Recent Safety Recalls — Toyota Sienna (2022–2026)
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022 Sienna vehicles equipped with seating for eight passengers. An incorrect seat belt webbing guide may have been installed on the outboard second-row seat belt assemblies.
Remedy: Dealers will replace the outboard second-row seat belt assemblies, free of charge. Owner notification letters were mailed January 10, 2022. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are 21TB08 and 21TA08.
Toyota Engineering & Manufacturing (Toyota) is recalling certain 2022 Lexus LX 600, Lexus NX 350h, Lexus NX 450h+, Toytoa RAV4 Hybrid, 2021-2022 Lexus LS 500h, Toyota Mirai, RAV4 PRIME, Sienna, Venza, and 2020-2022 Toyota Highlander Hybrid vehicles. The Skid Control ECU software may not turn on the Vehicle Stability Control (VSC) system when the vehicle is restarted after the driver has disabled the VSC system. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 126, "Electronic Stability Control Systems."
Remedy: Dealers will update the Skid Control ECU software, free of charge. Owner notification letters were mailed May 27, 2022. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is 22TA03. Lexus' number for this recall is 22LA01.
Vantage Mobility, LLC (VM) is recalling certain 2021-2026 Honda Odyssey, 2021-2025 Chrysler Pacifica, and Toyota Sienna vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. The retractors may not lock, preventing the wheelchair from being properly secured.
Remedy: Dealers will inspect and replace the retractors, free of charge. Owner notification letters was mailed January 27, 2026. Owners may contact Vantage Mobility customer service at 1-800-488-9082.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2021-2025 Sienna Hybrid vehicles. The bolts for the seatbacks on the third-row seat assemblies may not be tightened properly. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 202, "Head Restraints," 207, "Seating Systems," and 225, "Child Restraint Anchorage Systems."
Remedy: Dealers will tighten the seat assembly bolts, free of charge. Owner notification letters were mailed April 1, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's number for this recall is 25TA05.
Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.
Remedy: Rollx will work with Q'Straint to inspect and replace the retractors as necessary, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Rollx's customer service at 1-800-956-6668.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2025 Sienna Hybrid vehicles. The second-row seat rails may be welded improperly.
Remedy: Dealers will replace the second-row seat rails, free of charge. Interim letters notifying owners of the safety risk were mailed December 4, 2025. Additional letters will be sent once the final remedy is available, anticipated in Q3 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

The complaint was filed on behalf of the contact's employer. The contact drives a 2023 Toyota Sienna Hybrid. The contact stated that while driving at 5 MPH, the vehicle independently accelerated. The contact stated that the acceleration occurred while the contact was making a right turn. The contact stated that the vehicle shifted from hybrid to high speed within a second. The brake pedal was depressed, and the vehicle failed to stop as intended. The vehicle crashed into another vehicle before coming to a complete stop. The vehicle sustained damage to the front driver's side door. No warning lights were illuminated. No Police report was filed. No injuries were sustained. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The contact referenced an unknown recall which had a similar failure description, however the VIN was not associated. The manufacturer was not made aware of the failure. The failure mileage was approximately 12,000. VIN unavailable.
The contact owns a 2024 Toyota Sienna Hybrid. The contact stated that while the vehicle was at a complete stop, while attempting to depress the brake pedal, the vehicle unexpectedly lunged forward. There was no warning light illuminated. The contact stated that the failure had occurred several times without a warning light illuminated. The vehicle was taken to the dealer several times; however, the failure could not be duplicated. The vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 31,600.
Due to safety recall 25TA12 anyone sitting in the second row has an increased risk of injury during a collision. This remedy has taken far too long to implement. The reason for the purchase included passenger capacity and safety. My use is now limited. Toyota should have already remedied this situation.

