-up display based on map data. The same module also controls connectivity to On-Star, a safety monitoring service. There appears to be a bug in the software whereby the module doesn't appropriately initialized properly. This has several effects. The OnStar system ceases to function (and the attendant feature of calling emergency services in a collision). The bug also causes false GPS coordinate data to be fed to the car; since the reporting of the speed limit and other alerts are based on this data, along with the Blue Cruise hands-off driving, the driver is presented with false information about the speed limit on the current road (instead using data about the road at the false GPS coordinates). If using guidance, the guidance presents the driver with false and confusing directions (which change frequently as the system believes the driver is in a different place). This bug has happened about 6 times in the past 6 months. There's no mechanism to reset the unit other than disconnecting the battery or fuse. The attached file shows a typical event. The guidance clearly shows the car in the middle of the Charles River in Boston, and the navigation system is instructing the driver to get on Memorial Drive. The HUD (not shown) indicates that the current speed limit is 20 MPH. This picture was taken while on Interstate 93 in Medford MA (speed limit 65 MPH).
on 10/9/2025 I pulled out of my driveway and less than 300 feet from my house I struck a deer. Weather was approx 50 degree and clear. There was no dew on my windows as I park my car in the garage. There was approximately $6200 worth of damage done to my vehicle and what concerned me the most was, the car never engaged the emergency stop. I mentioned this to the insurance company and they really had nothing to say. I have had bicyclists get too close to me on the road and my vehicle engaged the emergency stop. After repairs were done on the vehicle, they had a third party come in to do a calibration of all the sensors. He could not perform the calibration due to #PIT6274: Error Message Referring to Vision Camera system with DTC B1C48 - (Nov 4, 2024). I just dropped my vehicle off at the dealer and mentioned this to them and they are claiming there is nothing they can do about it. I started looking around online and have seen several other people complain about this and I knew it affected my Adaptive Cruise Control as I have gotten this error a couple times before and I just thought my window was dirty. What I did not know is this also affected my emergency stop as it does not tell me that in the warning. If this is a known issue, this is a serious safety concern and to have the dealer tell me there is nothing they can do is not acceptable. If the camera was functioning properly that morning, there is a good chance that I would not have gotten into an accident.
Visibility under certain conditions is very poor. Reflection on the windshield from the dash creates unsafe driving conditions. It is especially bad when driving into a dark area like a parking garage or car wash. Almost impossible to see other vehicles or people. Polarized, sunglasses, eliminates or reduces the reflection but they are not always within reach. This is not an isolated incident. It is a constant problem with the vehicle design.
When driving in sunny conditions, there is an intense glare reflecting from the interior trim- specifically the "chrome" trim on the middle console. The reflection is so bright that it becomes blinding at times, making it difficult to see the road clearly and creating a serious safety hazard. This issue occurs consistently whenever sunlight enters the cabin and reflects off the trim's surface. It is extremely distracting and has the potential to cause temporary vision impairment when driving.
When there is dew, condensation, or rain on the windshield, the Front End Collision Warning system becomes inactive and displays the message: "Front End Collision System Inactive-Clean Windshield." This happens frequently, even with light moisture, and the warning system remains disabled until the windshield dries completely. Given that wet or low-visibility conditions are precisely when collision avoidance features are most critical, this failure creates a serious safety risk. I have verified that the windshield is clean and that three are no obstructions on the camera or sensors. Despite this, the system consistently deactivates whenever moisture is present. This issue appears to be a design or sensor calibration problem, not a maintenance issue.
When using the factory-installed wireless charging pad, my phone becomes extremely hot within minutes of charging. The heat builds quickly, to the point that the phone must be removed immediately to prevent potential damages or safety risks. This has happened consistently, and I have not removed the phone case because I should not have to- the charging pad is designed and advertised to work safely under normal use conditions, which includes phones in standard protective cases. Regardless, the level of heat generated is excessive and concerning. This issue raises serious safety and design concerns, including the potential for battery damage, device malfunction, or even fire risk.
My vehicle has been at the dealership for over a month for an issue with the HVAC system. The technician and GM engineering support believe the problem is related to a faulty acuator, which requires the entire dash to be removed for repair. (See pictures) This is extremely concerning for a new vehicle with very low mileage. An HVAC acuator failure in a brand new car should not occur, and the repair is invasive- involving disassembly of the major interior components. I am worried this will lead to creaks, rattles, or compromised fit and finish afterward, as well as potential damage to electronics, airbags, and trim panels. In addition, this problem appears to be widespread among other 2025 Equinox owners, based on multiple reports in owner forums describing identical HVAC noise or clicking problems linked to acuators. GM has not yet issued a recall or service bulletin addressing it. Given the scope of disassembly required and the frequency of similar reports, this should be reviewed as potential design or component defect affecting vehicle reliability and safety systems (defrosting performance and interior air management).
Telematics module failure. Causes GPS to provide erroneous navigation prior to complete failure. Started as 1/2 mile, then grew to over 30 mile deviation from actual location. The module also takes out several safety and monitoring systems monitored by OnStar. All in-car communications are lost, which means driver has to revert to using a cell phone directly for basic functions proved through infotainment system.
Electrically, the time will not stay set at the correct time.Nor will the date. Other issue is there will be a loud roaring sound when driving. This happens frequently, there are no codes that pop up. No, one is sure what the loud roaring is.
The contact owns a 2025 Chevrolet Equinox. While the contact's wife was driving at an undisclosed speed, there was an abnormally loud sound coming from the vehicle, and the front sunroof panel fractured. The contact's wife had described the loud sound as like a loud gunshot. The contact stated that the sunroof glass had fractured without impact to the sunroof. The vehicle was taken to the dealer, where the contact was informed that the sunroof needed to be replaced. The vehicle was in the process of being repaired. The manufacturer was made aware of the failure but provided no assistance. The contact was advised to contact the NHTSA Hotline to report the failure. The failure mileage was approximately 13,500.
When driving with the sun in front of me, the reflections from the front dash on the windshield makes visibility very poor.
OnStar telematics unit stopped working with no notification in the infotainment system. The LED light remained GREEN in color advising me that the OnStar system was working. Hundreds of people reporting the same issue across multiple GM vehicles. My vehicle has been at the dealership for almost two weeks and there is no ETA on when the replacement part will be received from GM. This is a known problem component that should have initiated a safety recall. Due to the non operation of the OnStar system, we no longer have access to the emergency notification if involved a traffic collision. We also no longer have access to the stolen vehicle tracking system. NHTSA needs to investigate this issue, especially since GM is taking their time with getting replacement parts to dealerships.
My vehicle seemed to accelerate when pulling into a parking spot at my work. It then launched over the curb sending me rolling down the hillside several feet. I required extraction and the car is now totaled. I am available to explain more and share pictures at your convince.
The contact owns a 2025 Chevrolet Equinox. The contact stated that while driving approximately 65 MPH, the vehicle stalled unexpectedly. There was no warning light illuminated. The contact stated that the accelerator pedal was depressed and traveled to the floorboard. The vehicle was able to be restarted. The vehicle was taken to a dealer, but was not diagnosed or repaired. The manufacturer was not made aware of the failure. The approximate failure mileage was 6,500.
My windshield has had distortion since I bought it (11/27/2024) and I finally took it to the dealership to get it replaced (09/30/2025). After replacing it they notify me that the new windshield also came with distortion and that unfortunately there is nothing they can do until GM (General Motors) puts something out about it. They said they compared it to the other vehicles and they're all like that but it's not as "bad" as it use to be. The fact that there is no fix and I just have to deal with a brand-new car that causes headaches from how much I have to focus my eyes to be able to see is ridiculous. I don't understand how they're just ok with letting all their customers drive with glares in their windshields and all they can say is "well there's nothing we can do" it's completely unsafe.
Over-torqued lugs, broken wheel studs, safety issue
The vehicle had a fault in its GPS and it also had no communication with emergency assistance. This also caused battery problems that generated on two occasions that the vehicle did not start
The contact owns a 2025 Chevrolet Equinox. The contact stated that during a heatwave, the vehicle was parked, and the contact became aware of a "S" shape crack starting from the driver’s side upper windshield. The contact observed multiple reports with other consumers with identical cracks. The vehicle was repaired by a mobile technician. The manufacturer was notified of the failure and a complaint was filed. The contact stated that while looking out of the passenger’s side windshield, the view was distorted. The vehicle was not diagnosed or repaired. The failure mileage was 21,000.