Toyota issued a recall in fourth quarter 2025 for this batch of Toyota Siennas that have bad welds on the second row seats and as a result of these, no one should be sitting in the second row. Toyota still doesn't have a fix in place for the 50k vehicles with this problem. Publicly Toyota says a fix is on the way, but at the dealership the service department has indicated that they've heard nothing on this. I am in a loaner from Toyota for the foreseeable future and it doesn't seem like I have much recouse through lemon laws based on the way Toyota issued the recall.
Rear windshield shattered between 4/16/2026, 10:04 AM and 4/16/2026, 6:43 PM without reason, no one near it to cause this broken windshield. I have Simplisafe camera that showed no one was near the car. See attached pictures.
2023 Toyota Sienna occasionally, without warning, "lurches" forward at low speeds when braking. While putting steady pressure on the brake when coming to a stop at a stop sign or traffic light etc. it feels as if the brakes stop applying pressure and if we wouldn't have quick reflexes to brake harder, we would roll out into the road or into the car in front of us. I believe it is something to do with the transition between regenerative braking (vehicle is a hybrid) and the hydraulic braking. I have spoken to Toyota, and they don't have any recall. People posting on online forums seem to believe there is a software update (T-SB-0047-24) that helps address the concern. Toyota charges to diagnose and apply software updates as the vehicle is out of warranty. I believe this to be a safety issue and something Toyota should cover. Based on my online search, there seems to be a lot of people with the same issue. This has been an ongoing issue the last couple of years and I'm thinking it should be reported. I have not brought this to a dealer to have diagnosed. These are largely family vehicles and my wife, and I don't feel very comfortable every time it happens.
I am filing a complaint regarding an unresolved safety recall for my 2025 Toyota Sienna involving the second-row seat welds. VIN: [XXX] NHTSA Recall ID: 25V668 Recall Date: October 7, 2025 Manufacturer/Dealer Recall Reference: 25TB12 This recall involves the structural integrity of the second-row seat welds. A failure of these welds could increase the risk of injury to occupants in the event of a crash or sudden stop. As of today, more than six months after the recall was issued, no remedy has been made available. I have not been provided with a repair, a timeline for when a fix will be available, or an adequate interim solution. On February 25, 2026, I brought my vehicle to the dealership, and it was placed out of service due to this recall. I was provided with a smaller loaner vehicle, which is not comparable to my Toyota Sienna and does not meet my transportation needs. This situation has left me without use of my vehicle for an extended period of time while also dealing with a known safety defect. I request that NHTSA review whether the delay in providing a remedy is reasonable and compliant with federal safety requirements, and whether additional action is necessary to protect vehicle occupants. See attached document for complaint. My husband and I purchased a Sienna, 7-passenger van, April 2025 and have received two safety recalls, the first being the lack of second-row seats being installed correctly presenting a safety problem, and later a possible problem with the backup camera. We purchased a vehicle of that size especially for the convenience of transporting family when they visit. Also, we have a granddaughter, husband and one-year old great-grandson who live nearby and thought having a van would be helpful for trips together. Unfortunately, all our hopes were diminished at being able to enjoy the reason we purchased such an expensive vehicle, $65,000+. We are an elderly, retired couple, considered the vehicle an investment, because our two previous Toyota vehicles had lasted us for 15 years with no problems. Therefore hoping our daughter and grandchildren would be able to use the van for many years. The van has been nothing but an Albatross, not being able to drive it with only two passengers, in the front, since learning of the first recall on the second row seats. The local dealer told me if I’d get an appointment and be at their door at 7:00 a.m. they could repair the camera. BUT, dealers have no fix from Toyota for the second row seats. However, I went on line to the Toyota web site and they have a “FIX” process listed. I cannot imagine how a vehicle maker can be allowed to sell a vehicle when they find out it has such safety hazards? We consumers can beg our Legislators to pass laws to that effect, but, hope your agency will take the lead on protecting us consumers since we know this Congress will do nothing. I ask your agency to please take action on this issue and prod Toyota to do something concerning the major safety hazards with their 2025 model Sienna vans.