Vehicle was being parked in garage when it accelerated in reverse, on it's own, very quickly. Fortunately I was able to avoid colliding with any thing behind me by braking in time. I then shifted into drive when vehicle accelerated forward, on it's own again, very quickly. I was not able to brake before vehicle crashed into concrete garage wall, severely damaging front end. Vehicle is still under warranty, and I intend to have dealership haul it in for repairs. I have found other drivers complain on the Internet of experiencing the exact same issue as I described above. This model, Chevrolet, Equinox, 2025 is not safe to drive!
Odometer Fraud. The contact purchased a 2025 Chevrolet Equinox. The contact stated that the vehicle was purchased at 66 miles. The contact stated that while driving the vehicle and approaching 100 miles; the odometer rolled over to 1,000 miles. The vehicle was taken back to the dealer where the vehicle was inspected and the contact was informed that based on the oil life, the vehicle had been driven over 1,000 miles. The vehicle was taken to several other dealers who came to the same conclusion. The vehicle was a dealer sale. At the time of purchase, the vehicle mileage was 66; it was later discovered that the mileage was over 1,000.
Vehicle rolls back very often when in drive. Even holding down the brake at a hill then hitting the accelerator it still rolls back. When in cruise control and I take over it also tends to make the vehicle jump when the vehicle takes back over. Overall harsh riding. Feels like it’s all coming from that CVT. Dealer says that it may be an issue but can’t replicate it unless it stays there with them for a while until they can replicate the issue. There have been incidents where I’ve almost rolled back and hit other vehicles due to this cause. The vehicle has not been inspected by anyone. No warning lamps or anything on the vehicle
The contact owns a 2025 Chevrolet Equinox. The contact stated that while depressing the brake pedal, the engine revved to a higher RPM unexpectedly. The contact stated that the failure had occurred on several occasions, and the brake pedal needed to be depressed very hard while attempting to stop the vehicle. The contact stated the vehicle had started independently two to four times in a row. No warning light was illuminated. A dealer was contacted. The vehicle was taken to the dealer to be diagnosed; however, the diagnostic result was unknown. The vehicle was not repaired. The manufacturer was not notified of the failure. The approximate failure mileage was 14,417.
The contact owns a 2025 Chevrolet Equinox. The contact stated that while driving approximately 35 MPH, the vehicle independently activated the brakes. The contact stated that the vehicle stopped unintendedly. There was no warning light illuminated. The vehicle was taken to a local dealer. The dealer was unable to detect any fault codes and was unable to determine the cause of the failure. The vehicle was not repaired. The contact stated that the failure was intermittent and had first occurred in a parking lot while driving at approximately 10 MPH during the failure. The manufacturer was not informed of the failure. The failure mileage was approximately 11,000.
The contact owns a 2025 Chevrolet Equinox. The contact stated that after starting the vehicle, the check engine warning light illuminated. In addition, while driving at various speeds, the check engine warning light illuminated. The vehicle was taken to the dealer. The vehicle was not diagnosed or repaired. The contact stated that the failure persisted. The manufacturer was not notified of the failure. The failure mileage was unknown.
2025 Chevy Equinox - beginning of July, i got back to my car at the airport and it would not turn on. nothing happened and i had to call onstar with my cell phone. a tow truck was sent and my 2025 Equinox (which i bought end of Oct 2024) with only 8000 miles, was towed to Chevy dealership. the next day the dealership said it was a faulty battery and they replaced it. i went to pick up my car and then a couple weeks later i tried to autostart my car and it did not respond. I called Onstar from my phone to talk with them and then they asked me to go to my car and push the button. it was green however it was not responding. i was told to take it into the dealership and tell them i am having an onstar issue and it wasn't responding. I took my car 3 weeks ago (Aug 11th) sat for 4 hours waiting on my car only to be told that they were not able to fix it. that my entire onstar system needs to be replaced. they said they ordered it but that it was back ordered and do not know when it will be in. and to call in 2 weeks to see if there were any changes. i called 3 weeks later and still they had nothing new to tell me except that they don't know anything. from reading online i see that this onstar module issue had contributed to other issues with the car. i knew that there was an issue that triggered the battery to completely die. and with all the other people that have said they have had issues , it is very apparent that this caused my battery to die. Now i am still paying for onstar but not able to use it. i am obviously paying my payment on my car without being able to use all functions of the car. I am at a total loss on what to do next.
Bought vehicle in October 2024. About 5 hours after we purchased vehicle the check engine light came on, dealership did reset and everything checked out to be safe and sent us on our way. Fast forward to January (do not remember date) check engine light came on again, this time took it to auto zone to hook the car up to the scanner, it did give an error code of P04FB which is a crankcase ventilation sensor. From auto zone we did take it back to the dealership and the had the same code as auto zone. Dealer ship did reset and acknowledged that Chevy was “working on it” but no fix is available. Everything seemed to be ok until 6/5/2025 and the another time in June when the check engine light came back on and again the dealership reset it. Since June we have had the vehicle into the dealership once a month, July (not sure of date) and 3 times in August 8/26 8/27 & 8/29. The check engine light came back on and now we have to take it back to the dealership 9/2/2025. This is super frustrating and an inconvenience to have to take a brand new vehicle to the dealership every day. We have had the vehicle into the dealership a total of 8 times so far with a 9th visit on 9/2/2025 for the SAME issue when will chevy get this figured out?
they have the gear shift stick near the wheel like older models. you have to brake to move the gear shift and push a button in on the gear shift to park. However, when driving you can move the gear shift, which I have done accidentally. The gear shift does not lock while driving. Twice the gear switched to L2, which I do not know what that is but it causes my engine to over work. This is dangerous especially trying to switch the gear back to D when at a speed of over 50mph.
I leased this vehicle brand new. At around 500 miles, the vehicle had occasionally a humming noice, at 1900 miles the humming noice is loud with vibration. My suspicion is a structural defect, my fear is to end up with half a vehicle on one lane, the other half in the other lane at the expressway. I have recorded several of the occurrences, provided a GM dealership with the recordings, contacted GM Executive offices, and have not been able to resolve or advance, meanwhile my household is without a vehicle. Neither GM nor the dealership is willing to provide a courtesy vehicle. The dealership was unable to duplicate my complaint, GM executive office agent recommended me to take the vehicle to a different dealership for a "second opinion". This is depriving me of work, transportation and giving me additional work.
8 speed transmission will not shift to 8th gear at highway speeds. It will shift at a lower speed which makes merging onto an interstate dangerous. The vehicle gets better fuel mileage if it will use 8th gear. For it to use 8th gear on the interstate you must merge onto the road at a much slower speed than posted speed limit and then very slowly accelerate to posted speed limit which puts driver at greater risk of an accident.
The onstar module installed originally from manufacturer was faulty, causing electrical issues. This ended up draining the battery unexpectedly. No warning that the battery was low. Only had .03 volts left when tested at dealership. The onstar button was experiencing issues such as blinking and loss of connection. Onstar Support was no help, and the car manufacturer ignored issues as my phones software problem, until new battery was drained and issues persisted while driving. Ultimately this left the OnStar not working for weeks leading up to the drained battery which was unsafe in case of accident as well.
Telematics module stopped working out of nowhere and drains my battery to my vehicle. Had to have the vehicle towed to nearby dealship. Huge safety hazard as it is essentially the brains to my computer. App doesn't work without it, battery doesn't work and can't even get inside vehicle with key fob. Please issue recall.
Factory installed module for ONSTAR went out.
I purchased my 2025 chevy equinox in January of 25. In March I noticed some distortion on the passenger side of my window while making a right turn. I have had the windshield replaced twice by chevy/gm. Both replacements have the same issue. The dealership pulled up a new 25 or 26 equinox to find it has the same issue. The local chevy/gm rep claims, even without seeing my vehicle, that this is with tolerances/spec. This is a safety issue as it distorts my vision.
Vehicle may be involved in GM Service Update N252494220 , Reduced Brake Performance. I cannot get a response from Chevrolet if the vehicle is part of the brake problem issue. Based on the information provided, this should be escalated to a recall. I also have not been able to get an answer from the selling dealer. Per the information in the Service Update bulletin, the vehicle was not to be sold until it was inspected for the problem.
FIRST INCIDENT: On uncertain date between 10/10/2024 (vehicle purchase date) and 04/22/2024 (initial service date), on a Sunday, 6:30 a.m., Infotainment and dash display went blank/dark on Sawyer County Highway B. Recovery within 30 seconds. Dark conditions, operating at recommended speed of 55 MPH. Fortunately no surrounding traffic at the time. Issue brought to attention of Don Johnson Motors Service Department at initial oil/filter/tire rotation on 04/22/2025. Mileage at time was 3287. SECOND INCIDENT: 07/15/2025 at approximately 9:30 a.m. on Sawyer County Highway N, Infotainment and dash display went blank/dark. Recovery within 15-20 seconds. Operating at recommended speed of 55 MPH. Fortunately no surrounding traffic at the time. Mileage at time of incident just over 7200. Have not contacted above dealership, but will do so shortly. This defect renders the vehicle out of the driver's control similar to a pilot losing engine power on an aircraft. Checked online to locate possible sources for the problem. Determined this is a known issue.
We purchased the vehicle New. Within approx. 300 miles started to hear a bearing noise intermittently while driving. We returned to the dealership to have it diagnosed. We were advised that the could not duplicate it. A few days later the noise was brominate while driving at highway speeds. Returned to the dealer. We were told that it was caused by the Active Noise Cancelling system and that they performed an update(per GM service bulletin) to the control module to correct it. Picked up the vehicle. Driving around town no issue. Driving at highway speeds the noise would come and go. On June 14, 2025. while driving on the highway at 70 MPH the engine shut down. My wife and daughter were in the vehicle in the middle lane of a busy highway. The dash displayed "Automatic Braking Engaged! WHILE TRYING TO GET OFF THE ROAD. The vehicle brought itself to a COMPLETE STOP. On The HIGHWAY and Engaged the Parking Brake Automatically. This is unbelievable and extremely Dangerous. How or why did some engineer think this was good idea for the vehicle to Lock Itself down ON THE ROAD!
The contact owns a 2025 Chevrolet Equinox. The contact stated that while driving at various speeds, a yellow check engine warning light illuminated. The vehicle was taken to the dealer where it was diagnosed, and contact was provided DTC: P04FB (Crankcase Ventilation Systems Pressure Sensor Issue). The contact was informed that the Manufacturer's engineers were working on an improved solution. The vehicle was not repaired. The dealer cleared the code. The contact stated that the failure persisted. The manufacturer was not made aware of the failure. The failure mileage was approximately 5,000.
Yellow check engine light comes on and off. Car isn’t 1 yr old yet im being told there is no recall yet or a fix.
Sunroof is throwing a code that there is an obstruction. Sunroof will not close. Chevrolet could not fix. I was sent home with out any answers and was told they would contact me. That was over a week ago.
The contact owns a 2025 Chevrolet Equinox. The contact stated that upon attempting to park their vehicle, or attempting to shift into reverse, neutral, or into drive, the shifter becomes extremely difficult to put into gear, and a message will display on the instrument panel that stated "Conditions not correct for shift", and all of the warning lights were illuminated. The vehicle was taken to a local dealer where the vehicle was diagnosed and the contact was advised that the valve body and would need to be replaced. The vehicle was repaired however the failure persisted. The contact noted that this is the second occurrence of their vehicle needing to have the valve body replaced. The manufacturer was notified of the failure. The approximate failure mileage was 870.
When activating the blinker to make a right turn, after completing the turn the blinker turns off, but the force causes the lever to drop far enough that the left blinker activates. Since I've had the vehicle (about 8 months total), I notice this happen more than half the time I use the right blinker. From a quick Google search, this seems like a common problem which may lead to confusion when driving. The dealership I went to doesn't seem to have a fix for it.
On May 8,2025 the lift gate failed and crashed down on my head, vehicle is available for inspection and only a GM inspector has looked at it so far. The dealership told me because I filed a complaint with GM they can't touch it unless directed by GM now. It almost knocked me out, I saw black and was hard to see for at first within a few hours it was a huge goose egg on my head. It put me and my 6yr old at risk as she was with me but luckily in the car already. When approaching my car I used the liftgate remote to open it ahead of time, got my daughter in car and went to put our backpacks in the back when it crashed on my head full force no warning. It was extremely painful and caused a concussion with multiple symptoms. Headaches, painful bump, ear ringing, feeling so hot, major brain fog, forgetfulness, watery eyes, feeling really tired and hard to wake up in the morning. It has been about 3 weeks and the symptoms are mostly gone except bad ear ringing, light sensativity, on and off headaches easily, can't put my hair in ponytail as it hurts still. I still have a small dent in my head. The doc says these lasting concussion symptoms could last for months. I don't understand why this has problem has not been fixed when research shows it has been a problem since 2010. The only other issue I had with the car was my instrument panel going black while I was driving a few weeks prior.
The contact owns a 2025 Chevrolet Equinox. The contact stated while reversing at an undisclosed speed, the vehicle shuddered and stalled. The contact stated that the failure had occurred on several occasions. The contact stated that the check engine warning light was intermittently illuminated. The vehicle was taken to a dealer to be diagnosed, and the contact became aware that transmission fluid was leaking from the bottom of the vehicle and the transmission needed to be replaced. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 8,000.
The windshield has a defect on the right hand side. It seems like blurry waves. I noticed it because I got nervous thinking that there were cars riding next to me very close but it is just a glare from the defect.
Steering feels slightly loose and keeps drifting to the left. When I have people in the backseat and I take off from a stop, the vehicle swerves to the left. Hoping the dealership will do an alignment when I take it in.
My new 2025 Equinox RS AWD ran out of gas entering a 55 MPH highway. I tried to roll to the shoulder, and I came to a stop half on the right shoulder and half in the lane. When a 2025 Equinox RS AWD won't run, in this case because it is out of gas, there is no way to shift the car out of Park and into Neutral so it could be rolled out off the road, or on to a Tow truck ramp. When a 2025 Equinox RS AWD isn't running, it's locked in Park, and in order to move it needs to be jacked up and set on wheel dollies, for all four wheels.
Backup camera not working and the display on dash shows triangle and a camera crossed out. Dealer has been advised 1 month ago. The Camera is supposed to be on backorder.
The steering wheel seizes at highway speeds after driving for an extended period (2+ hours.) The steering seems to lock into a position so while approaching a curve you're unable to steer toward the center of the lane, requiring you to jerk the steering wheel towards the center of the lane to prevent going out of the lane. This issue was reported and taken to multiple GM dealerships to address and they keep insisting it's Lane keep assist, it is not as this issue happens when it's turned of, further more lane keep assist usually nudges a vehicle back in lane or vibrates to warn you're drifting out of a lane. It does not actively try to pull you out of a lane as We've experienced with this car. This is a deadly hazard that may cause an inattentive drive to over steer into an accident, and because of it's intermittent nature, It will be surprising and scary causing less experienced drivers to overreact to it. I'm afraid this may be a latent safety fault within the 2025 Chevy Equinox, that will eventually cost lives, and worst part, this cause may not be detected for a long time.. I hope more instances of this gets reported to force GM and it's dealerships to spend the proper effort to hunt down this safety issue and address it before it's too late
Center screen went black and is completely inoperational. Cannot utilize any center screen functions including headlights and rear backup camera. Took vehicle to dealer and they diagnosed as a bad display control unit, which is on backorder.
The car vibrates hard, like being on the rumble strip, but only while driving about 1.7 rpm to 2 rpm and only sometimes. The transmission light came on. The dealer said it was leaking and fixed it. A few days later, the vibrating is back. It's at the dealer again. Still don't know what's wrong. I only have 4,500 miles on the car so far.
The contact owns a 2025 Chevrolet Equinox. The contact stated that while attempting to use the vehicle, there was an automatic update alert. The contact's wife's cellphone had alerted her to the update; however, she was unable to access the vehicle for 15 minutes. The contact stated that while attempting to reverse, the transmission failed to respond as needed. The contact stated that the reverse symbol was displayed on the instrument panel, and there was an abnormal clunking sound coming from the vehicle. The local dealer was contacted, and informed the contact that the automatic update was normal. The vehicle was not diagnosed or repaired. The manufacturer was not contacted. The failure mileage was approximately 999.
While driving, all my displays which includes speed and car warning indicators, went blank and the red light (collision warning) started flashing as if there was something in front of my car, but my car acted fine. I pulled over and then everything slowly came back on. The car continued to run just fine. It was just both displays went totally black and then the red collision warning lights were flashing. Car never turned off. Never had to restart car. No resets performed. Displays came back on after a short period of time. Interesting, the radio and volume had to be turned back on. Acted as if displays rebooted, but did not affect car mobility.
The contact owns a 2025 Chevrolet Equinox. The contact stated that while driving 20 MPH, the transmission unexpectedly shifted into neutral(N). No warning light was illuminated. The contact was able to shift back to drive(D); however, the vehicle idled while driving. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The manufacturer was not notified of the failure. The failure mileage was approximately 1,060.
WIRELESS CHARGING OVERHEATS - CAUSES BURNS POTENTIAL PHONE DAMAGE
While driving my 2025 Chevrolet Equinox, both the digital instrument cluster and infotainment screen suddenly turned off without warning. This blackout lasted for several seconds before both systems rebooted and returned to normal. This has happened multiple times and does not seem related to any specific driving condition, speed, or weather. The vehicle itself continues to operate normally during the blackout, but the sudden loss of all visual information — including speed, alerts, and navigation — poses a serious safety risk. No warning lights or error messages appeared before or during the incident. This issue started shortly after receiving an over-the-air software update labeled GM N25-F172AE. The problem has not yet been confirmed or resolved by a dealer, and the vehicle has not been inspected by police or insurance.
The contact owns a 2025 Chevrolet Equinox. The contact stated while driving at various speeds, the vehicle made an abnormal sound. The vehicle was taken to a nearby dealer, where the rear differential oil was drained, and metal shavings were found in the oil. The dealer determined that the rear end assembly needed to be replaced; however, the part was not available. The dealer had ordered the part. The contact stated that the vehicle had been at the dealer for repairs since January 10, 2025. The vehicle was not repaired. The manufacturer was notified of the failure and recently sent an email, advising the contact that the part was not yet available to repair the vehicle. The failure mileage was approximately 1,600.
Inappropriately activates forward emergency braking without a vehicle ahead or obstruction ahead. Inappropriately activates reverse emergency braking without a vehicle or obstruction behind. Inappropriately activating forward collision alert without vehicle or obstruction ahead. Gauge cluster display goes black while vehicle is in motion. Hypersensitivity of the left blind spot monitor. Activates without a vehicle or obstruction within the blind spot area. Causes distracting strobing during dark hours. Dealership claims to be unable to reproduce any of these issues. Claims there is no software update available to address these specific concerns nor is there a software update available in general for the vehicle. The unexpected emergency braking has left me stuck in the middle of a normally busy intersection as well as blocking another two lane road when my gauge cluster also turned off with a emergency braking incident and I could not change gears and did not know the parking brake engaged as well due to the display being off as well.
Windshield has cracked on 12/18/24. Chevrolet and General Motors have provided no update on windshield availability which is now a safety concern as the crack continues to spread
On two separate occasions, the dash and radio restarted while we were driving. Whole driving down the highway, the speakers stopped working, both screens would go blank and the red pre collision lights would turn on. This causes the inability to see your speedometer or other required information while driving. I am unsure if this affects any other safety equipment from functioning if there was a crash during this timeframe. This lasted for several seconds while the system essentially restarted itself. Once everything turned back on, everything worked as it should. It went into the dealer for the first time it did this and they did a software update to prevent this but it did not apparently fix the issue.
1. Windshield has distortion on passenger side. 2. Vehicle is available for inspection upon request. Distortion in windshield causes blurriness to driver and could cause a collision/accident due to driver having visual restrictions. 3. Vehicle was taken to dealer and dealer confirmed the distortion, contacted GM, who also confirmed distortion. 4. vehicle has been inspected by Ourisman Chevrolet of Rockville, MD. They sent videos/pictures to GM and GM acknowledged distortion/defect in windshield. 5. The car was on show room floor and not test driven prior to purchase. Purchaser/Owner did not notice until following day during daylight hours.
I bought the vehicle new and the next day, I noticed a distortion on the passenger and drivers side windshield. When I drive it makes me nauseous and dizzy. I took it to the dealership and they said the windshield is on backorder. It is not safe to drive!
The screen for the radio goes blank and then the dash goes blank this happend a few times randomly but this time it was longer and then it told me there was no key found in car while driving and I was at a light the car in front moved forward and I started and went forward and my car braked unexpectedly I adjusted the gap settings but I don’t believe this should have happened the car seemed far
There are 2 big and long distortions in the windshield that effect the view. Viewing out of the windshield is hindered by causing objects to look distorted and different than reality. There is one on the right and left side of the windshield effecting the view of both sides.
Both the speedometer/digital information center and radio went black while driving. I had no access to either of these for about 5-7 seconds. I had cruise control set at the time, and it did not affect my speed. I heard a “click” after everything went black and after restarting, it was normal. My red indicator above the steering wheel flashed a couple times and the radio came on first, then the speedometer. It would be available for inspection upon request. This put both mine and others’ safety at risk. I had no way to see my speed, cruise setting, or access any of my tools that uses either this or the radio to function. This has not happened again, I have reported the issue to the dealership the next day with no contact back. It has not been inspected, and there were no warning lamps or indication of the problem before it happened.
Infotainment and Main Driver Information Center blank. While driving on highway Infotainments screen went blank followed by the Main Driver Information Center. Problem only lasted for 2 seconds before screens reappeared.
The Instrument Cluster and the Infotainment Display blanked out for about 3-5 seconds while driving one evening. I'm not certain of the time interval. Just noticed both went blank, then returned.
Two major complaints this Vechile #1 The current std operating control features are a flat out Safety Issue NO-ONE has taken into account the display of to name a few such as Fan-Speed Hi/Low, AC On/Off ,Heated Seat adjustment you cannot View while driving during daytime hours or late afternoon 1-2 hrs before dark But you can view 100% only when the Sun is directly behind you ( THEY DO NOT LITE UP UNTIL ITS NIGHT TIME & ONLY AT NIGHT) You might be laughing well zip it up ( WHY IT FORCES THE DRIVER TO TAKE THEIR EYES OFF THE ROAD) it’s not funny Gone is Std button or knob you can reach and adj .Trying do certain adjustments I find myself wandering off the highway onto Rumble strips searching for control of certain p.b. #2 This car while Traveling on [XXX] threw PA into Ohio with only 500+ miles we come up on quick stop Traffic back up ( TRYING TO QUICKLY TURN ON THE HAZARD WARNING SW —-NOT FOUND )2-days latter still have not found I should not have to be searching the I/Net or reading some now small portion of a new idea shortened version of a operators manual Basic functions we all should be able to Operate any Brand Vehicle Not this Car this [XXX] design Control Panel is a Ralph Nader example of how to Distract every driver behind the wheel. Also a lot of overall reviews many give this car great reviews But No One Takes Ease of Std Operator Functions into Account of any Safety Factor and I don’t trust you or any Government Agency will be telling GM this design of Std Control access eventually will cause many accidents . Oh yes everyone makes a big deal about one talking on a cell phone that is an Ticket in waiting pales as a Joke to just plain driving this Vehicle ( IT IS A VERY DISTRACTING NIGHTMARE ) thanks [XXX] Camden NY Cell [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
Was driving down the road with a sudden loss of power to the infotainment system plus dashboard resulting in all black and being unable to see your speed/turn signals. The steering wheel lights suddenly flashed then shut off and won’t turn back on.
After I engage the right turn signal and complete a right turn, it occasionally bounces the turn signal back too hard and then the "soft" (not fully clicked in) and lights up my left signal about three times. This has happened multiple times since we leased it in early September.
The blue line shows 170 total NHTSA complaints filed over 3 years, with the worst year being 2025 (103 complaints, including 4 crash reports).
The amber reliability line is declining — recent years score around 10/100, lower than earlier in the vehicle's life. This pattern indicates issues are accumulating over time rather than being resolved, which is a concern for used buyers.
Notably, despite 170 complaints on file, no recall has been issued — either the defect pattern has not met NHTSA's threshold for a formal campaign, or a recall may be forthcoming.
Red dots on the amber line flag 2 years (2025, 2026) where complaints included crash events — these are the highest-severity incidents in the dataset.
Estimated 5-year cost of ownership: $31,344 (~$522/mo). Repair risk buffer is standard due to vehicle risk profile.
20% estimated recall probability — some indicators present but not alarming.
- ▸Accelerating complaint rate — increasing pressure on NHTSA to act
Maintenance intervals vary significantly by make, model, year, engine, and driving conditions. Always follow the manufacturer's official schedule — not generic industry estimates.
Tip: Request the seller's service records and compare against the manufacturer schedule. Missing intervals are a negotiation point.
